Terveydenhuollon ammattilaisten kokemuksia potilastietojärjestelmistään on maailmanlaajuisesti kartoitettu vähän, vaikka käyttäjäkokemukset ovat keskeinen osa järjestelmien kehitystä ja arviointia. Sairaanhoitajien kokemusten valtakunnallinen kartoitus tehtiin ensimmäistä kertaa Suomessa alkuvuodesta 2017. Kohteena olivat työikäiset sairaanhoitajat, terveydenhoitajat ja kätilöt Sairaanhoitajaliiton ja Tehyn jäsenrekisteristä. Vastaava tutkimus on tehty lääkäreille vuosina 2010, 2014 ja 2017. Menetelmänä oli lääkärikyselyn pohjalta muokattu sähköinen kyselylomake, jonka Sairaanhoitajaliitto ja Tehy lähettivät jäsenilleen. Yhteensä 3607 sairaanhoitajaa vastasi kyselyyn. Vastaukset ryhmiteltiin Sote-tieto hyötykäyttöön -strategian aihealueen ”Kyvykkäille käyttäjille fiksut järjestelmät” tavoitteiden mukaisiin osa-alueisiin. Kaikissa osa-alueissa todettiin eroja sekä potilastietojärjestelmien tuotemerkkien että vastaajien toimintaympäristöjen välillä. Yksi keskeisimpiä korjattavista ongelmista oli saman asian kirjaaminen moneen paikkaan. Sen voidaan katsoa altistavan virheille ja osoittavan huonosti onnistunutta tietojärjestelmäintegraatiota. Kehittämiskohteiksi nousivat myös tietojärjestelmien käyttökatkot, jotka on aiemmassa kirjallisuudessa todettu riskiksi potilasturvallisuudelle. Tutkimuksessa löytyi eroja tuotemerkkien välillä tietojärjestelmien käytettävyydessä ja potilaan hoidossa tarvittavan, ajantasaisen ja laadukkaan tiedon saamisessa paikallisesti, alueellisesti ja kansallisesti. Vastaajat kokivat tietojärjestelmien edellyttämien toimintatapojen muutosten käyttökoulutuksen riittämättömäksi. Tietojärjestelmien kehittämistyöhön aktiivisesti osallistuvat antoivat muita paremman kouluarvosanan tietojärjestelmälleen. Tätä selvitetään tarkemmin erillisessä artikkelissa. Osallistuminen tietojärjestelmien kehittämistyöhön ja koulutus vievät alkuun aikaa asiakas- ja potilastyöltä, mutta hyvän käytettävyyden ja käyttötaidon voidaan myöhemmin odottaa säästävän ammattilaisten aikaa tietojärjestelmien käytöltä suoraan asiakas- ja potilastyöhön. Tunnistetut erot potilastietojärjestelmien käytettävyydessä ja tuessa työlle sekä hoidon laadulle auttavat tunnistamaan hyviä käytäntöjä eri toimintaympäristöissä sairaanhoitajan ajankäytön tehostamiseksi ja potilasturvallisuuden ja yhteistyön lisäämiseksi. Sairaanhoitajia on kuultava paremmin tietojärjestelmiä kehitettäessä. Organisaatioiden on syytä suunnitella tietojärjestelmäkoulutukset niin, että ammattilaisten osaaminen pysyy ajan tasalla tietojärjestelmien ja niiden käyttötapojen kehittyessä. Myös sairaanhoitajaliiton sähköisten terveyspalveluiden strategia korostaa digiosaamista ja kehitystyöhön osallistumista.
Aim To formulate, validate, and disseminate policy, modelling nurses' career pathway from registered to advanced practice nurse. Method The evidence‐informed policy and practice pathway framework was utilized. Multiple methods were used, including scoping review of literature, consultation of key informants, survey study, and expert group round‐table discussions during 5‐year project between 2013 and 2018. Results Through (a) sourcing, (b) using, and (c) implementing the evidence, the expert group worked systematically to formulate a policy on a career pathway from registered to advanced practice nurse. The formulated career pathway includes three competence levels: registered nurse, specialized nurse, and advanced practice nurse, which includes the roles of nurse practitioner and clinical nurse specialist. In addition, validation and dissemination of the policy, as well as its effective implementation and the process of integrating it into practice, were examined. Conclusion Evidence‐informed policymaking is an effective, interactive way to work collaboratively in achieving consensus and translating knowledge into practice. The formulated policy will contribute to the increased awareness, acknowledgement, and implementation of the registered nurses' traditional and new roles within health care environments. Implementing and integrating the policy in national health care policy, legislation, education, and organizations across the country is a work in progress.
Nurses' collegiality and its content are well recognized in clinical practice but seldom studied. Collegiality can be supported by guidelines, and nurses working in clinical practice, together with teachers and managers, have shared responsibilities to support and develop it. More research in different nursing environments is needed to improve understanding of the content and practice of nursing collegiality.
TiivistelmäVuodesta 2012 hoitotyön tiedonhallinnan erityispätevyysnimike on voitu myöntää Sairaanhoitajaliiton jäsenelle, jolla on riittävää näyttöä perustutkinnon jälkeisestä erityispätevyysalueeseen liittyvästä koulutuksesta, työkoke-muksesta sekä julkaisu-, opetus-ja ohjaus-sekä kehittämistoiminnasta. Myöntämiskriteereitä päivitetään joiltain osin keväällä 2017 vastaamaan muiden ammatillisten yhteistyöjärjestöjen kriteereitä sekä tämän päivän hoitotyön tiedonhallinnan ja tietotekniikan sisältöjä. Tiedonhallinnan erityispätevyys on tähän mennessä myönnetty viidelle sairaanhoitajalle eri puolilla Suomea. Tämän artikkelin tarkoituksena on esitellä erityispätevyysjärjestelmää sekä kertoa pätevyyden saaneiden henkilöiden kokemuksia hakuprosessista, nimikkeen vaikutuksista työtehtäviin sekä ideoista järjestelmän edelleen kehittämiseksi. Avainsanat: asiantuntijuus, hoitotyön tiedonhallinta, sairaanhoitajat AbstractSince 2012 nursing specialty certificate in Nursing Informatics (NI) has been available for members of the Finnish Nurses Association (FNA). The applicant must be a RN and a member of FNA. The evaluation is based on a portfolio which the applicant must draw up as a proof of the accomplishments in her/his post-degree career in three categories: work experience, formal education and cooperation and developmental activities relating to nursing informatics specialty.Awarding criteria will be updated in the spring 2017 to meet the other criteria of professional cooperative organizations, as well as nursing informatics and information technology in today's nursing content. Nursing informatics specialty certificates have been awarded to five nurse. This article aims to introduce the certification system as well as to tell experiences of nurses with the NI specialty certificate, their experiences of the application process, effects on duties and ideas for further development of the certificate system.
