Estado final del recién nacidoCuando la muerte se debió al aborto se buscó el grado de infección y la iniciación del mismo (espontáneo o provocado).Las causas de muerte se dividieron en: Directas, indirectas y no relacionadas. Además en muertes por aborto, parto y puerperio.La tasa de mortalidad materna fué de 25.5 por 10.000 nacidos vivos.Las siguientes fueron las causas de muerte:1. Infección—44.3%2. Indirectas=21.4%3. Toxemia—20.1%4. Hemorragia=14.2%Cincuenta y cuatro pacientes murieron como consecuencia del aborto, esto representa el 36.2% de todas las muertes. Las restantes, fueron por complicaciones del parto o del puerperio.De las 66 madres muertas por infección el 57.5% se debió a un aborto infectado y el 70% de las que fallecieron por aborto tuvieron como complicación una infección.El 63.6% de las muertes debidas a hemorragias presentaron placenta previa, desprendimiento prematuro de la misma o ruptura uterina. De las 29 muertes debidas a la toxemia el 24.1% correspondió a la eclampsia.El estudio anatomopatológico fué practicado en el 41.6% de las madres fallecidas.La consulta prenatal sólo fué hecha en 17 pacientes (11.4%).
Se analizan los datos maternos y fetoneonatales de 3.404 partos consecutivos ocurridos en el Departamento de Obstetricia y Ginecología de la Facultad de Medicina de la Universidad de Antioquia (Fundación Hospitalaria San Vicente de Paúl), durante el año de 1982. Se hace énfasis en la morbimortalidad perinatal. En el presente estudio sólo se incluyen los recién nacidos productos de embarazos únicos. El total de nacidos vivos fue de 3.323 (97.6 %). El 82.7 % de las muertes fetales ocurrieron durante el parto. La principal causa de morbilidad neonatal fue la infección (10.5%) y la primera causa de letalidad neonatal la ocupó la anomalía congénita (147 x 1.000).Se incluyeron todos los recién nacidos con un peso superior a 500 gramos y se analizan los datos relacionados con el embarazo, parto y período neonatal. Durante el mismo lapso murieron nueve madres de causa obstétrica directa (0.3%).
El embarazo gemelar se ha considerado como un estado patológico y como tal debe realizarse su manejo. (3) El riesgo de esta patología gestacional repercute en la madre, en los fetos y recién nacidos. Desde el punto de vista materno las complicaciones más frecuentes son el hidramnios, la anemia y la hipertensión inducida por el embarazo.
Se analizan desde el punto de vista anatomopatológico las causas de las muertes perinatales atendidas en el Departamento de Obstetricia y Ginecología desde el 1o. de julio de 1986 hasta el 30 de junio de 1987. Durante el lapso mencionado se presentaron 99 muertes perinatales y la autopsia se realizó en 55 de ellas (55.5% ) y en 26 placentas. Se incluyeron fetos con un peso de 1000 gramos o más y 28 semanas o más de gestación. Se excluyeron los fetos macerados y se practicó la autopsia únicamente cuando la muerte ocurrió 12 horas antes del estudio. El registro de la información se realizó de acuerdo con el protocolo que se sigue en el Departamento de Anatomía Patológica. La coloración empleada en la mayoría de los casos fue la Hematoxilina-Eosina. De los cadáveres recibidos 44 (80%) correspondieron a muertes fetales y 11 (20%) a neonatales. La primera causa de muerte fue la axonia la cual se encontró en el 67.3% de los casos. El principal hallazgo en los casos de anoxia correspondió a la patología del cordón umbilical (24.3% ). Vale la pena resaltar que a pesar del estudio anatomopatológico no fue posible determinar la causa que desencadenó la anoxia en 11 casos (29.8% ).De los 17 casos con anomalías congénitas se encontró que la causa de la muerte fue la misma anomalía en 11 (64.7% ). La anencefalia fue la más frecuente con 10 casos (58.8% ). El bajo peso estuvo presente en 36.4% de los muertos.
