BackgroundSince 2015, a major economic crisis in Brazil has led to increasing poverty and the implementation of long-term fiscal austerity measures that will substantially reduce expenditure on social welfare programmes as a percentage of the country’s GDP over the next 20 years. The Bolsa Família Programme (BFP)—one of the largest conditional cash transfer programmes in the world—and the nationwide primary healthcare strategy (Estratégia Saúde da Família [ESF]) are affected by fiscal austerity, despite being among the policy interventions with the strongest estimated impact on child mortality in the country. We investigated how reduced coverage of the BFP and ESF—compared to an alternative scenario where the level of social protection under these programmes is maintained—may affect the under-five mortality rate (U5MR) and socioeconomic inequalities in child health in the country until 2030, the end date of the Sustainable Development Goals.Methods and findingsWe developed and validated a microsimulation model, creating a synthetic cohort of all 5,507 Brazilian municipalities for the period 2017–2030. This model was based on the longitudinal dataset and effect estimates from a previously published study that evaluated the effects of poverty, the BFP, and the ESF on child health. We forecast the economic crisis and the effect of reductions in BFP and ESF coverage due to current fiscal austerity on the U5MR, and compared this scenario with a scenario where these programmes maintain the levels of social protection by increasing or decreasing with the size of Brazil’s vulnerable populations (policy response scenarios). We used fixed effects multivariate regression models including BFP and ESF coverage and accounting for secular trends, demographic and socioeconomic changes, and programme duration effects. With the maintenance of the levels of social protection provided by the BFP and ESF, in the most likely economic crisis scenario the U5MR is expected to be 8.57% (95% CI: 6.88%–10.24%) lower in 2030 than under fiscal austerity—a cumulative 19,732 (95% CI: 10,207–29,285) averted under-five deaths between 2017 and 2030. U5MRs from diarrhoea, malnutrition, and lower respiratory tract infections are projected to be 39.3% (95% CI: 36.9%–41.8%), 35.8% (95% CI: 31.5%–39.9%), and 8.5% (95% CI: 4.1%–12.0%) lower, respectively, in 2030 under the maintenance of BFP and ESF coverage, with 123,549 fewer under-five hospitalisations from all causes over the study period. Reduced coverage of the BFP and ESF will also disproportionately affect U5MR in the most vulnerable areas, with the U5MR in the poorest quintile of municipalities expected to be 11.0% (95% CI: 8.0%–13.8%) lower in 2030 under the maintenance of BFP and ESF levels of social protection than under fiscal austerity, compared to no difference in the richest quintile. Resumo Desde 2017, o sistema de saúde brasileiro enfrenta uma onda de contrarreformas em políticas que ampliaram cobertura e acesso e pretenderam mudar o modelo assistencial. A Atenção Primária à Saúde (APS) tem sido atingida por atos oficiais, sinérgicos e complementares, que a alteram substancialmente. A criação de serviço social autônomo federal para provisão de médicos; a consulta pública para instituir cesta de serviços; a flexibilização da carga horária semanal de médicos e enfermeiros, que poderão integrar mais de uma equipe; a não fixação de quantitativo mínimo de agentes comunitários de saúde por equipe; formam o contexto em que o Programa Previne Brasil foi lançado para vigorar em 2020. O argumento do governo é de aumento de flexibilidade e autonomia local para organizar serviços, maior eficiência e valorização do desempenho. Críticas de sanitaristas, de alguns conselhos estaduais de gestores municipais e do Conselho Nacional de Saúde apontam o caráter privatizante, mercadológico, seletivo e focalizado da proposta que caminha na direção da cobertura universal em saúde. Este artigo analisa o Programa Previne Brasil que altera o financiamento e sugere aumento de recursos para APS numa conjuntura de congelamento do gasto social. Identificam-se contradições e alternativas.
As despesas com planos de saúde correspondem a uma parcela importante dos gastos privados com saúde no Brasil. Este estudo teve como objetivo descrever a evolução dos gastos com planos de saúde das famílias brasileiras, segundo sua renda. Foram utilizados dados das Pesquisas de Orçamentos Familiares (POF) de 2002-2003 e 2008-2009. Para a comparação dos valores dos gastos entre as POF, foi feita correção mediante o Índice de Preços ao Consumidor Amplo (IPCA). A proporção de famílias que tiveram gastos com planos de saúde permaneceu estável, nas duas POF estudadas (2002-2003 e 2008-2009), em torno de 24%. Todavia, o valor do gasto das famílias com planos de saúde aumentou. Entre aquelas que tiveram gasto com planos de saúde, o valor médio elevou-se de R$ 154,35 para R$ 183,97. O gasto médio com planos de saúde foi maior à medida que aumentava a renda das famílias, assim como as parcelas da renda e da despesa comprometidas com esses gastos. O gasto com planos de saúde está concentrado entre as famílias com maior renda. Para estas, os planos de saúde foram o principal componente do gasto total com saúde.
In 1988, Brazil became one of the first countries in Latin America to frame access to health care as a constitutional right. However, it would be misleading to call Brazil's Unified Health System (Sistema Único de Saúde, or SUS) a public health system that provides universal access and comprehensive care. This paper reveals a strong contradiction between the re-distribution model set out in the Brazilian Constitution and the inadequate level of public spending on health care. The law states that health care is a basic social right, allocated by need rather than means. Meanwhile, in 2003, Brazil spent US$ 597 per capita on health, or 7.6 per cent of its gross domestic product (GDP), while the average country from the Organization for Economic Cooperation and Development (OECD) spent US$ 3145, or 10.8 per cent, and Argentina spent US$ 1067, or 8.9 per cent of its GDP.
A hipótese central do trabalho afirma que o mercado de planos de saúde se expandiu no Brasil contando com o apoio do padrão de financiamento público mediante a aplicação de um conjunto variado de incentivos governamentais. Os procedimentos metodológicos adotados para investigar esta hipótese se apoiaram no estudo de parte da produção teórica que ilumina a área da economia política da saúde e na descrição de determinadas ações do Estado no campo das políticas de saúde, que acabaram patrocinando o crescimento dos planos e seguros privados de saúde nos últimos quarenta anos.
