A identificação e prevenção de interações entre medicamentos estão diretamente relacionadas com a segurança do paciente e são de grande importância no ambiente hospitalar. O objetivo deste trabalho foi identificar e avaliar a existência de interações medicamentosas potenciais na farmacoterapia prescrita. Trata-se de estudo observacional, com 308 prescrições de 195 pacientes internados na Unidade de Terapia Intensiva de um hospital de ensino, realizado de agosto a dezembro de 2015. Para identificação das interações utilizou-se a base de dados Micromedex®. Das 308 prescrições analisadas, 101 apresentaram pelo menos uma interação medicamentosa, totalizando 173 interações, referentes a 71 pacientes. Destes, 43 eram do sexo masculino, com idade média de 61,9 anos. A média de internação na unidade foi de 7 dias. Nas 101 prescrições que apresentaram interações medicamentosas, foi prescrito o total de 1.349 medicamentos com média de 13,3 medicamentos/paciente/dia. Identificar essas interações possibilita evitar situações de insucesso terapêutico e toxicidade medicamentosa.
The aims of this study were to investigate the use of psychotropic drugs and factors associated with the use among health personnel at a hospital in Santa Cruz do Sul, RS factors. It was a cross-sectional study and the data collection was performed through a self-administered, unidentifiable and structured questionnaire with open and closed questions, between September and October 2012. 106 professionals were interviewed, 91.5% women, 74.5% nursing and 40.57% working in inpatient units. About the participants, 23 (21.7%) were using psychotropics, with higher consumption, among professionals working in Intensive Care Units (34.78%) as well as in inpatient units (34.78%). These workers had over 60 hours weekly working (50%), including cases of more than one bond employment. It could be observed a higher prevalence of the consumption of psychotropic medications among nursing staff (22.8%). The main reasons for the early use of drugs studied were depression, anxiety and insomnia. The lack of knowledge about the characteristics of medicines and treatment were also observed. The results indicate a high use of psychotropic drugs among health professionals and that there is a need for more communication between prescribers and users, and the search for pharmaceutical care in order to clarify doubts regarding treatment, for proper rational use of medications.Keywords: psychotropic drugs; health personnel. rational use of medications RESUMOOs objetivos deste estudo foram investigar a utilização de psicofármacos e os fatores associados ao seu uso entre os profissionais da saúde de um hospital de Santa Cruz do Sul, RS. Caracteriza-se por um estudo transversal, com coleta de dados por meio de questionário autoaplicável, não identificável e estruturado com perguntas abertas e fechadas, no período de setembro e outubro de 2012. Foram entrevistados 106 profissionais, sendo 91,5% mulheres, 74,5% téc-nicos de enfermagem e 40,57% atuantes em Unidades de Internação. Dos 106 participantes da pesquisa, 23 (21,7%) faziam uso de psicofármacos; com maior consumo entre profissionais atuantes em Unidades de Terapia Intensiva (34,78%) e Unidades de Internação (34,78%), com carga horária semanal de trabalho superior a 60 horas (50%), incluindo situações de mais de um vínculo empregatício. Observou-se maior prevalência no consumo de psicofármacos entre técnicos de enfermagem (22,8%). Os principais motivos apontados para o início do uso dos medicamentos em estudo foram a depressão, a ansiedade e a insônia. A falta de conhecimento sobre as características dos medicamentos e tratamento também foram observadas. Os resultados desse estudo apontam para um elevado consumo de psicofár-macos entre profissionais de saúde e que há necessidade de maior comunicação entre usuários e prescritores, além da busca por orientação farmacêutica, a fim de elucidar dúvidas quanto ao tratamento, visando o uso racional e correto de medicamentos.Palavras chave: psicofármacos; profissionais da saúde; uso racional de medicamentos
RESUMOInteração medicamentosa indesejável está entre as principais causas dos problemas relacionados aos medicamentos, podendo ocasionar prejuízos na recuperação da saúde do paciente. O objetivo foi investigar a ocorrência de interações medicamentosas na farmacoterapia dos idosos atendidos na Farmácia Básica de Santa Cruz do Sul -RS. Estudo transversal, com coleta de dados entre Agosto a Setembro de 2014, através de entrevista individual. As interações medicamentosas foram obtidas através do software Micromedex ® Solutions. Dos 204 regimes terapêuticos analisados, 54,4% continham pelo menos uma interação medicamentosa. A gravidade das interações encontradas foi: menor (n=26), moderada (n=160) e maior (n=45). O Ácido Acetilsalicílico teve a maior prevalência (23,8%) de interações, e Hidroclorotiazida foi o mais prescrito (18,6%). O manejo clínico mais indicado para as interações foi monitorização do paciente (73,6%). A identificação das interações permite tratamentos mais efetivos, menos complicações, racionalidade dos medicamentos, melhorias na qualidade das prescrições e na qualidade de vida dos pacientes. ABSTRACTThe undesirable drug interactions is among the main causes of the problems related to drugs and may cause losses in the recovery of the patient's health. The objective was to investigate the occurrence of drug interactions in the pharmacotherapy of the elderly attended in Basic Pharmacy in Santa Cruz do Sul -RS. Cross-sectional study with data collection from August to September 2014, through individual interviews. Drug interactions were obtained by Micromedex ® Solutions software. 54.4% of the 204 therapeutic regimens analyzed had at least one drug interaction. The severity of the interactions found was: minor (n = 26), moderate (n = 160) and major (n = 45). The Aspirin had the highest prevalence (23.8%) of interactions and Hydrochlorothiazide was the most prescribed (18.6%). The most appropriate clinical management of interactions was the patient's monitoring (73.6%). The identification of the interactions allows treatments more effective, fewer complications, rationality of drugs, improvements in the quality of prescriptions and in the quality of patients life.
The results of this review suggest that access to insurance or another program that assists with medication costs was a protection factor for non-adherence. On the other hand, a high frequency of dosing was a risk factor for non-adherence. Therefore, these characteristics of pharmacotherapy must be considered to improve medication adherence among CVD patients.
Introdução: o aumento da longevidade e o predomínio de doenças colocam o idoso no grupo etário com maior diversidade terapêutica da sociedade. Objetivo: identificação dos medicamentos potencialmente inapropriados durante dispensação para idosos em Farmácia Municipal. Método: estudo transversal, com coleta de dados nos meses de Agosto e Setembro de 2014, através de entrevista individual com questionário semiestruturado. Os medicamentos potencialmente inapropriados para idosos foram identificados através do Critério de Beers de 2003. A análise dos dados foi realizada no software SPSS versão 20.0 (Statistical Package for the Social Sciences 20.0). Resultados: foram entrevistados 300 idosos, com prevalência de medicamentos inapropriados para idosos de 43%, prevalecendo aqueles que atuam no sistema nervoso (83,9%). Conclusão: identificou-se que 43% da farmacoterapia utilizada pelos idosos são considerados inapropriados conforme critério utilizado. Observou-se a necessidade de readequar a assistência farmacêutica local, no sentido de disponibilizar à população idosa medicamentos mais seguros e igualmente eficazes.

Background and Objectives:The practice of Clinical Pharmacy in hospitals is of utmost importance, as the pharmacist cares for the patient, making it possible the pharmacotherapy optimization, promoting the health and well-being, and preventing diseases and improving the quality of health care. Therefore, the aim of this study was to quantify the occurrence of potential drug interactions, incompatibilities and pharmaceutical interventions carried out on prescriptions of patients treated at an Emergency Care Unit (ECU). Methods: This is a descriptive and quantitative study. The activities were developed in a teaching hospital located in the interior of the state of Rio Grande do Sul from April to July 2016, by resident pharmacists. The study sample consisted of 265 prescriptions of 107 patients. Results: There was a predominance of males (51.4%), with a mean age of 63.41 years. 2,087 prescribed drugs were identified, with a mean of 7.9 (± 3.4) drugs per prescription. RESUMO ABSTRACTThe aim of this study was to identify and quantify the number of pharmaceutical interventions for pharmacotherapeutic monitoring of patients admitted to the Emergency Unit of a teaching hospital, from March to May 2015, by recording in spreadsheets made by pharmaceutical residents after evaluation of medical prescriptions. 278 prescriptions were evaluated 190 patients, 59% were female. The mean age was 62.9 (± 17.8) years. 52 pharmaceutical interventions, of which 80.8% were accepted were held. Through clinical activities seek to ensure security by detecting errors before they impact the quality of patient drug therapy. The acceptability of found pharmaceutical interventions indicates the need and importance of the clinical pharmacist activities in hospitals working with the healthcare team in patient care, both in the economic point of view, the clinical, ensuring the rational use of medicines, safety and quality treatment.
The complexity of a pharmacological treatment can lead to a lack of adherence to the proposed treatment and consequently damage the recovery of the patient's health. The objective was to evaluate the complexity of the treatment regimen used by elderly patients attended in Basic Pharmacy of Santa Cruz do Sul, RS, and associated factors. I was done a cross-sectional study with data collection in the period from August to September 2014, through individual interviews with a semi-structured questionnaire containing sociodemographic variables, behavioral, health and pharmacotherapy variables. The therapeutic complexity index (TCI) was obtained by direct measurement of the necessary actions to administer the medicine. 300 elderly were interviewed; 76.7% were female, 70% aged 60-69 (± 6) years, and 77% had physician consultation in the last six months. The most cited diseases were hypertension (44.3%), depression (43%), high cholesterol (32%), diabetes (21%) and osteoporosis (20.7%). In the 15 days preceding the interview, the interviewers took a total of 1088 medicines (mean = 3.6 (± 1.74)). The TCI ranged from 1 to 30 (mean = 6.67 (± 5.35)) with statistically significant association (p < 0.05) with the female, referred diseases (diabetes, high cholesterol, osteoporosis, bronchitis, hypertension), take several medications at the same time and non-adherence to treatment. The evaluation of the complexity of pharmacotherapy is necessary and important for the restoration and maintenance of elderly's health. Simplifying treatment regimens can improve medication adherence and benefit on the outcome of therapy, allowing an understanding and a more accurate approximation of the elderly with their treatment.Keywords: elderly; pharmacotherapy; medication adherence RESUMO A complexidade de um tratamento farmacológico pode acarretar falta de adesão ao tratamento proposto e, consequentemente, prejuízos na recuperação da saúde do paciente.
