IntroductionPrimary hypoparathyroidism is a rare condition caused by parathyroid hormone deficiency and characterized by hypocalcemia. The clinical manifestations of primary hypoparathyroidism include tetany, seizures, paresthesias, dementia, and parkinsonism. Psychiatric manifestations such as mood disorders are unusual and may constitute a major diagnostic challenge, especially if the typical manifestations caused by hypocalcemia are absent.Case presentationThe patient was a 22-year-old Caucasian man with a history of chronic omeprazole use and periodic seizures, who presented to the emergency department of a secondary hospital in Southern Brazil with symptoms of major depression (sadness, anhedonia, loss of appetite, insomnia, and fatigue) associated with paresthesias affecting his toes. The initial electrocardiogram revealed a prolonged QTc interval. A computed tomography scan of his brain revealed bilateral, nonenhancing hyperdense calcifications involving the putamen and caudate nucleus. An electroencephalogram showed generalized bursts of slow spikes. Blood laboratory study results indicated serum hypocalcemia, hypomagnesemia, and hyperphosphatemia associated with a low parathyroid hormone level. His serum levels of albumin, 25-hydroxyvitamin D, thyroid-stimulating hormone, T3 and T4 thyroid hormones, as well as the results of kidney function tests, were normal. The definitive diagnosis was primary hypoparathyroidism with psychiatric manifestations due to chronic hypomagnesemia induced by proton pump inhibitor use.ConclusionsIn some cases, to differentiate between a primary psychiatric disorder and primary hypoparathyroidism with neuropsychiatric symptoms may represent a challenge given that the classical manifestations of hypocalcemia, especially tetany, may be absent in the setting of chronic hypoparathyroidism. Clinicians and psychiatrists should consider primary hypoparathyroidism part of the differential diagnosis during the evaluation of patients with mood symptoms, especially in the context of atypical presentations associated with hypocalcemia.

Neonaticide is an infant murder occurring on the day of birth. The case reports found in the literature are often focused on the mother as the agent in the context of pregnancy denial, dissociative symptoms, or psychosis. However, this report describes a rare case of attempted serial neonaticides, in which the acts were committed by a nurse at the nursery of a referral hospital in Brazil. The authors describe a forensic psychiatric evaluation for criminal responsibility and correlate the information from this particular case with relevant forensic themes, namely neonaticide, Munchausen by proxy syndrome, and serial healthcare killers.

Lower levels of resilience, especially the ability to solve situations and having personal values that give meaning to life, immature defense mechanisms, and emotional and physical abuse in childhood are associated with PTSD in adult Brazilian victims of urban violence.

ReferencesIntimate partner violence (IPV), also known as domestic violence, is defined by the World Health Organization 1 as ''any behavior within an intimate relationship that causes physical, psychological, or sexual harm to those in the relationship.'' While IPV may be perpetrated by women against men or in homosexual relations of both sexes, most cases (85%) consist of men victimizing women. This produces a worldwide problem with serious implications for women's health and well-being. 2,3 Special populations of women are at increased risk of IPV, including pregnant women (especially in unplanned pregnancies), those living in low-income settings, rural women, and older, indigenous, military, and immigrant women. Within this context, a study on the prevalence of IPV among Japanese women noted that all those victimized before pregnancy continued to suffer violence during pregnancy. 4 A prospective cohort study conducted by Silva et al. in Recife, state of Pernambuco, Brazil, found a high incidence of IPV in the postpartum period (9.3%), with predominance of psychological violence. 5Lévesque & Chamberland investigated IPV in the perinatal period in young women and warned about the difficulty these women experience in identifying themselves as victims and categorizing the acts of their partners as domestic violence. The fear of family separation and the victims' desire to protect their children contribute to the complexity of violence experienced during maternity. 6 In addition to the sequelae experienced by non-pregnant women victimized by conjugal violence, IPV can have additional negative effects when perpetrated against pregnant women, increasing the risk of inappropriate antenatal care, poor weight gain, anemia, infections, bleeding, maternal depression, and suicidal ideation and suicide attempts.We illustrate the aspects listed above with the case of a young couple from the Southern region of Brazil. The husband was an unemployed man with a criminal record who kept his wife (5 months along a planned pregnancy) under false imprisonment while causing her grievous bodily harm. A complaint to police was made by neighbors who noticed the false imprisonment. Although there was a previous history of domestic violence, the victim and the perpetrator minimized the aggressive acts to the police, judge, and forensic psychiatrists who evaluated the case. The victim did not report any concerns about the baby's health or physical integrity during any of the psychiatric interviews. Surprisingly, both the wife and the husband attributed the responsibility of the facts to the victim, including in a handwritten letter by the wife, directed to the judge in the case, blaming herself for having received such treatment from her husband. Violence committed by individuals with severe mental disorders has become a growing focus of interest among physicians, law enforcement officials, and the general population. Homicide involving relatives, specially parricide, matricide, and filicide, despite the relatively low incidence of these crimes, may be enigmatic, so forensic psychiatrist are frequently called on the courts to answer questions about insanity and criminal responsibility. The current study aims to describe Brazilian cases of parricide, matricide, and filicide associated with presence of major mental disorders and personality disorders, discussing the assessment of criminal responsibility in each case. The case series described were specifically related to people with mental illness, as bipolar disorder, schizophrenia, and borderline personality with comorbidity of drug abuse. Two of them were considered not guilty by reason of insanity, and the other one was considered partially criminally responsible, according to Brazilian Law and Forensic Psychiatric Reports of the cases. The justice determined compulsory psychiatric treatment for all of them. The question of criminal responsibility of individuals with mental disorders is challenging for criminal justice, psychiatry, and society. Adequate treatment is mandatory to prevent crimes involving mental disorders, as shown in literature. The verification of criminal responsibility is essential for persons' adequate referral in any system of criminal law, thus protecting human rights and referring those who need psychiatric treatment.
