Introduction les complications infectieuses constituent la première cause de morbi-mortalité chez le drépanocytaire surtout avant l´âge de 5 ans. Le but de cette étude est de ressortir les aspects épidémiologiques, les étiologies et la prise en charge des infections ostéo-articulaires chez les drépanocytaires à Lubumbashi. Méthodes il s´agissait d´une étude descriptive transversale et rétrospective réalisée au centre de recherche de drépanocytose de Lubumbashi (CRDL) sur une période de trois ans soit de juin 2014 à juin 2017. Elle a porté sur tous les patients drépanocytaires suivis au CDRL ayant développé une infection ostéo-articulaire. La récolte des données s´était réalisée sur base d´une fiche d´enquête reprenant comme paramètres d´étude l´âge du patient, l´âge de la première consultation, le sexe, le motif de consultation, les antécédents, les signes physiques, le diagnostic posé, les bilans paracliniques, le traitement. Résultats nous avions répertorié 35 cas d´infections ostéo-articulaires sur un total de 380 patients drépanocytaires soit une fréquence 9,2%. La tranche d´âge la plus touchée était celle de moins de 5 ans (37,1%); l´âge moyen était de 10,9±9,5 ans pour des extrêmes de 8 mois et 37 ans. Il y a une légère prédominance féminine avec 51,4% des cas (sexe ratio de 1,06 en faveur des femmes). Les antécédents majeurs étaient la transfusion (16,6%) et la splénectomie (8,6%). Le motif de consultation était dominé par la douleur des membres (84%). Vingt patients (57,1%) avaient des conjonctives bulbaires ictériques et 26 (74,3%) étaient pâles. A l´inspection, la tuméfaction des membres et la plaie étaient retrouvées respectivement chez 27 patients (77,1%) et 19 patients (54,3%). La splénomégalie cliniquement palpée chez 6 patients (17,1%). Trois types d´infections ostéo-articulaires avaient été décelés, dominés par l´ostéomyélite avec 24 cas (68,57%) suivi de l´ostéite avec 7 cas (20%) et l´arthrite suppurée avec 4 cas (11,43%). Sur les 24 cas d´ostéomyélites, 18 étaient aiguës (75%) et 6 étaient chroniques (25%) dont 4 hyperostosantes et 2 fistulisantes. Le tibia a été l´os le plus touché (18 cas), la radiographie était dominée par des images d´ostéolyse avec 27 cas (77,1%) puis de réaction périostée avec 15 cas (42,9%). Le type homozygote était retrouvé chez 88,6% des cas. L´hémoculture a été réalisée chez 17 patients sur les 35 et la salmonelle a été isolée dans 15 cultures sur les 17 soit 88,23% alors que la pyoculture réalisée chez 10 patients avaient isolé également d´autres germes. Le bilan inflammatoire avait été réalisé chez 21 patients dont 15 avaient une hyperleucocytose, 13 une formule leucocytaire pathologique, tous avaient une vitesse de sédimentation accélérée (supérieure à 20mm à la 1 ère heure). Concernant le calendrier vaccinal, les vaccins du PEV étaient suivis par 62,86% des patients alors que le taux de vaccination spécifique pour les drépanocytaires était de 17,14%. Sur le plan...
L´angiosarcome est une tumeur rare à multiples localisations mais celle cutanée est la plus fréquente faisant de sa symptomatologie un polymorphisme clinique. C´est une tumeur de mauvais pronostic à cause de sa grande propension à la récidive locale et aux métastases à distances précoce. Nous rapportons un cas de découverte fortuite d´un angiosarcome de l´épaule sur un ancien foyer de traumatisme direct datant d´une année chez un patient de 72 ans, venu consulter pour douleur persistante en regard d´une tuméfaction à la face postérieure de l´épaule contemporaine au traumatisme. L´examen clinique conclu en un hématome ancien partiellement calcifié soutenu par la ponction de la masse ayant ramené 5 cc de sang d´aspect noirâtre et renforcé par une échographie. La radiographie de l´épaule n´étant pas pathologique, une exploration de la tuméfaction a été entreprise. Celle-ci a permis de mettre en évidence et reséquer des tissus friables, d´aspect rougeâtre, avec un saignement important difficilement maitrisé pendant deux jours compliqué d´une anémie non tolérée corrigée par deux transfusions sanguines. L´anatomie pathologique des tissus réséqués a conclu en un angiosarcome moyennement différentié avec malheureusement des berges non saines. Un bilan d´extension a été réalisé à la recherche des métastases. Pas de récidive locale sur trois mois et le patient a été transféré dans un centre spécialisé à Lusaka pour la radiothérapie complémentaire. L´objectif du présent travail est de présenter un cas rare d´angiosarcome de découverte fortuite sur un antécédent de traumatisme de l´épaule gauche et de ressortir les aspects thérapeutiques en passant en faisant une revue de la littérature.
Fournier's gangrene, a rapidly progressive necrotizing fasciitis of the perineum and external genitalia that leads to extensive soft tissue necrosis, remains a medical‐surgical emergency. The authors present a case of coverage of a scrotal loss of substance with testicular exposure by a medial fasciocutaneous flap of the unilateral thigh after debridement and initial medical treatment of Fournier's gangrene in a 50‐year‐old quadriplegic patient. The clinical examination at 8 months showed an aesthetic and functional result. The trophic‐cutaneous thigh flap that the authors propose for frail, elderly, and neurological subjects with low functional requirements allows coverage of the scrotal region with relatively thick skin, color, and texture close to those of the scrotum. It also offers technical simplicity, moderate scarring, and functional remnants at the price of less morbidity.
