The study's main objective is to analyze how discourses of medicalization and humanization reconnect in primary healthcare and shape prenatal care for pregnant women provided by family health teams. This was a single and integrated case study with multiple analytical units and a qualitative approach. A total of 17 focus groups were performed, in which 47 health professionals were heard (14 physicians, 19 nurses, and 14 dentists), members of 17 family health teams in 16 municipalities in the South of Brazil. The empirical material was analyzed from the perspective of Foucauldian discourse analysis. The family health teams, adopting general practice, reported difficulties in conducting prenatal care, evoking and bolstering the discourse of obstetric medicalization that their practice should supposedly offset. The discourse officially adopted by humanization, prioritized in the generalist model of prenatal care, continues to function as a complementary discourse to that of medicalization and specialization, which prevails in the practices reported by the teams. The emphasis on humanized care for pregnant women tests the limits of professional territories and assumes the renegotiation of competencies. Efforts at collaboration between the family health teams and obstetricians have not proved very successful in this specific case.
Resumo O artigo apresenta um estudo que teve como objetivo geral conhecer como os profissionais de uma unidade de Estratégia Saúde da Família (ESF) lidam com as demandas relacionadas ao tema do suicídio. Trata-se de uma pesquisa qualitativa com abordagem interpretativa, mediante grupo focal narrativo, realizado com profissionais de uma unidade de ESF, no município de Porto Alegre/RS. Foram identificadas as dificuldades que ocorrem no trabalho com o tema do suicídio, e os efeitos psíquicos que elas provocam nos trabalhadores. As dificuldades em relação ao trabalho incluem: as vulnerabilidades sociais do território, o tabu do suicídio, as dificuldades de manejo dessas demandas pelos profissionais e a insuficiência da rede de serviços. Os efeitos psíquicos identificados nos trabalhadores foram: sentimento de impotência, culpa e frustração, bem como estresse e sensação de sobrecarga de trabalho. As principais conclusões incluem a potencialidade do trabalho em ESF para a prevenção do suicídio, a necessidade de fortalecimento da rede de serviços e a importância da qualificação e do apoio permanentes aos profissionais, buscando ofertar os recursos necessários ao enfrentamento das dificuldades relacionadas ao trabalho com essa temática.
Introduction: The Family Doctor (FD), whose scope of action is Primary Health Care (PHC), is a professional trained to care for people, their families, and communities. The best FD training should be performed through Medical Residency Programs in Family Medicine (MRPFM), an in-service teaching field where a preceptor, also a FD, accompanies the resident. A pedagogically well-trained preceptor can work in the learning process of young doctors, who in turn are able to develop the necessary skills to work as a future FD. Active Learning Methodologies (ALM) are based on critical pedagogy and work with problems for the development of teaching-learning processes. They seek the theory from the practice, thus preparing the students to become aware of their environment and act aiming at its transformation. ALMs are achieved in the pedagogical processes through Active Teaching-Learning Strategies (ATLS), which are effective pedagogical resources used by the preceptors in the residents’ daily training. They can be divided into two groups: Active Teaching-Learning Dynamics and Active Teaching Actions. This article reports on the effects of a training course for Family Medicine preceptors on the use of Active Teaching-Learning Dynamics and Active Teaching Actions. Method: Qualitative comparative case study using sociodemographic survey, individual semi structured interview and field notes. The information was examined under content analysis, having participated ten preceptors from four MRPFM from the state of São Paulo, Brazil (half having attended the Leonardo EURACT level 1 course and half not having done so). Results: Those preceptors who attended the course expressed having more knowledge and use of Active Teaching-Learning Strategies, specifically of Active Teaching-Learning Dynamics and Active Teaching Actions. Conclusions: It was observed that the Leonardo EURACT level 1 teaching technology modifies the teaching practice of those who took the course, as they demonstrate a greater use and domain of its tools in their pedagogical practice. Moreover, the course allows a greater understanding of the presented dynamics and activities, thus promoting the residents’ critical learning and encouraging autonomy.
O presente artigo relata uma pesquisa qualitativa, orientada pelo método dialético-crítico, cujo objetivo central foi identificar como a interconsulta vem sendo realizada nos serviços de atenção primária em saúde (APS) de Porto Alegre, Brasil, tendo em vista a falta de referencial teórico acerca da temática neste campo de atuação e a compreensão do potencial para o seu exercício neste âmbito de atenção, visando à integralidade do cuidado. Profissionais de saúde de serviços de APS preencheram um questionário semiestruturado, elaborado para conhecer seu entendimento sobre esta ação, para quais demandas a realizavam, o fluxo utilizado e os limites e as dificuldades para efetivação desta prática no processo de trabalho. O estudo identificou que a falta de referencial sobre interconsultas em APS não está relacionada com o nível de compreensão dos profissionais, pois a maioria a realiza conforme a produção teórica existente, sendo este espaço entendido como campo especial para o seu exercício. Introduction: The 2014 curricular guidelines for medical courses in Brazil aim to train humanist, critic, reflexive and ethic general practitioners, able to work at different levels of health care, primarily based within the National Unified Health System scope. To achieve this goal, higher education institutions must adopt educational technologies such as active learning methodologies, which encourage students to seek and build their own knowledge. This study aims to identify and analyze the challenges faced by Public Health teachers from two medical courses in the state of Rio Grande do Sul, Brazil in the use of Active Learning Methodologies. Method: The study was conducted as a qualitative research with a comparative case study approach, and the subjects comprised teachers of the abovementioned discipline in both universities. The information was collected through a sociodemographic questionnaire, semi-structured interviews, and semi-directed observation. The findings were assessed using thematic analysis and the questionnaire data were tabulated to assess its frequency. Result: The main findings were the understanding, on the part of the teachers, of the relevance of active methodologies in the teaching pedagogical processes, the difficulty of including them due to the lack of preparation, and the understanding of the need for the active teaching methodologies to guide the pedagogical project. Conclusion: It is concluded that the universities should be encouraged to build a continuing education project for the teaching staff in which these methodologies are assumed as the main point in the teaching practice, also contributing to reflect on the established training model, as well as to rethink the organization of the political pedagogical project of medical courses.
Resumo Este estudo tem como objetivo investigar o que psicólogos na atenção básica de Porto Alegre identificam como necessidades em saúde mental das mulheres atendidas, buscando verificar se sua escuta profissional permite estabelecer nexos entre o sofrimento psíquico e a desigualdade de gênero vivenciada pelas usuárias. Em 2017, foi realizado grupo focal com nove psicólogas que atuam em unidades de Saúde da Família, em que foi construída coletivamente uma narrativa segundo o método de Grupo Focal Narrativo. Constituem a narrativa final oito núcleos argumentais: “Sofrimento mascarado”; “Os homens também sofrem com isso”; “Várias gerações de mulheres cuidadoras”; “Aquele desejo de constituir uma família a qualquer custo”; “Por que o homem acha que pode usar uma mulher como se fosse um objeto?”; “Maternidade compulsória”; “Novas formas de exercício da sexualidade”; e “O hospital tomou como uma afronta o empoderamento da mulher”. Considera-se que o estudo proporcionou às participantes um espaço de reflexão coletiva sobre si mesmas e suas práticas.
O objetivo deste estudo foi pesquisar a utilização de plantas medicinais por idosos diabéticos, hipertensos e polimedicados vinculados à Unidade de Saúde Costa e Silva, Porto Alegre/RS e descrever potenciais interações entre as plantas e os medicamentos utilizados, conforme literatura. Vinte e dois idosos foram entrevistados sobre as plantas medicinais utilizadas, forma e frequência de preparo, local de obtenção e conhecimento sobre possíveis efeitos adversos das plantas. Demonstrou-se que além dos medicamentos prescritos estes idosos têm como prática de cuidado o uso de chás medicinais, destacando-se a marcela (Achyrocline satureioides) e a camomila (Matricaria chamomilla). A prática é exercida através de conhecimentos aprendidos nas famílias e predomina a noção de que as plantas medicinais não oferecem riscos à saúde. Contudo, a maioria das espécies mencionadas possui descrição de potenciais interações com medicamentos (ex. gengibre e anticoagulantes) e/ou modificações fisiopatológicas que podem ser prejudiciais no contexto de polimedicação e doenças apresentadas pelos entrevistados (ex. alecrim e indução do metabolismo de fármacos; poejo e risco de hepatotoxicidade). Conclui-se necessária a avaliação e orientação sobre o uso de plantas medicinais pelos serviços de saúde na atenção aos idosos polimedicados, levando em consideração a maior vulnerabilidade desta população aos potenciais riscos aqui descritos.
