Purpose The purpose of this study is to report the association of dorsal wrist capsular avulsion with scapholunate ligament instability and to evaluate the results of an arthroscopy-assisted repair. Methods We retrospectively reviewed 10 patients with a mean age of 39.1 years suffering from chronic dorsal wrist pain. They underwent a wrist arthroscopy with an evaluation of the scapholunate ligament complex from the radiocarpal and midcarpal compartments. An avulsion of the dorsal intercarpal ligament (DICL) from the scapholunate interosseous ligament (SLIL) was visible from the radiocarpal compartment in all cases, while the SLIL was intact. The DICL tear was repaired with an arthroscopy-assisted dorsal capsuloplasty. Patients were assessed preoperatively and postoperatively by the QuickDASH (Disabilities of the Arm, Shoulder, and Hand) questionnaire, by the Visual Analog Scale (VAS) for pain, and by a clinical and radiological examination. Results Preoperatively, all patients had reduced flexion and radial deviation of the affected wrist. On the lateral radiograph, 5 of the 10 patients showed an increase of the scapholunate angle (60 to 85°). The scapholunate instability was graded as Messina-European Wrist Arthroscopy Society (EWAS) II in five cases and as grade IIIB in five cases. A tear of the ulnar part of the triangular fibrocartilage complex (TFCC) was found in seven cases. At a mean followup of 16 months, the wrist range of motion (ROM), the grip strength, the QuickDASH, and the VAS of pain improved significatively. The scapholunate angle was normalized in all cases. Discussion Isolated tears of the DICL at its insertion from the dorsal part of the SLIL can be associated with scapholunate instability in the absence of an injury to the SLIL. The diagnosis is made arthroscopically. The arthroscopic dorsal capsuloplasty is a minimally invasive technique that provides short-term satisfactory results. Further studies are needed to determine whether repair of the DICL tear could prevent secondary destabilization of the scapholunate ligament complex. Level of evidence IV (case series) Diagnosis.
Zusammenfassung Einleitung Alkoholkonsum in der Schwangerschaft birgt viele Risiken für das ungeborene Kind. Trotz präventiver Maßnahmen konsumieren manche Schwangere Alkohol. Diese Untersuchung zielt darauf ab, mögliche Gründe für den Konsum in der Schwangerschaft zu identifizieren und die Zuschreibung der Expert:innenrolle an medizinisches Fachpersonal durch die Betroffenen zu beleuchten. Material und Methoden In dieser Untersuchung wurden 9 Diskussionsthreads aus Onlineforen zum Thema Alkoholkonsum in der Schwangerschaft mit insgesamt 115 Diskussionsteilnehmern mittels Grounded Theory analysiert. Die diskursive Validierung der Ergebnisse erfolgte im Forschungsteam. Ergebnisse Es zeigte sich, dass die persönliche Definition der Mutterrolle Einfluss auf das Konsumverhalten hat. Frauen, die sehr um das Kindeswohl besorgt waren, konsumierten gelegentlich in Stresssituationen oder bei sozialen Anlässen, berichteten aber häufig über ein nach dem Konsum auftretendes schlechtes Gewissen. Frauen, die eigene Bedürfnisse stärker gewichteten, konsumierten eher aus Genuss oder weil sie sich durch Verbote bevormundet fühlten. Als förderliche Faktoren für die Zuschreibung der Expert:innenrolle an Gynäkolog:innen wurden eine Arzt-Patientenbeziehung auf Augenhöhe sowie eine ausführliche, non-direktive Aufklärung identifiziert. Als hinderlich zeigte sich dagegen eine paternalistische Haltung und das Aussprechen von Verboten. Diskussion Bei allen Frauen zeigte sich ein hoher Informationsbedarf zum Thema Alkoholkonsum in der Schwangerschaft. Die Arzt-Patienten-Beziehung scheint in der Informationsvermittlung eine wichtige Rolle zu spielen. Zudem wurde deutlich, dass der Unterstützungswunsch bei Frauen, die eine Abstinenz anstreben, nicht ausreichend erfüllt wird. Schlussfolgerung Der Alkoholkonsum in der Schwangerschaft sollte in der Betreuung aller Schwangeren regelmäßig angesprochen werden. Neben einer Aufklärung über die Risiken des Konsums sind Informationen zu Unterstützungsangeboten relevant.

Background The dorsal capsuloligamentous scapholunate septum (DCSS) is a confluence of the dorsal capsule, the dorsal intercarpal ligament (DIC), and the scapholunate interosseous ligament (SLIOL). It appears to play a role in the stability of the scapholunate articulation. The purpose of this study was to describe the anatomical basis for this structure and to investigate its role in scapholunate instability through sectioning of this structure followed by an arthroscopic and fluoroscopic analysis. Material and Methods In the anatomical part of the study we dissected 3 fresh cadaver wrists to examine the anatomy of the DCSS. In the arthroscopic part of the study we assessed the EWAS grade of SL instability before and after sectioning the DCSS and measured the scapholunate and radiolunate angles fluoroscopically. Results Sectioning the DCSS increased the EWAS grade of SL instability but did not affect the scapholunate gap, the scapholunate angle or radiolunate angle. Conclusion We have demonstrated that there is a distinct structure that is separate from the dorsal capsule, which we have labeled the Dorsal Capsuloligamentous Scapholunate Septum. We believe that the DCSS is a previously unreported secondary stabilizer of the SL joint which may have therapeutic and prognostic implications.
Qualitative research methods, primarily interviews, have become more common in the field of addiction research. Other data sources were often neglected, although sources such as social media can offer insights into the realities of people, since social media also plays a relevant role in today's living environments. This article examines the use of online forums as an underutilized data source in contrast to telephone interviews, to identify methodological opportunities and challenges. We analyzed nine discussion threads and seven interviews about ‘alcohol consumption during pregnancy’. Discursive comparison of the results was performed with a focus on sampling issues, comparability and risks for participants and researchers. Key issues were present in both data sources. People with different opinions were openly hostile in forums, while tolerance was more often expressed in interviews. The interviews showed a rather mild communication style, which could be attributed to social desirability. In the forum discussions, the participants often expressed themselves very directly. To comprehensively grasp the subject matter of the research, it is important to recognize the types of communication promoted by different data sources. These results have implications for research about female substance use. Knowledge of the issues will bestow a valuable contribution to researchers working in the field of substance use to help develop appropriate research approaches, as they engage in research into this highly stigmatized and controversial area.
Zusammenfassung. Hintergrund: Qualitative Methoden ermöglichen bei unterschiedlichen Fragestellungen vertieften Erkenntnisgewinn im Feld der Suchtforschung. Allerdings ist der Zugang zu den entsprechenden Methoden nicht einfach, da ihre Komplexität und Vielfalt nicht leicht zu überblicken ist und die Umsetzung umfangreiches theoretisches und methodologisches Wissen erfordert. Bisher scheint der Einsatz qualitativer Methoden in der deutschen Suchtforschung nicht ausreichend methodisch untermauert zu sein. Wir fordern daher eine fundierte, reflektierte und methodisch korrekte Nutzung qualitativer Methoden im Feld der Suchtforschung. Ziel: Ziel dieses Artikels ist, anhand von Methodenliteratur und Originalarbeiten, in denen qualitative Methoden angewandt wurden, aufzuzeigen, wie qualitative Forschungsdesigns verstanden werden können, um Forschende zu ermutigen und zu befähigen, qualitative Methoden in angemessener Weise zu nutzen. In diesem Artikel werden mögliche Einsatzgebiete und grundlegende methodologische Überlegungen für die Entwicklung eines qualitativen Forschungsdesigns dargestellt. Dabei wird zusätzlich auf diverse Erhebungsmethoden sowie ethische Aspekte im Rahmen der Datengenerierung eingegangen.
