Objective Tertiary hyperparathyroidism, an autonomous hyperproduction of parathyroid hormone (PTH), has a challenge in its treatment. This study asked whether the intraoperative PTH and calcium drop can confirm the resection of all parathyroid tissues. Study Design Case series with planned data collection. Setting Tertiary referral medical center. Subjects and Methods The study assessed patients with tertiary hyperparathyroidism who were treated at the Hospital of the State University of Campinas from 2007 to 2015. All patients underwent total parathyroidectomy with autotransplantation of parathyroid fragments. PTH and calcium were collected during the preoperative period; at 10, 20, and 240 minutes after resection of the glands; and at 1 year after the procedure. Data were analyzed by analysis of variance and logistic regression analysis with statistical values of P < .05. Results Thirty-five patients were assessed: 17 women (48.57%) and 18 men (51.43%). The percentage of PTH drop was statistically significant at all times, unlike the calcium analysis, but only PTH collected at 20 minutes was able to confirm the removal of all parathyroid tissues ( P = .029). Based on the receiver operating characteristic curve, the 71.2% drop obtained high sensitivity and specificity ( P = .028). Conclusions Treatment success can be predicted by analyzing the decrease of intraoperative PTH and not by calcium. The 71.2% PTH drop at 20 minutes after parathyroidectomy had high sensitivity and specificity to predict surgical cure.
The poor prognosis of head and neck cancer (HNC) is associated with metastasis within the lymph nodes (LNs). Herein, the proteome of 140 multisite samples from a 59-HNC patient cohort, including primary and matched LN-negative or -positive tissues, saliva, and blood cells, reveals insights into the biology and potential metastasis biomarkers that may assist in clinical decision-making. Protein profiles are strictly associated with immune modulation across datasets, and this provides the basis for investigating immune markers associated with metastasis. The proteome of LN metastatic cells recapitulates the proteome of the primary tumor sites. Conversely, the LN microenvironment proteome highlights the candidate prognostic markers. By integrating prioritized peptide, protein, and transcript levels with machine learning models, we identify nodal metastasis signatures in blood and saliva. We present a proteomic characterization wiring multiple sites in HNC, thus providing a promising basis for understanding tumoral biology and identifying metastasis-associated signatures.
Background: Oral cavity cancer is still an important public health problem throughout the world. Oral squamous cell carcinomas (OSCCs) can be quite aggressive and metastatic, with a low survival rate and poor prognosis. However, this is usually related to the clinical stage and histological grade, and molecular prognostic markers for clinical practice are yet to be defined. Heparanase (HPSE1) is an endoglycosidase associated with extracellular matrix remodeling, and although involved in several malignancies, the clinical implications of HPSE1 expression in OSCCs are still unknown.Methods: We sought to investigate HPSE1 expression in a series of primary OSCCs and further explore whether its overexpression plays a relevant role in OSCC tumorigenesis. mRNA and protein expression analyses were performed in OSCC tissue samples and cell lines. A loss-of-function strategy using shRNA and a gain-of-function strategy using an ORF vector targeting HPSE1 were employed to investigate the endogenous modulation of HPSE1 and its effects on proliferation, apoptosis, adhesion, epithelial–mesenchymal transition (EMT), angiogenesis, migration, and invasion of oral cancer in vitro.Results: We demonstrated that HPSE1 is frequently upregulated in OSCC samples and cell lines and is an unfavorable prognostic indicator of disease-specific survival when combined with advanced pT stages. Moreover, abrogation of HPSE1 in OSCC cells significantly promoted apoptosis and inhibited proliferation, migration, invasion, and epithelial–mesenchymal transition by significantly decreasing the expression of N-cadherin and vimentin. Furthermore, a conditioned medium of HPSE1-downregulated cells resulted in reduced vascular endothelial growth.Conclusion: Our results confirm the overexpression of HPSE1 in OSCCs, suggest that HPSE1 expression correlates with disease progression as it is associated with several important biological processes for oral tumorigenesis, and can be managed as a prognostic marker for patients with OSCC.
RESUMOO melanoma da conjuntiva (MC) é um tumor que pode acometer a conjuntiva bulbar próxima ao limbo, conjuntiva palpebral, forniceal, na plica ou na região da carúncula, podendo originar a partir de áreas de melanose adquirida primária com atipia, de áreas com nevos conjuntivais, e em locais sem lesões. Clinicamente, apresenta-se como uma massa ou uma lesão conjuntival pigmentada elevada. Este estudo objetiva descrever um caso de melanoma conjuntival multifocal proveniente de nevus pigmentado preexistente. O diagnóstico histopatológico e o estadiamento precoce da lesão conjuntival é de fundamental importância para designar a conduta frente ao paciente. INTRODUÇÃOAs lesões melanocíticas da superfície ocular representam um amplo espectro de condições, desde as benignas a malignas, como o melanoma da conjuntiva, que apesar de raro, representa a segunda lesão maligna conjuntival mais frequente depois do carcinoma de células escamosas, corresponde a apenas de 2% dos melanomas oculares e menos de 1% de todos os tumores malignos do olho. 1,2O melanoma da conjuntiva (MC) é um tumor que pode acometer a conjuntiva bulbar próxima ao limbo, conjuntiva palpebral, forniceal, na plica ou na região da carúncula, podendo originar a partir de áreas de melanose adquirida primária com atipia, constituindo 75% dos casos, de áreas com nevos conjuntivais em cerca de 20% dos casos, e em locais sem lesões prévias (5% dos casos).3 É mais frequente em indivíduos idosos com idade média variando de 55 a 70 anos, caucasianos e com maior exposição aos raios ultravioleta. [4][5][6] Os tumores originados na conjuntiva bulbar tem o melhor prognóstico, com uma sobrevida aos cinco anos de 100%. Essa taxa cai para 80% com os tumores de limbo e para 50% com os originados na conjuntiva palpebral (prognóstico desfavorável pelo atraso no diagnostico, em virtude da localização). 7,8 Clinicamente, o MC pode ter diversos tipos de apresentação. RESUMOO carcinoma mucoepidermoide (CME) é considerado o tumor maligno mais comum das glândulas salivares e representa de 3 a 5% de todos os tumores das glândulas. Pelo seu comportamento agressivo, deve ser considerado entre as hipóteses diagnósticas em regiões onde possam ter glândulas salivares. Diagnóstico precoce e manejo tumoral adequado são prioridades para sobrevida, qualidade de vida e até cura. Este trabalho relata um caso de CME de palato duro em criança de 12 anos. O diagnóstico precoce, a avaliação intraoral multidisciplinar e o tratamento cirúrgico foram fundamentais para determinar a boa evolução em pacientes com CME de glândula salivar menor. INTRODUÇÃOOs tumores de glândulas salivares menores são incomuns, representando de 3 a 5% de todas as neoplasias de cabeça e pescoço e, destes, 10 a 15% são de origem em glându-las salivares menores.1 Os tipos histopatológico mais comum são o carcinoma adenoide cístico (CAC) e o carcinoma mucoepidermoide (CME).2 São tumores que podem acometer desde lábio, seios paranasais e tonsilas até a traqueia e os pulmões.3 Na cavidade oral, acomete especialmente o palato duro, local de maior acometimento na boca. 4O CME, primeiramente descrito em 1945 , tem etiologia desconhecida, porém, tem relação com exposição ao fumo e à radiação e com fatores genéticos.5 A literatura aponta uma prevalência para o gênero feminino, sem haver predileção por gênero. Verifica-se acometimento mais comum entre a segunda e a oitava décadas de vida, com idade média de 50 anos, sem predileção para raça.6 Em crianças, é o tumor mais frequente das glândulas salivares. O CME origina-se de metaplasias nos ductos das glândulas e é considerado como o tumor mais prevalente na população jovem. 7Metástases regionais não são comuns e, clinicamente, manifestam-se com lesões nodulares de coloração violácea, acompanhadas ou não de úlceras dolorosas. Pode associar-se com componente cístico com saída de sangue. O CME costuma ser confundido clinicamente com o mucocele; por isso, a importância de um diagnóstico precoce.
