Mast cells are well known for their role in allergic and anaphylactic reactions, as well as their involvement in acquired and innate immunity. Increasing evidence now implicates mast cells in inflammatory diseases where they are activated by non-allergic triggers, such as neuropeptides and cytokines, often exerting synergistic effects as in the case of IL-33. Mast cells can also release pro-inflammatory mediators selectively without degranulation. In particular, IL-1 induces selective release of IL-6, while corticotropin-releasing hormone secreted under stress induces the release of vascular endothelial growth factor. Many inflammatory diseases involve mast cells in cross-talk with T cells, such as atopic dermatitis, psoriasis and multiple sclerosis, which all worsen by stress. How mast cell differential responses are regulated is still unresolved. Preliminary evidence suggests that mitochondrial function and dynamics control mast cell degranulation, but not selective release. Recent findings also indicate that mast cells have immunomodulatory properties. Understanding selective release of mediators could explain how mast cells participate in numerous diverse biologic processes, and how they exert both immunostimulatory and immunosuppressive actions. Unraveling selective mast cell secretion could also help develop unique mast cell inhibitors with novel therapeutic applications.
Background Mast cells derive from hematopoietic cell precursors and participate in tissue allergic, immune, and inflammatory processes. They secrete many mediators, including preformed TNF, in response to allergic, neuropeptide, and environmental triggers. However, regulation of mast cell degranulation is not well understood. Objective We investigated the role of mitochondrial dynamics in degranulation of human cultured mast cells. Methods Human umbilical cord blood–derived mast cells (hCBMCs) and Laboratory of Allergic Diseases 2 (LAD2) mast cells were examined by confocal and differential interference contrast microscopy during activation by IgE/antigen and substance P (SP). Mast cells in control and atopic dermatitis (AD) skin were evaluated by transmission electron microscopy. LAD2 cells were pretreated with mitochondrial division inhibitor, a dynamin-related protein 1 (Drp1) inhibitor, and small interfering RNA for Drp1, which is necessary for mitochondrial fission and translocation. Calcineurin and Drp1 gene expression was analyzed in stimulated LAD2 cells and AD skin biopsies. Results Stimulation of hCBMCs with IgE/antigen or LAD2 cells with SP leads to rapid (30 minutes) secretion of preformed TNF. Degranulation is accompanied by mitochondrial translocation from a perinuclear location to exocytosis sites. Extracellular calcium depletion prevents these effects, indicating calcium requirement. The calcium-dependent calcineurin and Drp1 are activated 30 minutes after SP stimulation. Reduction of Drp1 activity by mitochondrial division inhibitor and decrease of Drp1 expression using small interfering RNA inhibit mitochondrial translocation, degranulation, and TNF secretion. Mitochondrial translocation is also evident by transmission electron microscopy in skin mast cells from AD biopsies, in which gene expression of calcineurin, Drp1, and SP is higher than in normal skin. Conclusion Human mast cell degranulation requires mitochondrial dynamics, also implicated in AD. (J Allergy Clin Immunol 2011;127:1522-31.)
Corticotropin-releasing hormone (CRH) is secreted under stress and regulates the hypothalamic-pituitary-adrenal (HPA) axis. However, CRH is also secreted outside the brain where it exerts pro-inflammatory effects through activation of mast cells, which are increasingly implicated in immunity and inflammation. Substance P (SP) is also involved in inflammatory diseases. Human LAD2 leukemic mast cells express only CRHR-1 mRNA weakly. Treatment of LAD2 cells with SP (0.5–2 µM) for 6 hr significantly increases CRHR-1 mRNA and protein expression. Addition of CRH (1 µM) to LAD2 cells “primed” with SP for 48 hr and then washed, induces synthesis and release of IL-8, tumor necrosis factor (TNF) and vascular endothelial growth factor (VEGF) 24 hr later. These effects are blocked by pretreatment with an NK-1 receptor antagonist. Treatment of LAD2 cells with CRH (1 µM) for 6 hr induces gene expression of NK-1 as compared to controls. However, repeated stimulation of mast cells with CRH (1 µM) leads to downregulation of CRHR-1 and upregulation in NK-1 gene expression. These results indicate that SP can stimulate mast cells and also increase expression of functional CRHR-1, while CRH induces NK-1 gene expression. These results may explain CRHR-1 and NK-1 expression in lesional skin of psoriatic patients.
Summary Vitiligo is a disorder of depigmentation, for which the pathogenesis is as yet unclear. Interleukin (IL)-8 is a key inflammatory chemokine. We investigated the regulation of IL-8 production in human melanocytes, and the IL-8 serum levels and skin gene expression in patients with vitiligo and in controls. Cultured melanocytes were stimulated for 24 h with tumour necrosis factor (TNF) 100 ng/mL and IL-1β 10 ng/mL, with or without pretreatment with luteolin 50 μmol/L for 30 min, and IL-8 release was measured by ELISA. Serum cytokines were measured by a microbead array. Skin biopsies were taken from healthy subjects (n = 14) as well as from marginal lesional and nonlesional skin from patients with vitiligo (n = 15). IL-8 gene expression was evaluated by quantitative reverse transcriptase PCR. Both TNF and IL-1β stimulated significant IL-8 release (P < 0.01) from melanocytes, whereas pretreatment with luteolin significantly inhibited this effect (P < 0.01). IL-8 gene expression was significantly increased in vitiligo compared with control skin (P < 0.05). IL-8 may be involved in vitiligo inflammation. Inhibition by luteolin of IL-8 release could be useful for vitiligo therapy.
These results suggest that interactions between NT and mast cells may occur and contribute to AD pathogenesis.
Vitiligo is a cutaneous disorder of depigmentation, clinically characterized by well-demarcated, white macules of varying size and distribution. It can affect up to 2 percent of the population, especially younger ages. In spite of recent findings implicating genetic, immune and oxidative stress factors, the exact pathogenesis of vitiligo remains obscure. Here, we briefly discuss the prevailing theories, and offer new suggestions that could explain in part the damage of melanocyte in the vitiliginous lesions. Our emerging hypothesis is that neuropeptides released from peripheral nerve endings could synergize with new cytokines to adversely affect melanocyte function and viability. These may include corticotropin- releasing hormone (CRH) and neurotensin (NT), as well as interleukin 33 (IL-33) and thymic stromal lymphopoietin (TSLP). Such interactions could serve the basis for further research, possibly leading to new treatments.
Η λεύκη αποτελεί μια νόσο αποχρωματισμού του δέρματος, που κλινικά χαρακτηρίζεται από αχρωμικές, σαφώς αφοριζόμενες κηλίδες ποικίλης διαμέτρου και εντόπισης. Η νόσος μπορεί να έχει έντονη ψυχολογική επιβάρυνση, κυρίως σε άτομα με σκουρόχρωμο δέρμα. Παρόλο που η κλινική διάγνωση της νόσου είναι σχετικά εύκολη, οι ανοσολογικοί και βιοχημικοί μηχανισμοί που οδηγούν στην ανάπτυξη αυτών των αχρωμικών κηλίδων στο δέρμα των ασθενών δεν είναι ακόμη πλήρως κατανοητή. Η αυτοανοσία, το οξειδωτικό στρες και τα νευροχημικά συμβάντα έχουν πρωταρχικώς συσχετιστεί με την καταστροφή του μελανοκυττάρου στο παθολογικό – με λεύκη – δέρμα. Πολυάριθμες μελέτες έχουν καταδείξει το δέρμα ως το ισοδύναμο του άξονα υποθάλαμος – υπόφυση – επινεφρίδια, με το τελευταίο να δρα ως κεντρικός ρυθμιστής της απόκρισης στο στρες. Ποικίλα διαφορετικά τμήματα της επιδερμίδας επικοινωνούν μεταξύ τους και ρυθμίζουν την τοπική ομοιστασία, και αυτό το επιτυγχάνουν μέσω της παραγωγής νευροδιαβιβαστών του στρες, νευροπεπτιδίων και νευρο-ορμονών που δρουν σε ειδικούς υποδοχείς με εκλεκτικό τρόπο. Παραδείγματα δραστικών επιδερμιδικών προϊόντων αποτελούν, μεταξύ άλλων, οι βιογενείς αμίνες (επινεφρίνη, νορεπινεφρίνη, σεροτονίνη, ισταμίνη), η μελατονίνη και οι μεταβολίτες της, η ACTH η εκκρινόμενη από την προ-οπιομελανοκορτίνη, η β-ενδορφίνη και τα πεπτίδια MSH, η εκλυτική ορμόνη της κορτικοτροπίνης και οι σχετιζόμενες με αυτό ουροκορτίνες, και οι σχετιζόμενες με το θυρεοειδή ορμόνες. Ο κύριος στόχος της παρούσας διδακτορικής διατριβής είναι η μελέτη πιθανών νευροχημικών αλληλεπιδράσεων μεταξύ μελανοκυττάρων και μαστοκυττάρων που θα μπορούσαν να συμβάλλουν στην παθογένεια της λεύκης. Επελέγησαν τα μαστοκύτταρα προς μελέτη 1) δεδομένων των πολυάριθμων μεσολαβητών που εκκρίνουν και 2) λόγω του ενεργού τους ρόλου στην παθογένεια άλλων αυτοάνοσων δερματικών νόσων όπως της ψωρίασης και της ατοπικής δερματίτιδας. Για το σκοπό αυτό χρησιμοποιήσαμε μοριακές μεθόδους όπως οι ELISA και Luminex για να μετρήσουμε τα επίπεδα νευροπεπτιδίων και κυτταροκινών που απελευθερώνονται από /ή ενεργοποιούν τα μαστοκύτταρα στον ορό ενηλίκων με λεύκη και υγιών μαρτύρων. Επίσης, χρησιμοποιήσαμε ανθρώπινη σειρά μαστοκυττάρων και μελανοκυττάρων για να ελέγξουμε τις δράσεις των εν λόγω μορίων/ κυτταροκινών. Η γονιδιακή έκφραση στο δέρμα των μορίων στόχων μετρήθηκε με τη μέθοδο real-time PCR. Τα αποτελέσματά μας δείχνουν ότι νευρορμόνες όπως η ουσία P αυξάνει την έκφραση του υποδοχέα της εκλυτικής ορμόνης τη κορτικοτροπίνης (CRH-R) στα μαστοκύτταρα, ενεργοποίηση των οποίων επάγει την απελευθέρωση άλλων χυμοκινών και μεσολαβητών, όπως των IL-8, του αγγειακού αυξητικού παράγοντα του όγκου (VEGF) και του παράγοντα νέκρωσης του όγκου (TNF). Καλλιέργειες ανθρώπινων μελανοκυττάρων εκκρίνουν IL-8, μετά από ερεθισμό με κυτταροκίνες των μαστοκυττάρων, όπως η ισταμίνη, ο TNF, η ιντερλευκίνη IL-1β και η λυμφοποιητίνη του στρώματος του θύμου (TSLP). Στο παθολογικό επίσης δέρμα των ασθενών βρέθηκε στατιστικά αυξημένη έκφραση της IL-8 σε σύγκριση με το δέρμα υγιών μαρτύρων, ενισχύοντας την υπόθεσή μας ότι στα αρχικά στάδια αποχρωματισμού, η IL-8 μπορεί να διαδραματίζει ρόλο χυμοκίνης προς ενεργοποίηση άλλων ραστικών μορίων κατά των μελανοκυττάρων. Πολυάριθμες μελέτες, κυρίως στην ψωρίαση, έχουν καταδείξει τον προστατευτικό ρόλο των φυσικών φλαβονοειδών όπως της κουερσετίνης και της λουτεολίνης στην επιδείνωση των δερματικών βλαβών. Στην παρούσα μελέτη, θέλαμε επιπλέον να μελετήσουμε αν η λουτεολίνη θα μπορούσε να έχει προστατευτικό ρόλο στη λεύκη. Τα αποτελέσματά μας έδειξαν ότι το φυσικό φλαβονοειδές λουτεολίνη μπλοκάρει την έκκριση της IL-8 από τα ανθρώπινα μαστοκύτταρα, τα ενεργοποιημένα με TNF και IL-1β, αναστέλλοντας τη φλεγμονή, γεγονός που δίνει ελπίδα και στη δημιουργία νέων φαρμάκων με βάση τα φυσικά φλαβονοειδή.
