RESUMENObjetivos: Conocer la situación actual, en cuanto a utilización de medicamentos, realización de controles, estado de salud y percepción que tienen sobre su enfermedad los diabéticos españoles que acuden a las farmacias comunitarias.Métodos: Estudio descriptivo transversal (enero-marzo 2014) en farmacias comunitarias españolas. Sujetos: personas diagnosticadas de diabetes, que acudían a la farmacia para retirar su tratamiento. La hoja de registro de datos incluía utilización de medicamentos, realización de controles periódicos, cuestionario de conocimiento sobre la enfermedad (Berbés), de conocimiento y cumplimiento del tratamiento y de satisfacción con la medicación (DTSQ). El tamaño muestral mínimo se estimó en 385 personas diabéticas.Resultados: 652 pacientes diabéticos, 49,5% hombres y 50,5% mujeres, 54,6% mayores de 65 años, 73,0% con dos o más factores de riesgo cardiovascular. El 84,1% utilizan ADO, el 38,0% insulina y el 22,9% ambos. 146 pacientes presentaron uno o más episodios de hipoglucemia en el último año. El 65,3% de los pacientes con insulina reutiliza las agujas. El 91,6% manifiesta no tener ningún problema en la realización del autoanálisis y el 72,8% reutiliza las lancetas. Conocían adecuadamente su tratamiento 196 (34,1%) y lo cumplían 432 (76,3%). La satisfacción con el tratamiento es buena (24,6±6,6 puntos sobre 36) (82,8% de pacientes satisfechos).Conclusiones: La utilización de medicamentos no se adecúa suficientemente a las guías clínicas. Las revisiones y los autocontroles que realizan son insuficientes. El conocimiento sobre la enfermedad y la medicación es bajo. El cumplimiento y la satisfacción con los tratamientos es alta, menor en los usuarios de insulina. La asignatura Prácticas Tuteladas (PT) es obligatoria en el grado de Farmacia, siendo la de mayor carga lectiva, entre 24 y 30 ECTS. Su objetivo es completar la formación del alumno e iniciarlo en las actividades asistenciales desarrolladas en la farmacia comunitaria (FC) y/o en la farmacia de hospital. Una gran mayoría de estudiantes las realizan en la FC donde, por primera vez, entran en contacto real con la profesión. Sin embargo cada universidad establece de manera autónoma la forma en que se imparten la asignatura, existiendo una gran diversidad en el desarrollo de las PT entre las distintas facultades de Farmacia. La Sociedad Española de Farmacia Comunitaria (SEFAC) elabora este documento para contribuir a homogeneizar la docencia de esta asignatura y unificar criterios entre todas las partes implicadas: responsables académicos, profesores asociados y farmacéuticos tutores encargados de la formación de los estudiantes. En definitiva, para contribuir a mejorar su calidad docente. Se propone la creación de un Departamento específico de PT, actualmente inexistente, se abordan los criterios para acreditar farmacias y farmacéuticos tutores (FT), se revisan las funciones del profesor asociado (PA) y de los coordinadores académicos, los requisitos que deben cumplir los alumnos para cursar esta asignatura, las competencias y habilidades mínimas que necesitan adquirir y los contenidos del programa docente. También se abordan la evaluación del alumno por el FT así como la evaluación de los profesores, de la metodología docente y de las FC donde se realizan las estancias por los estudiantes.
La prestación de servicios profesionales farmacéuticos asistenciales (SPFA) al paciente desde la farmacia comunitaria ha de realizarse observando una serie de garantías sanitarias y legales previas que aseguren los derechos del paciente. Toda actuación en el ámbito de la salud de un paciente necesita su consentimiento libre y voluntario una vez que haya recibido la información adecuada. Esta información que, como regla general, se proporcionará verbalmente (salvo en los casos de procedimientos invasivos, que son los que entrañan riesgos más graves), ha de ser veraz e incluir como mínimo la finalidad y la naturaleza de cada intervención, sus riesgos y sus consecuencias, y se debe comunicar al paciente de forma comprensible y adaptada a sus necesidades, de forma que le ayude a tomar decisiones de acuerdo con su propia y libre voluntad. De este modo, para que el paciente consienta que se le preste un SPFA ha de recibir previamente una información específica sobre dicho servicio. Aunque esta información se podría proporcionar solo verbalmente, al ser los SPFA servicios novedosos con los que el paciente no está familiarizado, es recomendable proporcionarla también por escrito ya que esta vía permite garantizar que se comunica de forma uniforme, comprensible, evitando equivocaciones y olvidos, y además el paciente tendrá más tiempo para estudiarla en su casa. Este artículo analiza la información mínima que se debe proporcionar a los pacientes y usuarios en la prestación de los SPFA. Además, muestra como ejemplo tanto la información que se proporciona al paciente en cinco de los SPFA que gestiona la aplicación SEFAC e_XPERT®, como un modelo de consentimiento informado válido para dichos servicios y que incluye el resto de información necesaria que también se debe comunicar previamente al paciente.
