Background:Surgical treatment of achalasia fails in 10%–20% of patients. The most frequent responsible cause is the performance of an incomplete myotomy at primary surgery. The treatment when the failure happens is not well defined. In this study, we review and evaluate the possible treatments to be carried out when surgical myotomy fails. We define its benefits and results, with the purpose of defining a therapeutic algorithm.Materials and Methods:The systematic review was performed following the guidelines established by the Meta-analysis of Observational Studies in Epidemiology statement. We searched several electronic databases (MEDLINE, PubMED, EMBASE and Cochrane) from January 1991 to March 2017, with the keywords 'recurrent achalasia' 'POEM remyotomy', 'esophagomyotomy failure', 'Heller myotomy failure', 'myotomy failure', 'pneumatic balloon dilatation achalasia' and combinations between them, 'redo Heller', 'redo myotomy', 'reoperative Heller'.Results:A total of 61 observational studies related to the treatment of patients with failure of Heller's myotomy were initially found. Finally, 37 articles were included in our study that provided data on 289 patients. Of these 289 patients, diagnosed of failed Heller's myotomy, 87 were treated with pneumatic dilatation (PD), 166 underwent surgical revision and finally 36 were treated with POEM. No randomised controlled trial was identified.Conclusions:The three therapeutic options analysed in this review are effective and safe in the treatment of patients with achalasia with failure of surgical myotomy. The best results can be achieved following an algorithm similar to the one proposed here, where each procedure must be performed by well-experienced team in the selected modality.
This surgical procedure is a first line in the treatment of esophageal achalasia. It is necessary to have special care in early diagnostic cases to avoid electrosurgical injury.
En julio de 2018, la Asamblea Nacional del Poder Popular (ANPP) de Cuba inició el trámite de discusión de un anteproyecto de nueva Constitución cubana. Entre el 13 de agosto y el 15 de noviembre de 2018, la ANPP convocó una fase de consulta popular entre la ciudadanía destinada a la discusión y recogida de propuestas, previa al debate para su consideración en la cámara y su posterior ratificación en referéndum en febrero de 2019. La nueva Constitución cubana es la concreción jurídico-formal de un proceso constituyente material que viene refundando las relaciones económicas y sociales de la isla desde inicios de la década de los 90 hasta hoy. El presente trabajo analizará cuales han sido estas transformaciones y realizará un análisis crítico de la Constitución cubana de 2019.

Universidad de ExtremaduraResumen.-Antonio Gramsci murió en la cárcel el 27 de abril de 1937 después de ser detenido en 1926 por la policía fascista de manera ilegal, puesto que era diputado del Partido Comunista Italiano (PCI), y condenado por el Tribunal especial por la defensa del Estado a veinte años de cárcel. Detrás quedaba una vida marcada por la lucha y el compromiso, por delante se levantaba el reto de interpretar una herencia teórica rica y compleja. Este artículo pretende recuperar y presentar el legado teórico de Gramsci, sus aportes a la teoría marxista que durante años, ahogada en el esquematismo y el dogmatismo, se había visto incapaz de entender la estructuración del Estado moderno, así como destacar la validez e importancia de su contribución teórica para la actual teoría del Estado y el poder.

Behind his dead remained a life characterized by the commitment and fight, in front of it the challenge to interpret a rich and difficult theoretical legacy. This article try to recover and present the theoretical legacy of Gramsci, his contributions to the marxist theory witch one during a lot of years, drowned in the schematism and dogmatism, it was unable to understand the structure of the modern state, and at the end too, emphasize validity and importance of his theoretical contribution for the now a days State and Power theory.
LABURPENA: Azken hamarkada bietan Latinoamerikako Andeen aldean onartutako konstituzioek forma konstituzional propioa sortu dute, konstituzionalismo tradizionaletik bereizten diren elementuak dauzkana. Elementu horietako bat demokrazia liberal ordezkatzailearen eredua gainditzea eta demokrazia parte-hartzailearen eredu berriak sartzea da. Demokrazia parte-hartzailearen eredu homogeneo bakarrari buruz hitz egin dezakegu, ordea, xxi. mendeko Andeetako konstituzionalismoaren barruan? Ezetz defendatzen du artikulu honek, eta herrialde horietan, bereziki Venezuelan eta Bolivian, demokrazia parte-hartzailearen zenbait eredu sorrarazi dituzten zioak eta eredu horien arteko aldeak aipatzen ditu.
RESUMEN: La Constituciones aprobadas durante las últimas dos décadas en la zona andina de América Latina han creado una forma constitucional propia, con elementos diferentes al constitucionalismo tradicional. Uno de estos elementos es la superación del modelo de democracia liberal representativa y la introducción de nuevos modelos de democracia participativa. Sin embargo, ¿podemos hablar de un único modelo homogéneo de democracia participativa dentro del constitucionalismo andino del siglo xxi? Este artículo defiende que no, señalando las causas que han dado lugar al surgimiento en estos países, en especial Venezuela y Bolivia, de distintos modelos de democracia participativa y cuáles son las diferencias entre tales modelos.
ABSTRACT: The Constitutions approved during the last two decades in the Andean zone of Latin America have created a constitutional form of their own, with elements different from traditional constitutionalism. One of these elements is the overcoming of the model of representative liberal democracy and the introduction of new models of participatory democracy. However, can we speak of a single homogeneous model of participatory democracy within the Andean constitutionalism of the 21st century? This article argues that no, pointing out the causes that have led to the emergence in these countries, especially Venezuela and Bolivia, of different models of participatory democracy and what are the differences between such models.
