BackgroundVisceral leishmaniasis (VL) is a severe disease caused by infection with protozoa of the genus Leishmania. Classic VL is characterized by a systemic infection of phagocytic cells and an intense activation of the inflammatory response. It is unclear why 90% of infected individuals do not develop the disease while a minority develop the classical form. Furthermore, among those that develop disease, a small group progresses to more severe form that is unresponsive to treatment. The presence of inflammatory mediators in serum could theoretically help to control the infection. However, there is also a release of anti-inflammatory mediators that could interfere with the control of parasite multiplication. In this study, we took advantage of the spectrum of outcomes to test the hypothesis that the immune profile of individuals infected with Leishmania (L.) infantum is associated with the development and severity of disease.Methodology/Principal FindingsSera from patients with confirmed diagnosis of VL were evaluated for the presence of numerous molecules, and levels compared with healthy control and asymptomatic infected individuals.Conclusions/Principal FindingsAlthough differences were not observed in LPS levels, higher levels of sCD14 were detected in VL patients. Our data suggest that L. infantum may activate the inflammatory response via CD14, stimulating a generalized inflammatory response with production of several cytokines and soluble molecules, including IFN-γ, IL-27, IL-10, IL-6 and sCD14. These molecules were strongly associated with hepatosplenomegaly, neutropenia and thrombocytopenia. We also observed that IL-6 levels greater than 200 pg/ml were strongly associated with death. Together our data reinforce the close relationship of IFN-γ, IL-10, IL-6, TNF-α and IL-27 in the immune dynamics of VL and suggest the direct participation of sCD14 in the activation of the immune response against L. infantum.
RESUMO Para avaliar se creches são ambientes protetores ou propiciadores de infestação intestinal, foi feito exame coproparasitológico de crianças de creche e grupo controle. Creche relacionou-se à maior prevalência de parasitoses (63% x 41,4 % ; p<0,01)As parasitoses intestinais estão intimamente relacionadas às condições sanitárias e representam um importante problema de saúde pública nos países subdesenvolvidos 4 . As crianças são as mais acometidas, podendo a maior prevalência de parasitas intestinais levar a déficit nutricional e do crescimento pôndero-estatural 7 .Em função da maior urbanização e maior participação feminina no mercado de trabalho, as creches passaram a ser o primeiro ambiente externo ao doméstico que a criança freqüenta, tornando-se potenciais ambientes de contaminação 1 2 3 9 .Este trabalho se propôs a verificar a prevalência de infestação intestinal por parasitas nas creches públicas da cidade de Aracaju, Nordeste do Brasil, e se esses estabelecimentos são protetores ou expositores para tal infestação.Foi realizado um estudo com seleção de dois grupos. Um grupo de estudo (creche) e um grupo controle para determinar a prevalência de enteroparasitoses intestinais, na população de crianças usuárias de creches públicas de Aracaju, Nordeste do Brasil durante o ano de 2003. Foram estudadas dez creches, sendo sorteadas duas em cada um dos 5 distritos sanitários de Aracaju, com o objetivo de se conseguir uma distribuição mais abrangente da população da cidade. Foram estudadas 26 crianças em cada creche selecionada escolhidas de forma aleatória. Para o grupo controle foram selecionadas crianças não freqüentadoras de creche, de mesma faixa etária (1 a 5 anos de idade, mediana dos dois grupos 3,7 anos), moradoras das vizinhanças das crianças índice (a mais próxima encontrada independente do gênero), a fim de comparar a prevalência de enteroparasitoses entre os dois grupos. O tamanho da amostra foi calculado para um nível de confiança de 95%, poder do teste de 80%, relação 1:1 creche e não creche, prevalência estimada de 40% na população infantil e 30% na população de creche, que resultou no total de 260 crianças para cada grupo.Foram aplicados questionários padronizados para avaliar as condições sócio-econômicas familiares e as características estruturais das creches estudadas. The identification of early markers that predict the response to anti-tuberculosis treatment would facilitate evaluation of new drugs and improve patient management. This study aimed to determine whether selected acute phase proteins and micronutrients measured at the time of diagnosis and during the first weeks of treatment could predict treatment responses during the 2-month standard intensive phase of therapy. For this purpose, alpha1-antitrypsin, alpha1-acid glycoprotein, alpha2-macroglobulin, C-reactive protein, C3, C4, zinc, copper and selenium concentrations were measured in Brazilian patients with smear-positive tuberculosis at the time of diagnosis and 1, 3, 5 and 8 weeks after initiation of therapy. Patients were classified into fast (n=29), intermediate (n=18) and slow responders (n=10) if they were smear-negative at 3, 5 or 8 weeks of treatment. alpha1-acid glycoprotein on enrolment and 1 week of treatment, alpha1-antitrypsin at week 1 and C-reactive protein and C3 after 3 weeks of therapy were higher in slow responders than in fast responders. alpha1-antitrypsin and alpha1-acid glycoprotein may be helpful in predicting treatment response at the time of initiation of therapy, and could be used as early markers to identify patients with an increased likelihood of treatment failure.
Introduction: We evaluated clinical and epidemiological characteristics of Trypanosoma cruzi infection in Sergipe. Methods: In this cross-sectional study, we collected serum samples to identify serological markers of Chagas disease. A questionnaire was used, and electrocardiogram, echocardiogram, chest radiography, and contrast radiography of esophagus and colon were performed. Results: T. cruzi infection seroprevalence was 12.1%, mean age of subjects was 55 years, 90% had an elementary school education, 78.6% were agriculture workers, and 60.5% had electrocardiographic abnormalities. Conclusions: A high prevalence of T. cruzi infection was observed in mostly elderly individuals.
RESUMO -Objetivo: Determinar a freqüência, as causas, o sítio específico e as manifestações clínicas e laboratoriais das infecções em crianças e adolescentes após a implantação de dispositivos neurológicos (DVP) no Hospital Governador João Alves Filho (Aracaju SE). Método: Estudo prospectivo, observacional, não controlado de 50 pacientes, submetidos a DVP (58 procedimentos), no período de janeiro de 2003 a outubro de 2004. Resultados: Observaram-se taxas de infecção por procedimento de 27,6%, taxas de infecção de índice cirúrgico zero, 1 e 2 de 25,7% e 30,4%, respectivamente (NNIS-CDC). A infecção de sítio cirúrgico foi a principal complicação com 50% das infecções. Conclusão: Taxa de infecção por procedimento, paciente e índice de risco cirúrgico mostraram-se elevadas. Não houve significância estatística com relação à idade, etiologia da hidrocefalia, ao tipo de procedimento (derivação primária e reinserção), tempo de internação pré-operatória, duração da cirurgia, antibioticoprofilaxia, cateter SNC prévio e índice de risco cirúrgico.PALAVRAS-CHAVE: hidrocefalia, derivação ventrículoperitoneal, infecção em sistema de derivação ventrículo-peritoneal. Ventriculoperitoneal shunt infections in children and adolescents with hydrocephalusABSTRACT -Objective: To determine frequency, etiology, site and clinical and laboratory findings of ventriculoperitoneal shunt (VPS) infections in children and adolescents with hydrocephalus managed in Hospital Governador João Alves Filho, Aracaju SE, Brazil. Method: A non-controlled prospective observational study comprising 50 patients that underwent VPS (58 procedures) from January/2003 to October/2004. Results: Infection rate per procedure was 27.6%; surgical risk index (NNISS-CDC) 0 and 1-2 were 25.7% and 30.4% respectively; surgical site infection was the main complication with 50% of the cases. Conclusion: Infection rates per procedure, per patient, and per surgical risk index were high. No statistical differences were found related to the following: age, etiology of hydrocephalus, type of procedure, pre-operative length of stay, duration of procedure, antibiotic prophylaxis, previous central nervous system catheter, and surgical risk index.
