CorrespondenciaResumen La obesidad es una enfermedad crónica de carácter multifactorial que ha alcanzado proporciones epidémicas a nivel global, representando una compleja condición médica con serias consecuencias sociales y psicológicas. El exceso de la adiposidad visceral se encuentra asociado con una plétora de disfunciones metabólicas (resistencia a la insulina, dislipidemia aterogénica, hipertensión arterial, disminución de la fibrinólisis, aumento del riesgo de trombosis, inflamación endotelial) que incrementan el riesgo de enfermedad cardiovascular. Las estrategias planteadas para su correcto abordaje terapéutico deben estar acorde con la gravedad del sobrepeso, la presencia de enfermedades crónicas coexistentes y las limitaciones funcionales de cada individuo. Abstract Obesity is a chronic multifactorial disease that has reached global epidemic proportions, representing a complex medical condition with serious social and psychological consequences. The excess of visceral adiposity is associated with a plethora of metabolic dysfunctions (insulin resistance, atherogenic dyslipidemia, hypertension, decreased fibrinolysis, increased risk of thrombosis, endothelial inflammation) that increase the risk of cardiovascular disease. The strategies proposed for the correct therapeutic approach must be in accordance with the severity of overweight, the presence of chronic coexisting diseases and the functional limitations of each individual.
Los trastornos hipertensivos durante el embarazo son complicaciones muy frecuentes consideradas causa importante de morbilidad y mortalidad materna y/o fetal. Sobre el manejo de la hipertensión durante la gestación, las diversas guías actuales coinciden en tratar la presión arterial si es ≥ 160/105-110 mmHg por el elevado riesgo de accidente cerebrovascular (stroke) asociado, y en normalizar la presión a < 140/90 si hay compromiso de órgano blanco. Sobre el tratamiento de la hipertensión leve-moderada, la evidencia concluye que reduce el riesgo de desarrollar hipertensión no controlada, pero no previene la preeclampsia. En lo que se refiere a la elección de un fármaco, entre los agentes recomendados, ningún agente específico es la primera opción porque no hay datos que apoyen uno sobre otro.
Como Coordinador General del Simposio sobre Función Endotelial es un gran orgullo poderles ofrecer este Simposio dirigido a los Profesionales de la Salud y que revisa el conocimiento de algunos aspectos del órgano endotelial para un mejor manejo de patologías tan importantes como la ateroesclerosis, hipertensión arterial, dislipidemias, diabetes mellitus, falla cardiaca, preeclampsia, entre otras. Son objetivos específicos prevenir el avance de la ateroesclerosis y sus complicaciones en el eje cardio-renal-metabólico.Hace más de 13 años un grupo de profesionales colombianos de la salud en diferentes disciplinas constituyeron e iniciaron el Colegio Panamericano del Endotelio con el afán de conocer la nueva frontera que conducirá a la medicina del futuro. Fue su iniciador y primer Presidente el Dr. Enrique Melgarejo, en unión de un grupo selecto de profesionales como fundadores. A este grupo se fueron vinculando profesionales de la salud de diferentes especialidades, constituyendo actualmente un grupo numeroso en toda América, todos ellos miembros interesados y vinculados de alguna manera con el endotelio. En la actualidad, el Colegio ha empezado una apertura a todas las áreas de la salud, invitándolos a unirse en el fascinante estudio del endotelio.En las últimas décadas ha sido importante descubrir funciones cruciales del endotelio vascular. El endotelio es capaz de sintetizar, almacenar y liberar moléculas especiales que inciden en el funcionamiento normal de diferentes órganos. Cumple una función de 'esponja' al almacenar moléculas producidas por él mismo y sustancias no endoteliales. Actualmente se está trabajando intensamente en su sistema de cavéolas que con seguridad reserva grandes sorpresas al investigador por su función de almacenamiento y señaliza-ción celular.Para el presente Simposio se ha invitado a distinguidos profesionales conocedores del tema, de diferentes especialidades médicas, de manera de compartir sus conocimientos con un enfoque especial en la región. Participan en el Simposio los siguientes científicos: Nuestro reconocimiento a los profesores invitados, quienes por muchos años comparten tribuna e intentan acercar más a América Latina, sobre todo a la Región Andina.El representante latinoamericano es sin duda el Profesor Salvador Moncada, quien obtuvo su PhD en los inicio de los 1970s en el Royal College of Surgeons de Londres, donde contribuyó al descubrimiento de que medicamentos parecidos a la aspirina inhiben la biosíntesis de prostaglandinas, lo que explica sus acciones analgésicas, antipiréticas y antiinflamatorias.En 1985 empezó un proyecto que llevó a la identificación del óxido ní-trico (NO) como el mediador biológico antes conocido como factor relajante derivado del endotelio. Él esclareció la vía de la síntesis del NO a partir del aminoácido L-arginina y descubrió muchas de las actividades biológicas de este nuevo mediador. Su hallazgo de
