Objective: to verify the association between the multimorbidity of the elderly and sociodemographic variables, self-perception of health and polypharmacy. Method: a cross-sectional study was performed. The research data was collected using the Health, Well-Being and Aging questionnaire. The sample was composed of 676 people aged 60 years or more, who were residents of small towns in the north of the state of Rio Grande do Sul, Brazil. The dependent variable was multimorbidity, that is, the occurrence of two or more chronic non-communicable diseases in the same person. The independent variables were demographic, socioeconomic and health-related characteristics. Poisson's raw and robust regression model was used to analyze the effect of the independent variables in relation to the outcome and p was considered significant when <0.05. Result: among the elderly interviewed, 45% presented multimorbidity, 51.1% reported a selfperception of poor/very poor health and 37.1% used polypharmacy. After the analysis was adjusted to the occurrence of multimorbidity, association with the following variables was found: health perception (regular/poor/very poor) PR=1.15 (CI95%; 1.09 -1.22) and use of polypharmacy PR=1.29 (CI95%; 1.22 -1.35). Conclusion: Multimorbidity may interfere negatively in the self-perception of health of the elderly contributing to increased medicine consumption.
ResumoObjetivou-se avaliar a prevalência e os fatores associados a dependência funcional de idosos longevos residentes em municípios de pequeno porte. Trata-se de um estudo transversal de base populacional com idosos de 80 anos e mais, recorte dos estudos sobre as condições de saúde de idosos dos municípios de Coxilha-RS e Estação-RS. Os dados foram coletados por meio de questionário adaptado da pesquisa Saúde, Bem-Estar e Envelhecimento (SABE). Participaram 77 idosos longevos (63,3% mulheres). A prevalência de dependência funcional foi 26% (IC 95% 16,4) e dentre as atividades básicas da vida diá-ria (ABVD) deitar e levantar da cama (31,6%) e locomover-se (31,2%) representaram a maior frequência de dependência. Na análise de regressão multivariada mostraram-se associadas à dependência funcional a autopercepção de saúde negativa (RP= 1,20; IC 95% 1,05-1,37), não praticar atividade física (RP= 1,20; IC 95% 1,05-1,36) e possuir três ou mais doenças crônicas (RP= 1,24; IC 95% 1,08-1,41). Nesse sentido, observamos que os idosos longevos com a condição de dependência possuem como razão de prevalência fatores passíveis de prevenção, e sobretudo que essa condição é evitável ou possível de ser postergada. BACKGROUND AND OBJECTIVES: Chronic pain is a problem for the elderly, eliciting physical and psychological limitations. Few are the studies primarily focusing on evaluating pain in men. This study aimed at identifying the prevalence of chronic pain in male elderly people of a city of Northern Rio Grande do Sul. METHODS: Crossover, population-based study carried out with people aged 60 years or above, from the city of Coxilha, state of Rio Grande do Sul. Data were collected by home interviews with validated tool from June to July 2010. Descriptive statistics was used for analysis. RESULTS: Mean age was 68.7 years. Chronic pain prevalence was 43.7%, primarily located in lower limbs and lumbar spine. CONCLUSION: There has been high prevalence of chronic pain in the studied population. Results point to the need for intervention strategies to prevent and control pain in elderly males in different health care levels.
Resumo Objetivo Verificar a prevalência e os fatores associados à polifarmácia excessiva em pessoas idosas institucionalizadas. Método Estudo transversal com 478 pessoas idosas residentes em instituições de longa permanência para idosos. A variável dependente foi polifarmácia excessiva, definida como o uso concomitante de dez ou mais medicamentos. As variáveis independentes incluíram informações sociodemográficas e de saúde. Utilizou-se a regressão de Poisson com variância robusta para analisar o efeito das variáveis independentes em relação ao desfecho. Resultados A prevalência de polifarmácia excessiva foi de 29,3%, associada à cardiopatia (RP=1,40; IC95% 1,03-1,91), diabetes mellitus (RP=1,52; IC95% 1,15-2,01), depressão (RP=1,42; IC95% 1,08-1,87), internação hospitalar no último ano (RP=1,36; IC95% 1,02-1,80) e ao uso de medicamento potencialmente inapropriado para idosos (RP=2,13; IC95% 1,60-2,83). Conclusão A polifarmácia excessiva foi frequente entre pessoas idosas institucionalizadas. Os resultados sugerem que as doenças prevalentes entre pessoas idosas, a hospitalização e o uso de medicamentos potencialmente inapropriados são fatores para o uso de polifarmácia excessiva nessa população. Esses achados podem instruir ações com vistas à otimização da farmacoterapia prescrita às pessoas idosas.
Background Polypharmacy is recognized as a global public health problem and one of the greatest challenges related to the aging population. Few studies have investigated the incidence and risk factors for polypharmacy among elderly individuals. These studies provided important information on the issue but were developed in high-income countries. This study investigates the incidence and risk factors for polypharmacy among elderly people assisted by primary health care over a period of 11 years. Methods This was a census-based prospective longitudinal study that included people aged 60 years or older living in a small municipality in the state of Rio Grande do Sul, Brazil. The baseline occurred in 2010 and the second wave of the study occurred in 2021. The study population consisted of elderly individuals who did not use polypharmacy at baseline and were reinterviewed in 2021 (N = 128). Data collection in the first and second waves was performed through a household survey using a structured questionnaire. The dependent variable was polypharmacy, defined as the simultaneous use of 5 or more drugs. The independent variables included sociodemographic, health and functionality factors. For multivariate analyses, Poisson regression with robust variance was used, estimating the relative risk and 95% confidence intervals. Results The incidence of polypharmacy was 46.1% in the 11-year period. The highest number of health problems was a risk factor for polypharmacy (RR = 1.177; 95% CI 1.093–1.267). Conclusions The incidence of polypharmacy among elderly people assisted in primary health care in Brazil is high. The number of diseases is a risk factor for polypharmacy. These results have implications for future primary health care practices and may support the development of policies, actions and services aimed at reducing polypharmacy and promoting the rational use of drugs in the population at higher risk.
RESUMO Objetivo: avaliar as condições visuais autorrelatadas e sua relação com a ocorrência de quedas, em idosos do meio rural, de um município de pequeno porte, do estado do Rio Grande do Sul, Brasil. Método: Trata-se de um estudo transversal de base censitária. A população do estudo foi composta por 140 idosos residentes no meio rural. A coleta de dados foi feita por inquérito domiciliar utilizando-se uma adaptação do instrumento da pesquisa Saúde, Bem-Estar e Envelhecimento. Para análise empregou-se a estatística descritiva e analítica. Resultados: Os idosos do estudo apresentaram média de idade de 69,9 anos, eram na maioria homens (52,1%), referiram problemas de visão (88,6%) e tiveram quedas no último ano (22,9%). Não houve associação entre déficit visual e quedas. As quedas foram mais frequentes entre as mulheres (29,9%), na faixa etária de 80 anos e mais (29,2%), para os viúvos, separados e solteiros (25,7%) e entre os que possuíam renda de até dois salários mínimos (26,5%). Houve associação entre quedas e menor renda (p<0,05). Conclusão: Os resultados do estudo apontam para a necessidade da implementação de ações e estratégias voltadas à identificação precoce de problemas visuais e de outros fatores de risco para quedas em idosos rurais, bem como, para a adoção das medidas necessárias à sua minimização.
