The purpose of this prospective randomised double‐blind study was to determine the effective dose of propofol required for the successful first attempt insertion of the laryngeal tube compared with the laryngeal mask airway in patients co‐induced using alfentanil 5 μg.kg–1, undergoing short elective gynaecological procedures. The first patient in each group received propofol 2.5 mg.kg−1 for induction. In accordance with Dixon's up‐and‐down method, the dose of propofol for consecutive patients in each group was varied with increments or decrements of 0.5 mg.kg−1 based on the previous patient ‘all‐or‐none’ purposeful movement response to first attempt of insertion of the randomised device. The ED50 (SD) of propofol was 2.66 (0.86) mg.kg−1 and 2.33 (0.37) mg.kg−1 for the laryngeal tube and laryngeal mask patients, respectively, which did not reach statistical significance (p = 0.40). We conclude therefore that the insertion of the two airway devices requires similar bolus doses of propofol when alfentanil is used as the co‐induction drug.
This study was designed to evaluate the variations in decision making among Brazilian oral and maxillofacial surgeons (OMFS) and trainees in relation to third molar surgery. A survey on 18 diverse clinical situations related to the assessment and treatment of the third molar surgeries was conducted during the 20th Brazilian National OMFS meeting. Participants were divided into three groups according to their level of training. Another variable studied was length of experience. Correlation between the question answers and the variables was analysed using the chi-square test and the f test. The mean age of participants was 32.68 years, and their mean length of experience was 5.24 years. There were no statistical differences between the level of training and number of years of experience and the responses to 15 of the 18 questions on clinical situations. However, differences were found in responses to prophylactic extraction of asymptomatic third molars, use of non-steriodal anti-inflammatory drugs (NSAIDs) during the preoperative surgical period and the use of additional imaging to plan extractions. The group with shorter time of experience (3.8 ± 3.94 years) tended to recommend extractions of asymptomatic third molars more frequently compared with the more experienced surgeons (P = 0.041). More experienced surgeons used NSAIDs in the preoperative surgical period, whereas the majority of the youngest surgeons (4.1 ± 5.96 years of experience) did not (P = 0.0042). The certificated trained and in practice group tended to treat deep lower third molar impactions based on the findings of a panoramic radiograph, without obtaining additional imaging [cone beam computed tomography (CBCT)] before treatment (P = 0.0132). Decision making regarding third molar treatment differs according to the level of training and is influenced by the number of years of experience. Therefore, further continuous education programmes in this area are warranted to make recommendations regarding third molars consistent with the current literature.
A lack of technical information was observed among teachers regarding management of tooth avulsion. Educational campaigns at schools are necessary to modify the behavior of the teachers with regard to management of tooth avulsion. Further studies should be carried out for the assessment of teachers who have participated in educational campaigns to make the treatment of dental trauma a matter of public interest.
A COVID-19 é uma infecção altamente contagiosa e rapidamente se tornou uma pandemia. Um dos desafios enfrentados é o diagnóstico, visto que os métodos utilizados possuem limitações, como a dependência de insumos importados. O objetivo desse estudo foi divulgar ações e estratégias de universidades e institutos de pesquisa para o diagnóstico da COVID-19 no Brasil. Deste modo, foi realizada uma busca de patentes no INPI e de notícias em sites oficiais no período de 25 de fevereiro a 10 de maio de 2020. Além disso, uma análise comparativa das iniciativas foi realizada. Neste estudo não foram encontradas patentes relacionadas ao tema, porém 88 iniciativas abordando diagnósticos molecular e imunológico, além de métodos alternativos foram selecionadas. Por meio da análise das iniciativas, foi possível demonstrar a relevância e aplicabilidade destas ações dentro do contexto da pandemia, visto que o diagnóstico se tornou uma estratégia fundamental para o controle da COVID-19.
O objetivo foi avaliar o impacto da implementação do serviço de residência em cirurgia e traumatologia buco-maxilo-facial em um hospital público em Campina Grande–PB, Brasil. Foram analisados 516 prontuários de pacientes operados no Hospital de Emergência e Trauma Dom Luiz Gonzaga Fernandes (HETDLGF), no período de março de 2017 a fevereiro de 2018 (período sem o serviço de residência) e março de 2018 a fevereiro de 2019 (1º ano do serviço de residência). Os resultados foram compilados em um banco de dados organizado em planilhas do Microsoft Excel® e posteriormente analisados no software SPSS® for Windows 25.0. Dentre os 516 pacientes, 236 foram operados no ano antes da residência (média de 19,6 cirurgias mensais) e 280 pacientes foram operados no ano da residência (média de 23,3 cirurgias por mês). Observa-se um acréscimo de 44 cirurgias, equivalendo a mais de dois meses de cirurgias a mais em relação ao ano antecessor à implementação da residência. O tempo médio de espera pelo paciente para a cirurgia diminuiu em 2,5 dias e o tempo médio de internação hospitalar em 2,6 dias. O hospital obteve uma economia média de R$ 1.102,58 por internação, em comparação aos anos anteriores. Da amostra total, 416 (80,6%) eram do sexo masculino e 100 (19,4%) eram do sexo feminino. Conclui-se que a implementação de um serviço de residência em um hospital público promove um impacto positivo, reduzindo o tempo de espera do paciente pelo tratamento cirúrgico, o tempo de internação hospitalar e consequentemente, os custos financeiros ao hospital.
Introdução: Corpos estranhos intraoculares são a maior causa de lesões traumáticas em órbita, possuem características diversas, que abrangem do edema palpebral ao trauma órbito craniano, resultando em sequelas graves e algumas vezes fatais. Objetivo: O objetivo do presente estudo é apresentar uma série de casos de trauma por corpo estranho penetrante na órbita ocular associado a uma revisão da literatura sobre o tema. Desenho: Estudo retrospectivo de casos. Metodologia: Revisão de acordo com a declaração PRISMA, por meio das bases de dados PubMed, Science Direct e SciELO, além dos relatos de casos e análise retrospectiva de 12 pacientes submetidos à diferentes manejos. Resultados: A idade média dos pacientes foi de 31,16 anos, sendo 91,6% dos pacientes do sexo masculino, a etiologia em 50% dos casos foi por material de madeira, em 41,7% foi por metal e em 8,3% por vidro. A tomografia computadorizada foi utilizada em todos os casos fornecendo resultados altamente efetivos. Em 58,3% dos casos ocorreram na órbita esquerda. Cinquenta por cento dos casos tratados evoluíram sem sequelas. Conclusão: O diagnóstico precoce e manejo imediato de lesões orbitárias são imprescindíveis para um prognóstico positivo, assim como exames de imagem adequados. A tomografia computadorizada, a ressonância magnética e a angiografia por tomografia computadorizada são modalidades de imagem frequentemente utilizadas e eficazes.
