During paediatric dental treatment, non-collaboration and fearful reactions are frequently observed in the child client. The dentistry student must be prepared to cope with these reactions, particularly considering the importance of the relationship between dentists and patients in the promotion of oral health. The present study aimed to assess undergraduate dentistry students' perceptions of their ability to cope with non-collaboration situations in paediatric dentistry. A Likert-style questionnaire was used to analyse students' self-confidence levels, and proposed solutions to 10 problem situations the students would be likely to encounter were recorded. The questionnaire was administered to two undergraduate dentistry student groups from two different Brazilian Public Faculties, comprising 122 respondents. The self-confidence analysis indicated that it varied according to the extent of the child's reaction and the invasiveness of the procedure. Responses to the open-ended questions were categorised by solution proposed, and the analysis indicated that the most frequent responses were categorised as follows: tranquilising, explanation and restriction. Significant differences were found in tranquilising (with higher values for Faculty 2 than 1, and higher values for female students than male students at Faculty 2) and restriction (with higher values for female students compared with male student at both Faculties). The results and discussion focused on the aspects of training dentistry students' social and behavioural management skills.
O SUS é uma conquista do povo brasileiro e trouxe com a sua criação um novo paradigma de saúde, deslocando o olhar da doença para o sujeito e seu contexto, que levou à reorganização do sistema de saúde, e a uma reflexão sobre a formação dos profissionais da área. A Fonoaudiologia está em processo de construção da sua identidade para atuação no sistema público de saúde, uma vez que é uma profissão com regulamentação recente, por volta de 30 anos, ainda em expansão. Dessa forma, é de grande importância a discussão sobre o dimensionamento dos profissionais dessa categoria dentro do SUS. O Estado de São Paulo concentra o maior número de profissionais de Fonoaudiologia e de cursos de graduação na área. Neste panorama surge o interesse em pesquisar a distribuição de profissionais de Fonoaudiologia no SUS do Estado de São Paulo para compreender a inserção desta categoria no serviço público. Foram analisados dados secundários dos bancos de informações públicos do Ministério da Saúde e do IBGE relativos ao porte dos municípios paulistas e ao número de profissionais de Fonoaudiologia atuando no SUS em cada um deles. É possível observar que não há lógica na distribuição dos profissionais, havendo relação inversamente proporcional entre o porte dos municípios e o número de fonoaudiólogos atuando no sistema público de saúde. O dimensionamento de profissionais de Fonoaudiologia pode ser instrumento importante para melhora na organização da oferta destes profissionais para a população.
The military are an understudied occupational category in relation to vocal health. The Brazilian Army Officer formation involves specific activities that could lead to vocal injury. One example is the Military Training Program which consists of a 48hour exercise that involves high vocal demand to produce military command shouts with intense physical training. During the training, students are submitted to controlled hydration and sleep deprivation. After vocal abuse at least 18 hours of complete vocal rest are necessary for functional recovery. However, the cell recovery demands more time until inflammatory process ends (at least 14 days after the injury). The goal of this study is to evaluate the immediate vocal impact and the vocal recovery of military students two weeks after completion of the Military Training Program. Vocal samples from young adult males (18 to 22 years old) were collected in three moments: before, immediately after and two weeks after Military Training Program. Phonatory measures were collect using vocalization of vowel /a/ and fricative consonants /s/ and /z/. Besides, some acoustic parameters were analyzed using speech samples: fundamental frequency, jitter, shimmer, mean of intensity, harmonic-to-noise ratio, H1-H2, spectral emphasis, range of frequency and intensity. The acoustic parameters were extracted or calculated from software Praat. Paired samples t-test was used to statistical analysis to compare differences among the parameters between two moments (before and immediately after Military Training Program) and One way ANOVA was used to statistical analysis to compare differences among the parameters in the three moments. Independent samples t-test was used compare the data found in the first moment of data collection and normative values from the literature, all with significance level of 0.05.The results of comparison of two first moments showed significant differences in maximum phonation times, mean of intensity and spectral emphasis for sustained vowel samples. Results of comparison of three moments showed no significant differences between first and third moment, except for maximum phonation time of vowel /a/, what suggest a vocal recovery after two weeks. There were differences between first moment data (before MTP) and normative values that suggest more researches about a vocal pattern different for this group.
RESUMO -É relatada a adaptação do questionário de Milgrom sobre autoconfiança e formas de lidar frente a situações-problemas em odontopediatria. A adaptação foi aprovada por especialistas e analisada em termos da quantidade, coerência e qualidade das respostas suscitadas. Durante o atendimento odontopediátrico, são freqüentes as situações em que a criança não colabora com o dentista, dificultando ou impedindo o tratamento. Essas ações da criança afetam o desempenho do profissional e, consequentemente, a qualidade do atendimento. A escolha e o uso de estratégias para obter a colaboração da criança dependem das informações a que o profissional teve acesso, principalmente durante a sua graduação, e, por isso, é importante que o curso de graduação aborde essas estratégias em um momento adequado para a formação do profissional. (Issáo & Guedes, 1999;Wright, 2000;Newton & Sturmey, 2003;Cardoso, Loureiro & Nelson-Filho, 2004).Tendo em vista a freqüência das situações de não--colaboração e a necessidade do profissional lidar com elas, considera-se necessária uma análise da avaliação, feita pelos próprios alunos de odontologia, em relação à sua percepção de habilidades para lidar com as questões relativas à colaboração de crianças durante o atendimento. Essa auto-avaliação permite uma reflexão sobre as condições em relação às quais os estudantes verbalizam dificuldades, apontando para aspectos a serem abordados em sua formação.Uma das formas de realizar essa avaliação foi proposta por Peter Milgrom, um pesquisador norte-americano que atua no Department of Dental Public Health Sciences da University of Washington, na área de estudos de ansiedade, medo e estratégias de ensino em Odontologia (Milgrom, Weistein & Getz, 1995). Esse autor elaborou um questionário com 20 questões, descrevendo situações-problema no atendimento odontológico de crianças não-colaboradoras, a ser respondido por odontólogos ou por estudantes de Odontologia.
