ObjectiveTo determine the presence of linear relationship between renal cortical thickness, bipolar length, and parenchymal thickness in chronic kidney disease patients presenting with different estimated glomerular filtration rates (GFRs) and to assess the reproducibility of these measurements using ultrasonography.Materials and MethodsUltrasonography was performed in 54 chronic renal failure patients. The scans were performed by two independent and blinded radiologists. The estimated GFR was calculated using the Cockcroft-Gault equation. Interobserver agreement was calculated and a linear correlation coefficient (r) was determined in order to establish the relationship between the different renal measurements and estimated GFR.ResultsThe correlation between GFR and measurements of renal cortical thickness, bipolar length, and parenchymal thickness was, respectively, moderate (r = 0.478; p < 0.001), poor (r = 0.380; p = 0.004), and poor (r = 0.277; p = 0.116). The interobserver agreement was considered excellent (0.754) for measurements of cortical thickness and bipolar length (0.833), and satisfactory for parenchymal thickness (0.523).ConclusionThe interobserver reproducibility for renal measurements obtained was good. A moderate correlation was observed between estimated GFR and cortical thickness, but bipolar length and parenchymal thickness were poorly correlated.
Chronic kidney disease is a disease with high morbidity and mortality. The daily life of affected patients includes negative feelings, fear of prognosis, disability, and economic dependence, as well as challenges related to changes in self-image. Religion and spirituality can be important resources in dealing with these difficulties. To test this hypothesis a qualitative, descriptive study was undertaken at the Hospital das Clínicas Samuel Libânio, Pouso Alegre/MG, in order to verify from the point of view of both doctors and patients if they believe that religion influences the treatment of the disease. For doctors, religion represents a strength and a comfort when facing any disease. Patients, however, identify religion with the hope that they will get better. While the significance of religion in their lives was different between the groups, both agreed that it represents a beneficial factor in the life of patients, providing relief, support and optimism. O cotidiano dos pacientes acometidos reveste-se de sentimentos negativos, medo do prognóstico, incapacidade, dependência econômica, além daqueles relacionados à alteração da autoimagem. A religião e a espiritualidade podem ser recursos importantes para lidar com essas dificuldades. Para verificar essa hipótese, realizou-se no Hospital das Clínicas Samuel Libânio de Pouso Alegre/MG estudo qualitativo descritivo, objetivando avaliar, do ponto de vista do médico e do paciente, em que medida a religião influi no tratamento da doença. Para os médicos, a religião representa força e conforto no enfrentamento de qualquer doença. Já os pacientes, todavia, depositam na religião a esperança de que irão melhorar. O significado da religião em suas vidas foi distinto entre os dois grupos analisados; entretanto, ambos concordam que a religião configura um fator benéfico na vida do paciente, propiciando alívio, suporte e otimismo. Objective: To evaluate the diagnostic capacity of abdominal computed tomography in the assessment of hepatic steatosis using the portal phase with a simplified calculation method as compared with the non-contrast-enhanced phase. Materials and Methods: In the present study, 150 patients were retrospectively evaluated by means of non-contrast-enhanced and contrast-enhanced computed tomography. One hundred patients had hepatic steatosis and 50 were control subjects. For the diagnosis of hepatic steatosis in the portal phase, the authors considered a result of < 104 HU calculated by the formula [L -0.3 × (0.75 × P + 0.25 × A)] / 0.7, where L, P and A represent the attenuation of the liver, of the main portal vein and abdominal aorta, respectively. Sensitivity, specificity, positive and negative predictive values were calculated, using non-contrast-enhanced computed tomography as the reference standard. Results: The simplified calculation method with portal phase for the diagnosis of hepatic steatosis showed 100% sensitivity, 36% specificity, negative predictive value of 100% and positive predictive value of 75.8%. The rate of false positive results was 64%. False negative results were not observed. Conclusion: The portal phase presents an excellent sensitivity in the diagnosis of hepatic steatosis, as compared with the non-contrast-enhanced phase of abdominal computed tomography. However, the method has low specificity. Materiais e Métodos: Foi realizado estudo retrospectivo em 150 pacientes submetidos a tomografia computadorizada de abdome sem e com contraste intravenoso, 100 deles com esteatose hepática e 50 controles. Para diagnóstico de esteatose hepática na fase portal considerou-se um resultado < 104 UH aplicando-se a fórmula [L -0,3 × (0,75 × P + 0,25 × A)] / 0,7, onde L, P e A representam a atenuação hepática, da veia porta e da aorta, respectivamente. Foram calculados sensibilidade, especificidade, valores preditivos positivos e negativos, utilizando-se a tomografia computadorizada sem contraste intravenoso como padrão de referência. Resultados: O método simplificado de cálculo para o diagnóstico de esteatose hepática na fase portal mostrou sensibilidade de 100%, especificidade de 36%, valor preditivo negativo de 100% e valor preditivo positivo de 75,8%. A taxa de falso-positivos foi 64%. Não foram encontrados falso-negativos. Conclusão: A utilização da fase portal apresenta elevada sensibilidade para o diagnóstico de esteatose hepática, quando comparada à fase sem contraste da tomografia computadorizada de abdome. OBJETIVO: Avaliar o grau de aceitação do paciente submetido a colonografia por tomografia computadorizada (CTC) em comparação com a colonoscopia, quando realizadas para rastreamento de doença colorretal. MATERIAIS E MÉTODOS: Cinquenta pacientes com suspeita de doença colorretal foram submetidos a CTC e colonoscopia. Questionários foram aplicados antes e após a realização da CTC e após a colonoscopia. Graduou-se o desconforto esperado e experimentado antes e após a realização da CTC e da colonoscopia, bem como a preferência do paciente por exame. RESULTADOS: Em relação à CTC, antes de iniciar o exame 18% dos pacientes afirmaram esperar pouco desconforto, 78%, desconforto moderado e 4%, muito desconforto. Após a realização do exame, 72% dos pacientes relataram pouco desconforto, 26%, desconforto moderado e apenas um (2%) dos pacientes referiu muito desconforto. Após a realização da colonoscopia, 86% dos pacientes relataram preferência pela CTC. O grau de distensão colônica e a quantidade de fluido residual não influenciaram na preferência dos pacientes. CONCLUSÃO: Os pacientes preferiram a CTC à colonoscopia, não havendo relação estatística com o grau de distensão colônica na CTC e a eficiência do preparo intestinal.
