Данный подход к выделению пациентов группы риска по развитию абдоминального сепсиса (АС) мо-жет послужить одним из факторов, способных улуч-шить исходы лечения тяжелого острого панкреатита (ТОП). Инфекционные осложнения развиваются лишь у 30-50 % пациентов с панкреонекрозом, однако именно эта категория больных более всего подвер-жена риску развития мультиорганной дисфункции и летального исхода [2, 7].Обязательным компонентом воспалительного ответа на инфекцию является синтез фактора некроза опухолей альфа (ФНО-α). Bashkir state medical university, ufa, russiaЦелью исследования являлась оценка взаимосвязи полиморфизма гена фактора некроза опухоли альфа с развитием абдоминального сепсиса у пациентов c тяжелым острым панкреатитом (ТОП). В исследование было включено 100 пациентов (39 женщин и 61 мужчина, средний возраст 48,1±14,2 лет). Пациенты были раз-делены на две группы: пациенты с доказанным абдоминальным сепсисом (АС, n=49) и пациенты с недоказан-ным АС (контроль, n=51). Полиморфизм генотипа -308 ФНО-α был определен у 89 пациентов: у 49 больных с ТОП и доказанным АС и 40 пациентов с ТОП и недоказанным АС. Не было выявлено статистически значимых отличий по возрасту, полу, сопутствующим заболеваниям, тяжести состояния и исходам среди носителей AG и GG генотипов -308 ФНО-α полиморфизма у пациентов с АС. Среди носителей AG генотипа нами было вы-явлено более частое развитие гнойно-септических осложнений, и эти пациенты были чаще подвержены риску развития иных, чем брюшная полость, очагов инфекции в сравнении с носителями GG генотипа -308 ФНО-α полиморфизма.
Ключевые слова: острый панкреатит, сепсис, ФНО-α, полиморфизм генаThe aim of study was to evaluate the relationship of tumor necrosis factor alpha gene polymorphism with the development of abdominal sepsis in patients with severe acute pancreatitis (SAP). The study included 100 patients (39 women and 61 men, mean age 48.1±14.2 years). Patients were divided into two groups: patients with proven abdominal sepsis (AS) (n=49) and patients with unproven AS (controls, n=51). The genotype of -308 TNF-α polymorphism was determined in 89 patients: 49 patients with TOP and proven AS and 40 patients with TOP and unproven AS. There were no statistically significant differences in age, sex, concomitant diseases, severity of the condition and outcomes among the carriers of AG and GG genotypes of -308 TNF-α polymorphism in patients with AS. Among carriers of AG genotype we revealed more frequent development of purulent septic complications, and these patients were more often at risk of developing other than the abdominal cavity foci of infection compared with carriers of the GG genotype of the -308 TNF-α polymorphism.
