Método: Estudio transversal llevado a cabo en un hospital de referencia, entre los meses de septiembre a noviembro de 2012. La muestra constó de 50 individuos divididos en dos grupos, siendo 25 cuidadores de niños sometidos a la queiloplastia y 25 niños sometidos a la palatoplastia. Las dudas fueron identificadas por medio de entrevista, realizada durante la consulta de Enfermería preoperatoria y, posteriormente, fueron categorizadas por similaridad. Se utilizó el análisis estadísti-co descriptivo para la construcción de los resultados. Resultados: Se observó que, con respecto a la queiloplastia, las dudas fueron acerca de la alimentación (36%), higienización y cicatrización (24% cada), y dolor e infección (8% cada). En cuanto a la palatoplastia, las dudas estuvieron relacionadas con: la alimentación (48%), higienización (24%), dolor (16%), sangrado (8%) e infección (4%). Conclusión: Se evidenció la preocupación de los cuidadores con respecto a la alimentación y los cuidados con la herida posoperatoria. Objective:To assess the nursing workload (NW) in Semi-intensive Therapy Unit, specialized in the care of children with Craniofacial anomalies and associated syndromes; to compare the amount of workforce required according to the Nursing Activities Score (NAS) and the COFEN Resolution 293/04. Method: Cross-sectional study, whose sample was composed of 72 patients. Nursing workload was assessed through retrospective application of the NAS. Results: The NAS mean was 49.5%. Nursing workload for the last day of hospitalization was lower in patients being discharged to home (p<0.001) and higher on the first compared to last day of hospitalization (p< 0.001). The number of professionals required according to NAS was superior to the COFEN Resolution 293/04, being 17 and 14, respectively. Conclusion: The nursing workload corresponded to approximately 50% of the working time of nursing professional and was influenced by day and outcome of hospitalization. The amount of professionals was greater than that determined by the existing legislation.
Objectives: to understand the psychosocial repercussions experienced by caregiving parents, resulting from care for the child with dysphagic cleft lip and palate. Methods: qualitative study, developed in a tertiary hospital in September 2016. The sample defined by theoretical saturation consisted of seven mothers. Data collection was performed by unstructured interview, being audio-recorded and fully transcribed. Symbolic Interactionism was used as theoretical framework, and Thematic Content Analysis as methodological framework. Results: the following themes emerged: diagnosisimpact and coping; coping with overload and stress; interaction between caregivers as an acceptance and coping strategy; impact on family and social life of caregivers; and curiosity coping, and family and community prejudice. Final considerations: despite the physical and emotional overload, the mother figure plays the main and determining role in care, reflecting the complexity of care.
Portuguese / English: www.scielo.br/reeusp RESUMO Este estudo teve como objetivo conhecer a importância da comunicação durante as orientações pós-operatórias fornecidas pela equipe de enfermagem a pacientes e/ou familiares de uma instituição privada e apreender a percepção destes indivíduos acerca das orientações recebidas. Foi um estudo transversal, descritivo, com abordagem qualitativa, que utilizou o referencial teórico da Comunicação Interpessoal e o referencial metodológico da Análise de Conteúdo. Participaram do estudo 16 pacientes entrevistados no período pós-operatório mediato. Os resultados evidenciaram que a equipe de enfermagem focaliza as orientações nas téc-nicas instrumentais da profissão, não abordando o indivíduo de forma holística. Também foi possível perceber que, quando o profissional enfermeiro permanece afastado do paciente e/ou não presta informações adequadas gera sentimentos de ansiedade, medo, insegurança e sensação de falta de cuidado. Por outro lado, quando a equipe de enfermagem se fez presente com cuidado e informações coerentes, os clientes relataram um alto nível de satisfação e a sensação de ser bem cuidado. DESCRITORES
Objective:To assess the impact of promoting self-care in nursing workload and associate it to the variables: age, gender, socioeconomic status, education, marital status and number of children of caregivers. Methods: Prospective study with 31 children and their caregivers. Participants were assessed at two moments, 1 st and 2 nd hospitalization, the nursing workload was measured by the Nursing Activities Score (NAS). Results: The mean NAS in the 1 st hospitalization was 60.9% and in the 2 nd hospitalization was 41.6%, that is, 14.6 and 9.9 hours of nursing, respectively. The nursing workload on the first day of hospitalization was higher compared to the last day, both for the 1 st (p<0.001) and for the 2 nd hospitalization (p<0.001), and higher in the first (p<0.001) and in the last day (p=0.025) in the 1 st hospitalization. Comparing the 1 st hospitalization to the 2 nd hospitalization, the first was higher (p<0.001), and NAS items related to the training of self-care was influenced (p<0.001). Conclusion: The nursing workload associated to self-care promotion corresponded to 14.6 hours and was higher than determined by the existing legislation.
The psychological and physical health of caregivers of children with cleft lip and palate can be strongly influenced by the child’s condition and behavior, and care required by the treatment. This study’s objective was to identify overload levels among these caregivers. A total of 100 caregivers, whose children were in the perioperative period of cheiloplasty and/or palatoplasty, were interviewed using the Scale Burden Interview. The data were analyzed quantitatively. Moderate and moderate severe burden levels were identified in 43% of the sample. The mothers of children aged around 12 months presented significantly higher overload. The results indicate that children in this developmental stage present greater care demand because they seek autonomy in locomotion, explore objects in their surrounding environment, and continually request communicative and affective-social interaction with their reference to greater attachment (the caregiver), which may have contributed to higher burden.
Objective: To check the knowledge informal caregivers of children with cleft lip and palate acquire about the postoperative care of cheiloplasty and palatoplasty through the use of an educational video. Methods: Randomized clinical trial conducted with 80 caregivers of children with cleft lip and palate, arranged into two groups: experimental (G1) and control (G2). Data collection occurred during the child's hospital stay, in three stages, using a structured questionnaire and an educational video developed and validated for this study.Results: There was evidence of improvement in the acquisition of knowledge about postoperative care in both groups (p <0.001), but it was higher in G1 compared to G2. Conclusion: The educational video was shown to be effective to train caregivers of children with cleft lip and palate after cheiloplasty and palatoplasty. ResumoObjetivo: Verificar o conhecimento adquirido por cuidadores informais de crianças com fissura labiopalatina sobre os cuidados pós-operatórios de queiloplastia e palatoplastia por meio da utilização de um vídeo educativo. Métodos: Ensaio clínico randomizado, realizado com 80 cuidadores de crianças com fissura labiopalatina, dispostos em dois grupos: experimental (G1) e controle (G2). A coleta de dados ocorreu durante o período de hospitalização da criança, em três etapas, utilizando um questionário estruturado e um vídeo educativo construído e validado para o presente estudo. Resultados: Foi evidenciada melhora na aquisição de conhecimento sobre os cuidados pós-operatórios em ambos os grupos (p<0,001), porém, foi maior o G1 em comparação a G2. Conclusão: O vídeo educativo mostrou-se eficaz no treinamento de cuidadores de crianças com fissura labiopalatina em situação pós-operatória de queiloplastia e palatoplastia.
ObjectivesTo calculate and compare the nursing staff size associated with self-care promotion at a pediatric semi-intensive care unit.MethodsThis was a prospective study in which 31 children and their caregivers participated. The nursing workload associated with each participant was evaluated at two different times (first and second hospital stays) using the Nursing Activities Score instrument. The first hospital stay corresponded to self-care promotion. Staff size was calculated according to the nursing hours recommended by the Nursing Activities Score instrument and by Conselho Federal de Enfermagem (COFEN) resolution no. 527/16, in the two hospital stays, and the results were compared.ResultsThe nursing workload in the first hospital stay (14.6 hours) was higher than the nursing workload in the second stay (9.9 hours) (p < 0.001). The Nursing Activities Score revealed that according to the nursing hours, the nursing staff size corresponded to 26 and 18 professionals in the first and second hospital stays, respectively, and to 15 professionals according to COFEN resolution no. 527/16.ConclusionThe number of personnel responsible for promoting self-care in pediatric semi-intensive care units, according to the nursing hours suggested by the Nursing Activities Score, was higher than that recommended by the existing legislation. This demonstrates the necessity of reconsidering staff size for this healthcare profile.
