Integralidade e Atenção Primária à Saúde: avaliação sob a ótica dos usuáriosThe comprehensive nature of primary health care: evaluation from the user standpoint
RESUMO CONTEXTO:Os transtornos mentais comuns (TMC) abrangem sintomas de fadiga, esquecimento, irritabilidade, dificuldades de concentração, queixas somáticas e sentimento de inutilidade. Altas prevalências destes agravos têm sido observadas em adultos no contexto da Atenção Primária, sendo consideradas uma das maiores problemáticas de saúde no mundo. OBJETIVO: Estimar a prevalência e fatores associados aos TMC em adultos assistidos por uma equipe da Estratégia Saúde da Família no Brasil. MÉTODOS:Estudo transversal e analítico, realizado no ano 2016, com uma amostra aleatória de 297 adultos. Para coleta dos dados, utilizou-se o questionário Self-Reporting Questionnaire (SRQ-20), além de informações sociodemográficas, sobre condições de vida e saúde. Para presença de TMC, considerou-se ponto de corte o escore igual ou maior que 7 para homens e mulheres. Testaram-se as associações dos TMC com as variáveis de interesse, por meio de análise univariada (qui-quadrado), com nível de significância de 5%. RESULTADOS: A prevalência dos TMC foi de 23,2%. A prevalência do desfecho foi significativamente maior nos adultos do sexo feminino, em união estável, com 0 a 4 anos de estudos, que faziam uso de medicamento controlado, e que informaram diagnóstico prévio/confirmado de TMC pessoal ou na família. CONCLUSÕES: Os resultados apresentados evidenciam que as características associadas são passíveis de serem investigadas e acompanhadas na prática clínica. Destaca-se a necessidade de qualificação dos profissionais da Atenção Primária à Saúde, visando à detecção e acolhimento das pessoas com TMC, com enfoque em uma assistência integral e que envolva a família. Objective: To evaluate the incidents spontaneously notifi ed in a general hospital in Minas Gerais. Method: Retrospective, descriptive, quantitative study performed at a general hospital in Montes Claros -Minas Gerais State. The sample comprised 1,316 incidents reported from 2011 to 2014. The data were submitted to descriptive statistical analysis using Statistical Package for the Social Sciences version 18.0. Results: The prevalence of incidents was 33.8 per 1,000 hospitalizations, with an increase during the investigation period and higher frequency in hospitalization units, emergency room and surgical center. These occurred mostly with adult clients and relative to the medication supply chain. The main causes were noncompliance with routines/protocols, necessitating changes in routines and training. Conclusion: There was a considerable prevalence of incidents and increase in notifi cations during the period investigated, which requires the attention of managers and hospital staff. Oral squamous cell carcinoma (OSCC) is the most frequent oral malignant neoplasia. As consequence of OSCC treatment, oral mucositis (OM) is one of the most common adverse effects of OSCC treatment. Currently, there is no consensus for OM treatment. The purpose of the current study was to test the combination of red and infrared low-level laser therapy (LLLT) for OM treatment. Primary culture of human fibroblast was performed to identify LLLT dose. After laboratory tests, a two-arm parallel, single-blind, controlled study was conducted. The two arms were group 1, both 660- and 808-nm wavelengths (300 J/cm, 9 J of total energy, 100 mW, spot size 3 mm), and group 2, only 660-nm wavelength (300 J/cm, 9 J of total energy, 100 mW, spot size 3 mm). Both treatments were performed twice a week. Group 1 presented a reduction of mucositis grade in comparison to group 2. Group 1 also presented reduction of analgesics prescription. But no significant differences between groups 1 and 2 were observed according to the pain scale. In conclusion, the current study demonstrated that a combination of red and infrared at a higher dose (300 J/cm) reduced both oral mucositis grade and analgesics prescription. The effects of the combination of RT and LLLT are unclear and need more studies.
Inserido no universo do cuidado de enfermagem à pessoa idosa, o estudo descreve o perfil sociodemográfico e verifica o uso de medicação segundo gênero em idosos cadastrados em um PSF. Método: Estudo quantitativo, descritivo, transversal com pesquisa de campo. Utilizou-se o questionário semiestruturado, possibilitando identificar o perfil desses usuários, as principais morbidades, os medicamentos utilizados e as dificuldades relatadas por eles quanto ao uso dos medicamentos. O universo foi de 211 idosos. Na análise dos dados utilizou-se o programa SPSS, priorizando a associação estatística de forma analítica. Resultados: Entre os idosos em uso de medicamentos, verificou-se predominância feminina, renda familiar e escolaridade baixas, idosos vivendo com familiares e sem dificuldades no uso de medicamentos. Uso de medicamentos foi comum em mais de 90% dos idosos. Verificou-se associação para "ausência de dificuldades no uso do medicamento" e "orientações do PSF" (P<0,001). Conclusão: Constatou-se que as mulheres encontram-se em situações menos privilegiadas.Palavras-chave: Idoso. Uso de Medicamentos. Atenção Primária à Saúde. Enfermagem Geriátrica. Abstract ResumenInserido en el universo del cuidado de enfermería al anciano, el estudio describe el perfil sociodemográfico y verifica el uso de medicación según género en acianos asistidos en una unidad del Programa de Salud Familiar. Método: Estudio cuantitativo, descriptivo y transversal con trabajo de campo. Fue utilizado cuestionario seme-estructurado, posibilitando identificar el perfil de los usuarios, las principales morbilidades, los medicamentos utilizados y las dificultades relatadas por ellos en relación al uso de los medicamentos. En el análisis de los dados, fue utilizado el programa SPSS, priorizando la asociación estadística de forma analítica. Resultados: Entre los ancianos con uso de medicamentos, fue verificado predominancia femenina, renda familiar y escolaridad bajas, ancianos viviendo con los familiares y sin dificultades en uso de los medicamentos. El uso de medicamento fue común en más de 90% de los ancianos. Fue verificado asociación para "ausencia de dificultades en uso de medicamento" y "orientaciones del Programa de Salud Familiar" (p<0,001). Conclusión: Fue constatado que mujeres están en situación menos privilegiada.
Objective: to identify the functional capacity of older people based on their performance on basic activities of daily living. Method: a cross-sectional study, carried out through the record of the Family Health teams working in the urban area of a city in northern Minas Gerais, in 2015. To assess the functional capacity of 373 older people, the Katz Scale was applied, which includes dimensions on the performance of daily living activities. The interviews took place in the participants' households. Results: of the total participants, 6.9% had some degree of dependence. Those aged over 80 years old and were living without a partner had about three times more chances of presenting higher levels of functional dependence. Conclusion: most older people enrolled in a Family Health Strategy had their functional capacity preserved. ResumoOs acidentes domésticos infantis são importante fonte de preocupação, pois são causas crescentes de mortalidade e invalidez no Brasil. O objetivo deste estudo foi identificar e descrever os acidentes domiciliares na infância na área AbstractThe childhood home accidents are an important source of concern, as they are growing causes of mortality and disability in Brazil. The aim of this study was to identify and describe childhood home accidents of a Health Community Agent Strategy (EACS) in Montes Claros, Minas Gerais, Brazil. This is a descriptive, retrospective, quantitative study accomplished through a structured questionnaire applied with parents or guardians of children aged between zero and 12 years in the home environment. The study included fifty parents or guardians. One hundred and four household accidents were recorded. Nineteen children (38.0%) were male and 31 (62.0%) were female. Regarding age, six accidents (12.0%) occurred among children aged from three to eight months, 15 accidents (30.0%) involved children from eight months to less than four-years, 11 accidents (22.0%) occurred in children from four to less than eight years and 18 accidents (36.0%) occurred with children from eight to 12 years. The study shows that the parents' presence does not prevent accidents from occurring, since in 97 (93.3%) accidents the children were accompanied by the parents. The types of accidents registered were electric shock, impact, cuts, slips, poisoning, airway obstruction, fall and burn. Among these, falls were the most common. It was concluded that children are very vulnerable to accidents and that the home environment may involve risks or safety.
Objective: To verify the prevalence of common mental disorders and associated factors in nursing technicians. Method: Cross-sectional analytical study conducted at a university hospital. A questionnaire with sociodemographic, educational and work-related variables and the Self Reporting Questionnaire-20 were used to verify the presence of common mental disorders. Association analysis was performed using the chi-squared test, with a significance level of 5%. Results: 280 nursing technicians participated in the study. The prevalence of common mental disorders was 46.9%. The variables family income (p=0.021) and working exclusively in health area (p<0.001) were associated with the outcome. There was a higher prevalence of CMD among individuals with a family income below four minimum wages (PR=1.41) and among professionals who worked exclusively in the health area (PR=1.95). Conclusion: Approximately half of the nursing technicians of the university hospital presented common mental disorders, which were associated with economic and work-related variables.
