Introduction: Otosclerosis causes conductive hearing loss that histologically corresponds to a bone remodeling process called otoespongiosis. Although multiple pathological studies describing onset and the characteristics of this process, there are no articles in literature that correlate histological disease with hearing impairment and surgical outcomes. Aim: Correlate pathological disease with clinical, intraoperative and audiometric variables and evaluate the stapedostomy outcome. Material and method: Retrospective study including patients undergoing stapedostomy between 1995 and 2011, with histopathologic evaluation of stapes bone and pre and postoperative audiometric evaluation. Statistically, univariate analysis for categorical and continuous variables was used, then a logistic model to identify predictors of surgical success was adjusted. For data tabulation and analysis Stata ® 12 (StataCorp. 2011. College Station, TX: StataCorp LP) statistical software was used. Results: 204 stapedectomies were assessed, 24.51% with abnormal stapes bones, the gross appearance of the stage was not related to the microscopic result (p =0.476), no association between pre-surgical air audiometric thresholds (p =0.291) and bone (p =0.824) with histopathologic severity was found. The improved postoperative airway threshold (p =0.326) and air-bone gap closure (p =0.23) did not correlate with histology. Conclusion: Stapedostomy primary outcome, ie, improvement in hearing thresholds, as well as the air-bone gap, were significant. Routine pathological stapes bone study had no prognostic value in contrast to other factors analyzed in this study.
A pesar de las ventajas de la audición biaural, el manejo quirúrgico del segundo oído en pacientes con otoesclerosis bilateral fue por años un tema controversial debido a su riesgo potencial de hipoacusia sensorioneural. Objetivo: Describir los resultados audiológicos y quirúrgicos en pacientes operados bilateralmente por otoesclerosis en nuestro hospital entre 1988-2011. Material y método: Estudio retrospectivo de 56 pacientes en quienes se realizó cirugía de otoesclerosis en ambos oídos, mediante revisión de fichas clínicas. Resultados: En 13,5% de 415 pacientes se realizó cirugía bilateral. La edad promedio fue 44,2 ± 7,7 (hombres) y 42,0 ± 10,4 años (mujeres). La mejoría de los umbrales auditivos aéreos promedios en la primera y segunda cirugía fue de 33,6 y 29,2 dB respectivamente, mientras que la de los umbrales óseos fue de 7,0 y 3,0 dB correspondientemente. Las dificultades quirúrgicas para la primera y segunda cirugía fueron, respectivamente: platina flotante 0% y 2,5%, platina obliterada 5,4% y 5,1%, nicho profundo 5,4% y 0%, facial proscidente 38,5% y 43,2%, facial dehiscente 18,2% y 13,8% y sección de cuerda del tímpano 2% y 5,7%. Discusión: Los resultados auditivos del segundo oído son comparables a los de la primera cirugía. Esto, junto a la ausencia de complicaciones significativas, apoya la realización de intervenciones quirúrgicas bilaterales en otoesclerosis. Conclusión: La cirugía del segundo oído en hipoacusia bilateral por otoesclerosis es una alternativa segura y confiable.
El síndrome de apnea e hipoapnea obstructiva del sueño (SAHOS) es una patología altamente prevalente y se asocia a importante comorbilidad cardiovascular. Su etiología es multifactorial. Dentro de las opciones terapéuticas, la cirugía de la vía aérea superior es una alternativa efectiva en casos seleccionados.
La rinometría acústica es una técnica de laboratorio que permite objetivar la geometría de la cavidad nasal. En nuestro medio aún hay discusión acerca de su utilidad y no existen estudios referenciales. Objetivos: Establecer parámetros referenciales para rinometría acústica en población chilena adulta sana. Material y método: Se seleccionaron prospectivamente adultos voluntarios sanos para evaluación rinológica y antropométrica. Se midieron las áreas mínimas de sección transversal y volúmenes endonasales en estado basal y posterior a la administración tópica de vasocontrictor. Resultados: Se obtuvieron 83 individuos adultos sanos, de ellos el 56,63% son hombres, con edad promedio de 24,3 años. Los parámetros rinométricos fueron: MCA1 (área de sección mínima de 10 a 32 mm) 0,61±0,14 cm 2 , MCA2 (área de sección mínima 32 a 64 mm) 0.62±0.018 cm 2 , Vol1 (volumen entre narina y 32mm) 1.99±0.039 cm 3 , Vol2 (volumen entre 32 mm y 64 mm) 4,43±0,15 cm 3. Posvasoconstrictor se encontró aumento significativo en áreas y volúmenes salvo en Vol1. Discusión: Es fundamental definir rangos normales de los diámetros de la cavidad nasal, para aplicar dicha información en la evaluación de la patología rinosinusal que supone anormalidad de la geometría endonasal. Conclusión: Los valores encontrados mostraron diferencias significativas en los parámetros rinométricos por sexo y respuesta posvasoconstricción, sin diferencias en relación a la antropometría.
Introducción: El estudio histopatológico de muestras obtenidas mediante cirugía endoscópica nasosinusal (CENS) es rutinaria en muchos centros hospitalarios, sin embargo, los reportes de sus resultados y el análisis de su real utilidad clínica son escasos en la literatura publicada.Objetivos: Describir y analizar el estudio de la anatomía patológica obtenida mediante CENS y conocer la correlación entre el diagnóstico clínico y la histopatología.Material y método: Se realizó un estudio descriptivo retrospectivo mediante revisión de fichas clínicas y recolección de datos anatomopatológicos, intervenciones por CENS en el Hospital Clínico Universidad de Chile.Resultados: El mayor porcentaje de los resultados histopatológicos fueron diferentes formas de rinosinusutis crónica (RSC) representando 65,67% (RSC simple, alérgica, poliposa alérgica y no alérgica). Entre los hallazgos con menor frecuencia destacan: papiloma invertido (4,9%), pólipo antrocoanal (9,31%), rinosinusitis fúngica no invasiva (2,45%) y 7 casos de patología maligna. Con respecto a la correlación clínica e histopatológica, se encontró para RSC una concordancia del 92,15%, para RSC poliposa del 95,94% y en pólipos antrocoanales 85,71%.Conclusiones: La mayoría de los resultados obtenidos fueron distintas formas de inflamación crónica de la mucosa nasosinusal. Se encontró una fuerte correlación entre el diagnóstico clínico y la histopatología, sin embargo, la aparición no excepcional de patología maligna inclina la conducta hacia el uso rutinario de solicitud de biopsia.Palabras clave: Cirugía endoscópica nasal, estudio anatomopatológico.
