Los trastornos hipertensivos durante el embarazo son complicaciones muy frecuentes consideradas causa importante de morbilidad y mortalidad materna y/o fetal. Sobre el manejo de la hipertensión durante la gestación, las diversas guías actuales coinciden en tratar la presión arterial si es ≥ 160/105-110 mmHg por el elevado riesgo de accidente cerebrovascular (stroke) asociado, y en normalizar la presión a < 140/90 si hay compromiso de órgano blanco. Sobre el tratamiento de la hipertensión leve-moderada, la evidencia concluye que reduce el riesgo de desarrollar hipertensión no controlada, pero no previene la preeclampsia. En lo que se refiere a la elección de un fármaco, entre los agentes recomendados, ningún agente específico es la primera opción porque no hay datos que apoyen uno sobre otro.
Objetivo: Determinar la prevalencia de los factores de riesgo cardiovascular en una cohorte de pacientes con infección por el VIH que recibían terapia antiretroviral de gran actividad (TARGA). Material y métodos: Llevamos a cabo un estudio prospectivo observacional entre los meses de enero a abril del 2006 en pacientes que acudían a consulta ambulatoria del Hospital Rebagliati-ESSALUD-Perú. Resultados: En 276 pacientes encontramos una pre- valencia de factores de riesgo cardiovascular como sigue: hipertensión en 3,26%, diabetes mellitus en 1,81%, dislipidemia en 34,05%, tabaquismo en 3,96%, y obesidad en 4,71%. Solo se encontró niveles mayores de colesterol total y triglicéridos en el grupo que recibia inhibidores de proteasa (p<0,05). El 4,16% de esta población tuvo una puntuación de riesgo cardiovascular elevada de acuerdo a la puntuación de Framinghan. Conclusiones: La prevalen- cia de factores de riesgo cardiovascular en nuestra cohorte fue baja con excepción de la prevalencia de dislipidemia, encontrándose diferencias estadísticamente significativas en la subpoblación de pacientes que recibían inhibidores de proteasa los cuales tuvieron niveles mayores de colesterol total y triglicéridos.
