The new Coronavirus infection, which was first seen in China in late December 2019 and eventually became a worldwide pandemic, poses a serious threat to public health. After a high spike in the number of new COVID-19 infection cases following increase in overall daily death toll in Turkey, Turkish Ministry of Health has taken immediate precautions to postpone elective surgeries in order to reduce the burden to the healthcare system which might be challenged. Whereas different areas of medicine were able to suspend their operative procedures during this period, this was not completely possible in pediatric cardiovascular surgery due to the severity and urgency of congenital heart disease patients requiring operation. Based on the guideline that was published by the Turkish Pediatric Cardiology and Cardiac Surgery Association, in which the patients requiring surgical intervention during the Covid-19 pandemic period are ranked according to the priority, directions were given regarding the operations that hereby, be delayed, we report our experience in 29 cases retrospectively, regarding the preoperative evaluation of these patients, makings of an emergency operation decision, and strategies taken about intraoperative and postoperative management and arrangements during the pandemic period. In this article, we present crucial precautions that was applied in pediatric cardiovascular surgery and extensive list of cases in order to deliver highest level of the patient safety and protection for the surgical team.
Bu çalışmada geleneksel heparin rejimi ile yağsız vücut ağırlığına göre hesaplanan heparin doz rejimi yeterli antikoagülasyonun sağlanması ve perioperatif kanama ile ilişkili komplikasyonlar açısından karşılaştırıldı. Ça lış ma pla nı: Bu prospektif, tek kör, randomize çalışmaya Haziran 2016-Ocak 2017 tarihleri arasında kardiyopulmoner baypas ile elektif kapak ameliyatı yapılan toplam 100 erişkin hasta (42 erkek, 58 kadın; ort. yaş 52.7 yıl; dağılım, 22-84 yıl) alındı. Kardiyopulmoner baypas öncesinde heparin dozu, aktüel vücut ağırlığına göre (n=50) ve yağsız vücut ağırlığına göre (n=50) olmak üzere 4 mg/kg olarak ayarlandı. Kardiyopulmoner baypasa başlanması için minimum aktive pıhtılaşma zamanı hedef değeri 480 sn. olarak kabul edildi. Demografik ve hemodinamik veriler, heparin sonrası aktive pıhtılaşma zamanı, ek heparin ve perioperatif transfüzyon, ameliyat sonrası drenaj miktarları, yeniden ameliyatlar ve mortalite kaydedildi. Bul gu lar: Demografik veriler, kros klemp ve kardiyopulmoner baypas süreleri ve ameliyat sırası transfüzyon ihtiyacı gruplar arasında anlamlı düzeyde farklı değildi. Heparinin başlangıç ve toplam dozlarının yanı sıra toplam protamin dozu, aktüel vücut ağırlığı grubunda anlamlı düzeyde daha yüksekti. Ameliyat sonrası transfüzyon oranları da, bu grupta daha yüksekti. Yağsız vücut ağırlığı grubunda hiçbir hastada yeniden ameliyat gerekli olmaz iken, aktüel vücut ağırlığı grubunda üç hasta yeniden ameliyat edildi. So nuç: Çalışma sonuçlarımız kardiyopulmoner baypas sırasında yağsız vücut ağırlığına göre hesaplanan heparin doz titrasyonu ile yeterli antikoagülasyon sağlanabileceğini ve toplam heparin ve protamin dozlarının düşürülmesi ile ameliyat sonrası kanama ve kan ürünü transfüzyonu gereksinimin azaltılabileceğini göstermiştir. Anomalous origin of coronary artery originating from the pulmonary artery in conjunction with the aorticopulmonary window (APW) is a rare but a significant anomaly in the era of congenital cardiac diseases. The occurrence of anomalous origin of the right coronary artery from the pulmonary artery among the associated anomalies is less than 5%. The severity of the clinical condition of these patients depends on the degree of left–right shunt and compromise of the pulmonary blood flow. We report surgical management of a case of a 45-day-old infant with APW, ventricular septal defect, and anomalous origin of coronary artery originated from the pulmonary artery.
rans-radial approach to cardiac catheterization certainly has advantages in terms of access-site complications. 1 Although very low, complications such as radial artery occlusion, nonocclusive injury, spasm, hand ischemia, nerve damage, bleeding and pseudoaneurysm formation may occur. 2 In this report, we presented a radial artery pseudoaneurysm in a patient on anticoagulation therapy. CASE REPORTA 42-year-old male underwent diagnostic coronary angiography prior to the surgical procedure. Catheterization was performed with the standard heparin regimen (5000 IU IV bolus only once). Hemostatic parameters were normal prior to surgery. Mechanical aortic valve replacement and ascending aorta graft interposition were performed. The patient was on warfarin therapy along with low molecular weight heparin (LMWH) following surgery.
Amaç: Bu çalışma, antegrad serebral perfüzyon ve distal ılımlı hipotermik sirkülatuararrest tekniğinin endotel fonksiyonuna ilişkin biyobelirteçler üzerine etkisini standart kalp cerrahisi ile karşılaştırmayı amaçlamaktadır. Materyal ve Metod: Asendan aort anevrizması için 28 ° C' da selektif antegrad serebral perfüzyon ve orta derecede hipotermik alt vücut dolaşım durması uygulanan 36 hasta (standart çalışma grubu), konvansiyonel kardiyopulmoner bypass ile standart kalp ameliyatı uygulanan (kontrol grubu) 36 hasta prospektif olarak karşılaştırıldı. Vena kava inferiordan elde edilen kan örneklerinde nitrik oksit, asimetrik dimetilarginin, hidrojen sülfit ve toplam antioksidan kapasite durumu ve laktat seviyeleri incelenmiştir. Klinik sonuçlar ve biyokimyasal parametreler değerlendirildi. Bulgular: Endotel fonksiyonuyla ilgili biyobelirteçler, asimetrik dimetilarjinin dışındaki gruplar arasında benzer bulundu. Asimetrik dimetilarjinin düzeyleri düşüktü, laktat düzeyleri ise kontrol grubuna göre anlamlı derecede yüksek bulundu. Kontrol gruptaki koroner arter hastalarının baskınlığını nedeniyle çıkan sonuçlarda kafa karıştırıcı bir faktör olarak düşünülerek koroner arter hastaları analiz dışında bırakıldığında, asimetrik dimetilarjinin düzeylerinin, iki alt grup arasında benzer olduğu bulundu. Sonuç: Çalışma grubundaki düşük plazma asimetrik dimetilarjinin seviyeleri, endotelyal nitrik oksit sentezinde koruyucu bir role sahip olabilir. Koroner arter hastalığı olan hastalar her iki gruptan da dışlandığında, endotel fonksiyonuna bağlı biyobelirteçler her iki grupta da benzerdi. Bu sebeple antegrad serebral perfüzyon ve distal ılımlı hipotermik sirkülatuar arrest tekniğinin kısa dönemde endotel fonksiyonlarını etkilemediğini düşünmekteyiz.
IntroductionIn this study we try to observe the fate of the left internal thoracic artery grafts that were bypassed to left anterior descending artery with moderate stenosis identified with fractional flow reserve (FFR) technique. Doppler ultrasonography was chosen as a noninvasive screening method.MethodsA total of 30 patients who underwent coronary artery bypass grafting depending on results of the fractional flow reserve between January 2007 and January 2012, were subjected to transthoracic color Doppler ultrasonographic evaluation irrespective of the presence of symptoms, and the presence of a systolic-diastolic flow pattern was investigated using the supraclavicular approach.ResultsThe left internal thoracic artery graft was found to be functional in 63.3% of patients within a mean period of 35.1±19.7 months between coronary bypass and color Doppler ultrasonography. This period was found to be 29.4±19.6 months in the functional graft group, and 44.7±16.6 months in the dysfunctional graft group (P=0.046). Preoperative complaints of angina were reported to fall from 88.9% to 16.7% in the functional graft group, when compared to the postoperative period (P<0.001), but fell from 90.9% to 36.4% in the dysfunctional graft group (P=0.034).ConclusionFunctional left internal thoracic artery graft rates of the study population were found to be lower than the studies reported in the literature.
