Perilesional edema (PE) is commonly observed in association with an intracranial mass. PE is thought to be determined by vasogenic effects in the cerebral parenchyma surrounding the mass due to the loss or absence of the blood-brain barrier (BBB) inside the lesion. Alterations in capillary permeability induce extrusion of fluids into the extravascular space around the mass. On Computed Tomography (CT) PE corresponds to an area of low density for the increased water content, outside the margins of the lesion. It is difficult to differentiate PE from areas of parenchymal compressive ischemia and sometimes the two events could be associated. A solitary mass with PE is more commonly discovered on a non-enhanced computed tomography (NECT) study performed for the onset of stable or rapidly progressive neurological symptoms. In such cases, a supplementary CT scan with contrast (CECT) is generally indicated to complete the baseline imaging before MRI. Contrast enhancement is generally present in a mass with PE and it is not specific for differential diagnosis. Perfusion computed tomography (PCT) requires a few minutes in addition to the time needed for CECT. PCT may give information on regional microvascular density, permeability and blood flow, thus it may play a role when tumoral neo-angiogenesis or non-neoplastic altered haemodynamics are suspected. We therefore investigated the utility of PCT in the differential diagnosis of the intracranial solitary masses with PE.
IntroduzioneLa tecnica descritta da J. Moret et Al 1 e nota come "remodelling" consente il trattamento di aneurismi con rimodellamento del colletto durante il rilascio delle spirali di Guglielmi (GDC). Lo scopo del seguente lavoro è stata la valutazione dell'efficacia e della sicurezza della tecnica di "remodelling" nel trattamento endovascolare di aneurismi intracranici ad ampio colletto in cui la tecnica convenzionale aveva fallito o non era indicata, in una serie di pazienti con follow-up. Pazienti e metodiLe cartelle cliniche e le indagini radiologiche di 37 pazienti (28 femmine, 9 maschi, età media: 56.6 aa, range: 27-81 aa) trattati di aneurisma cerebrale con "remodelling" sono state esaminate retrospettivamente. Ventinove pazienti erano giunti con emorragia subaracnoidea (ESA) da rottura aneurismatica e Hunt&Hess grading di I (n°= 2), II (n°= 13), III (n°= 11), IV (n°= 3), 2 pazienti presentavano una riduzione del visus ed 1 paziente cefalea e diplopia da compressione del III nervo cranico. In 5 pazienti l'aneurisma rappresentava un reperto occasionale. Aneurismi multipli erano presenti in 12 pazienti (n°: 2 in 9 pazienti e n°: 3 in 3 pazienti) ed il "remodelling" è stato impiegato in 1 aneurisma in 9 pazienti, in 2 aneurismi in 2 pazienti e in 3 (figura 1) aneurismi in 1 paziente in un'unica (n°=5) o più sessioni consecutive (n°=7).Quattro residui/ricrescite in pazienti differenti sono state ritrattate con "remodelling" e la seconda procedura sullo stesso aneurisma è stata considerata come una nuova procedura. Nel complesso sono stati embolizzati con "remodelling" 41 aneurismi (34" small", 7" large") per un totale di 45 procedure differenti. Il "remodelling" è stato impiegato in seguito al fallimento dell'embolizzazione con tecnica convenzionale o del clippaggio chirurgico rispettivamente in 4 e 2 casi. In 7 pazienti era presente vasospasmo focale (n°=5) o diffuso (n°=2) all'inizio della procedura. La localizzazione degli aneurismi era la seguente: comunicante posteriore (n°=12), paraclinoidei (ipofisaria superiore/carotido-oftalmica n°=15), apice basilare (n°=4), biforcazione carotidea (n°=4), comunicante anteriore (n°=2), cerebellare superiore (n°=1), cerebrale media tratto M1 (n°=1), corioidea anteriore (n°=1), carotido-cavo (n°=1). Tutti gli aneurismi avevano un colletto uguale o superiore a 3 mm e 21 avevano una dimensione della sacca uguale o superiore a 10 mm. Il trattamento è stato classificato sulla base della percentuale di occlusione angiografica in completo (100%) se sia il colletto che la sacca erano densamente stipati, subtotale (90-95%) se il disegno delle spirali era lasso, con residuo di colletto e con residuo di aneurisma. In tutti i pazienti è stato ottenuto un esame TC a termine della procedura. In 29 pazienti era disponibile un follow-up clinico e radiologico (durata media: 10 mesi). L'ultimo esame angiografico
La Cifoplastica (CPL) è una variante della vertebroplastica percutanea (PVP) e consiste nell'introduzione di cemento osseo acrilico o biocompatibile a base di fosfato di calcio 9 o idrossiapatite, direttamente nel corpo vertebrale, attraverso aghi da biopsia ossea, previa dilatazione del metamero collassato mediante dispositivi gonfiabili (palloncini) che vengono successivamente rimossi per iniettare il cemento. Lo scopo è quello di ottenere un effetto antalgico duraturo consolidando il corpo vertebrale dopo aver ripristinato, in tutto od in parte, l'altezza originaria del soma.Tale metodica, introdotta recentemente negli Stati Uniti, è derivata dalla PVP che è stata messa a punto invece in Francia nel 1987 da Deramond e si è molto diffusa in tutto il mondo. La PVP, nata per il trattamento di un angioma aggressivo di C2, ha trovato larga applicazione principalmente nel consolidamento dei crolli vertebrali osteoporotici 10 ma anche di angiomi e di lesioni sostitutive del rachide (mieloma multiplo, metastasi). Con l'innalzamento dell'età media della popolazione nei Paesi industrializzati e soprattutto in Italia, si assiste ad un progressivo aumento di incidenza della malattia osteoporotica e delle conseguenti fratture vertebrali che causano dolore, prolungata immobilità, decadimento delle condizioni generali di salute e complicanze cardio-respiratorie, fenomeno che sta acquisendo sempre maggiore rilevanza per gli elevati costi sociali ed economici.Nel caso di patologia sostitutiva del rachide l'iniezione di cemento osseo nella vertebra ha dimostrato in molti casi di svolgere anche una funzione antagonista alla progressione della malattia, oltre all'effetto antalgico e meccanico stabilizzatore, probabilmente per un'azione citotossica diretta dei componenti del cemento o in seguito alla reazione esotermica di polimerizzazione. Tali procedure sono inoltre associabili ai tradizionali trattamenti chemio-e radioterapici 4 .Nel seguente lavoro riportiamo l'iniziale esperienza del nostro Centro nel trattamento con CPL di pazienti con crolli vertebrali osteoporotici e post-traumatici. Tecnica CPLTutte le procedure che vengono di seguito descritte devono essere eseguite con scrupolosa osservanza della più rigorosa asepsi. Il paziente giace sul letto angiografico od operatorio in posizione prona, in anestesia generale, continuamente monitorizzato. Si procede con il posizionare uno o due aghi da biopsia per via percutanea nel corpo vertebrale con accesso posterolaterale o extrapeduncolare a livello dorsale medio-alto, e transpeduncolare a livello dorsale basso e lombare. È preferibile quando possibile l'accesso transpeduncolare bilaterale perché più sicuro, sempre radiologicamente assistito mediante scopia.Dopo aver eseguito un prelievo bioptico attraverso lo stesso accesso, si introducono i palloncini che vengono riempiti con soluzione fisiologica e mezzo di contrasto iodato, collegati ad un dispositivo di gonfiaggio manuale dotato di manometro, ed espansi progressivamente fino a raggiungere una pressione non superiore a 300 ...
