Reikšminiai žodžiai: kompiuterinė tomografija, šlapimo rūgšties akmenys, Haunsfildo vienetai, ne uratiniai akmenys, ekstrakorporinė smūginės bangos litotripsija (ESBL). Tikslas. Šlapimo takų akmenligė tampa vis dažnesne liga, ypač Vakarų šalyse. Akmenis sudarantys skirtingi cheminiai junginiai yra nevienodo tankio ir kietumo, todėl akmens sudėtis turi didelę reikšmę parenkant gydymo planą. Gydymo pasirinkimui taip pat reikšmingi yra akmens vieta ir dydis. Šio tyrimo tikslas – retrospektyviai ištirti šlapimo takų akmenų tankį in vivo, atliekant kompiuterinę tomografiją ir matuojant Haunsfildo vienetais (HV) bei palyginti su biochemine analize, atlikta infraraudonųjų spindulių pektrometrija. Metodai. Nuo 2010 iki 2014 m. 99 pacientams, kuriems įtariama šlapimo takų akmenligė, buvo atliekama kompiuterinė tomografija. Patyręs radiologas, kuris neturėjo duomenų apie akmenų cheminę sudėtį, retrospektyviai peržvelgė ir išanalizavo akmenų tankį Haunsfildo vienetais. Rezultatai buvo palyginti su gauta biochemine analize infraraudonųjų spindulių spektroskopija. Rezultatai. Iš 99 tirtų akmenų 57 buvo kalcio oksalato akmenys (57,6 proc.), 11 – struvito (11,1 proc.), 19 – grynos šlapimo rūgšties (19,2 proc.) ir 12 – kalcio oksalato bei šlapimo rūgšties akmenų (12,1 proc.). Patikrinus rezultatus, naudojant ANOVA palyginimą, buvo nustatyta, kad skirtingos cheminės sudėties akmenų tankiai nėra reikšmingai susiję. Naudojant nepriklausomų imčių t testą nustatyta, kad grynos šlapimo rūgšties akmenų tankis statistiškai reikšmingai skiriasi nuo kitą cheminę sudėtį turinčių akmenų (p < 0,001), tačiau pritaikius logistinę regresiją nustatyta, kad šis kompiuterinės tomografijos tankis yra nepakankamai patikimas prognozinis veiksnys. Išvados. HV dydis reikšmingai skiriasi tarp grynos šlapimo rūgšties ir kitą cheminę sudėtį turinčių akmenų, tačiau HV dydis nėra patikimas rodiklis, norint nustatyti akmens cheminę sudėtį. Taip pat HV dydis nėra patikimas nuspėjantysis rodiklis, diferencijuojant grynos šlapimo rūgšties akmenis nuo kitą cheminę sudėtį turinčių akmenų.
