O controlo percebido é um construto que se inscreve nas características psicossociais individuais. O seu estudo tem-se reportado essencialmente a indivíduos que passam por situações de saúde/doença, como as doenças do foro cardiovascular, potencialmente geradoras de resultados nefastos no seu bem-estar físico e psicológico. Este estudo teve como objetivo traduzir, adaptar linguística e culturalmente e validar para português a Control Attitudes Scale Revised (CAS-R). A amostra foi constituída por 160 doentes cardíacos, internados em serviços de cardiologia/UCIC de três hospitais do Norte de Portugal. A análise de componentes principais indiciou uma estrutura unidimensional da escala, tal como a proposta pelas autoras. Obtivemos um Alpha de Cronbach aceitável (α = 0.65). A validade discriminante revelou uma relação negativa entre o controlo percebido e a ansiedade e depressão. A versão portuguesa da CAS-R mostrou ser um instrumento adaptado para avaliar a perceção de controlo em pessoas com doença cardíaca, importante para ajudar a sinalizar os doentes cardíacos com maiores dificuldades de autogestão da sua doença e tratamento. Apesar de apresentar uma aceitável fidelidade e validade, necessita que sejam realizados novos estudos psicométricos que assegurem o rigor na sua mensuração.
RESUMO CONTEXTO: É hoje comumente aceite que fatores como a ansiedade e a depressão são favorecedores de efeitos negativos na saúde de indivíduos com doença cardíaca. Concomitantemente, constata-se que o sofrimento emocional resultante destes fatores é menor nos indivíduos com maiores níveis de controlo percebido. OBJETIVO: Determinar os fatores preditivos de depressão em pessoas com doença cardíaca. METODOLOGIA: Quantitativa, transversal, descritiva e correlacional. Amostra constituída por 160 indivíduos com doença cardíaca internados em serviços de cardiologia e unidades cuidados intensivos de cardiologia de três hospitais do norte de Portugal. RESULTADOS: Os indivíduos com maior ansiedade (ß= 0,383; p <0,01), menor controlo percebido (ß= -0,255; p <0,01), com mais idade (ß= 0,218; p <0,01), viúvos (ß= 0,213; p <0,01), com colesterol elevado (ß= 0,184; p <0,01) e que não fazem exercício físico (ß= -0,159; p <0,01) apresentaram maiores níveis depressão. CONCLUSÃO: Perante os resultados obtidos, infere-se que o desenvolvimento de intervenções que promovam o aumento do senso de controlo e a diminuição da ansiedade, podem ser e cazes na prevenção da depressão.
Introdução: A ventilação mecânica invasiva prolongada acarreta custos e tem complicações associadas. Um desmame ventilatório precoce com sucesso, assume cada vez mais importância, pretendendo-se neste estudo descrever as evidências acerca da utilização da Oxigenoterapia de Alto Fluxo por Traqueostomia neste processo. Metodologia: Scoping Review segundo a metodologia do Joanna Briggs Institute®, com pesquisa efetuada nas bases de dados: CINHAL, PubMed e LILACS. Resultados: Incluíram-se quatro estudos: um estudo de caso e três estudos controlados randomizados. Relativamente aos protocolos utilizados na implementação da oxigenoterapia de alto fluxo por traqueostomia, eles variaram entre diferentes fluxos, tempo de utilização e combinação com outros modos ventilatórios. Os principais parâmetros monitorizados foram: frequência respiratória, FiO2, SpO2, PaO2, PaCO2 e a relação PaO2/FiO2. Discussão: A utilização de oxigenoterapia de alto fluxo por traqueostomia apresenta vantagens face à utilização de outros modos ventilatórios durante o processo de desmame ventilatório. Esta estratégia repercute-se na melhoria dos parâmetros monitorizados e na diminuição do tempo de desmame. Conclusão: As principais contribuições da oxigenoterapia de alto fluxo por traqueostomia no desmame ventilatório são: melhoria da oxigenação, diminuição do esforço respiratório, aumento do volume corrente, diminuição do tempo de desmame e melhoria da eficácia da limpeza das vias aéreas. RESUMOIntrodução: A qualidade dos cuidados é atualmente um foco de atenção de todos os profissionais de saúde, nomeadamente dos enfermeiros. A evidência tem vindo a demonstrar que a implementação de processos supervisivos entre pares é promotora do desenvolvimento de competências profissionais, permitindo aos enfermeiros exercer uma prática profissional adequada e criticoreflexiva, o que consequentemente terá repercussões positivas na qualidade dos cuidados de enfermagem. Decorrente das alterações demográficas, tecnológicas e científicas, a informação integra na atualidade o discurso dos profissionais de saúde. Desenvolvimento: a informação é uma ferramenta essencial na orientação dos cuidados de enfermagem, pelo que importa averiguar qual a informação que sustenta a tomada de decisão dos enfermeiros. Esta indagação auxilia também a identificação das necessidades de formação destes profissionais, visando o desenvolvimento pessoal e de competências profissionais. Com a presente revisão narrativa pretende-se refletir sobre a pertinência da implementação de processos supervisivo de pares em enfermagem, bem como do suporte que a informação constitui para a identificação de áreas do conhecimento necessárias à transformação das práticas. Conclusões: Como limitação na concretização deste artigo, evidenciamos a pouca bibliografia disponível principalmente no que respeita à evidência acerca da utilização da informação enquanto suporte à SC de pares em enfermagem, o que nos faz acreditar ser necessário o desenvolvimento de investigação que combine estas duas áreas.Palavras-chave: supervisão clínica; cuidados de enfermagem; informação em saúde; qualidade da assistência à saúde; competência profissional. ABStrACtIntroduction: Quality of care is currently a focus of attention for all health professionals including nurses. Evidence has shown that implementing a supervisory process between peers, promotes the development of professional skills, allowing nurses to provide an appropriate professional and critical-reflexive practice, which in turn, will have positive impact on the quality of nursing care. Resulting from the demographic, technological and scientific changes today information integrates the discourse of health professionals. Development: It is a fact that information is an essential tool in guiding nursing care and it is important to find out what information will be the basis for nurses' decision-making. This question also helps to identify of these professionals' training needs, in order to enhance personal development and professional skills and consequently to improve the quality of nursing care. With this narrative review, we intend to reflect on the relevance of implementing a peer supervision process in nursing, as well as, the support that information provides to identify areas of knowledge that need to be addressed with a view to transforming practices. RESUMOIntrodução: Não sendo as doenças cardiovasculares exclusivas do homem nem da mulher, importa conhecer que diferenças existem entre os géneros, uma vez que é um facto que as características físicas e psicossociais que os distinguem, podem influenciar os vários domínios da sua saúde, nomeadamente a saúde cardiovascular. Objetivos: Identificar diferenças de género numa amostra de pessoas com doença cardíaca. Métodos: Estudo quantitativo, transversal, que utiliza a análise descritiva e multivariada de dados com recurso à regressão logística. Conclusões: Os enfermeiros devem consciencializar-se de que as diferenças de género entre o homem e a mulher os fazem diferir no seu risco cardiovascular. Por tal, devem estar sensibilizados para a necessidade de identificar os fatores que o podem influenciar e definir e implementar intervenções capazes de reduzir esse risco para cada género. Objectives: To identify gender differences in a sample of people with heart disease. Methods: A quantitative, cross-sectional study with descriptive and multivariate data analysis using logistic regression. Conclusions: Nurses should be aware that gender differences between men and women make mean they differ in terms of cardiovascular risk. For this reason, they should be aware of the need to identify the factors that can influence the risk and define and implement interventions that reduce this risk for each gender.
