RESUMEN Introducción:Las familias afrocolombianas se caracterizan por vivir en contextos de alta vulnerabilidad, que requieren su fortalecimiento con programas en promoción de la salud. Objetivo: Construir un programa de promoción de la salud, con familias afrocolombianas en situación de pobreza en Tumaco, año 2012. Materiales y Métodos: Investigación -Acción -Participativa, conformando un grupo y comisión de seguimiento, esto permitió la adaptación de la propuesta de Martí a través de cuatro etapas en el proceso. Resultados: El programa de Promoción de la Salud fue una construcción colectiva entre el grupo Investigación Acción Participación, la comisión de seguimiento y las familias, realizada en cuatro fases: Etapa 1: Inicio y preparación del campo. Etapa 2. Diagnóstico: Aproximación al universo de las familias afrocolombianas. Etapa 3. Construcción y aplicación de programa de promoción de la salud: "Del dicho al hecho superamos los trechos". Etapa 4. Evaluación y proyección del programa: "Evaluando vamos terminando". Discusión y Conclusiones: La construcción social del programa de promoción de la salud, así como el abordaje pedagógico y didáctico para el desarrollo del mismo, se desarrolló en función del contexto cultural, desde la perspectiva de la Investigación Acción Participación. El programa "Por un futuro mejor" permitió crear nuevos conocimientos a familias, líderes y profesionales de la salud a partir de la realidad e intercambio de saberes, con acciones dirigidas a fortalecer el amor propio, la comunicación, el entorno y la participación, con el fin de mejorar las condiciones de salud. Introducción: Los programas domiciliarios adaptados culturalmente, permiten obtener resultados eficientes en poblaciones vulnerables. Objetivo: Evaluar el efecto de un programa de salud domiciliaria en personas ancianas con dependencia funcional en tres grupos culturalmente diversos. Metodología: Estudio cuantitativo con datos secundarios, resultados de cuatro estudios cuasiexperimentales. Participaron en el Programa de salud domiciliaria adaptado culturalmente 155 personas ancianas de tres grupos culturales: habitantes del área urbana de Bogotá (58), campesinos de Cáqueza (30), afrocolombianos de Tumaco (18) y Guapi (49). La información fue recolectada con las escalas de: intensidad del dolor, autopercepción de la salud y actividades de la vida diaria (AVD). Se realizó un análisis descriptivo y prueba no paramétrica de rango signo de Wilcoxon en cada grupo. Se consideraron aspectos éticos. Resultados: El programa de salud domiciliaria para personas ancianas tuvo efecto positivo estadísticamente signifi cativo en las variables de dolor, autopercepción y dependencia en actividades de la vida diaria; se observó especialmente la intensidad de dolor en afrocolombianos y el aumento de la independencia de las actividades de la vida diaria en los campesinos. Conclusiones: El logro de resultados estadísticamente significativos en las variables de dolor, autopercepción y dependencia en los grupos culturalmente diversos de personas ancianas, sugiere la necesidad de realizar investigaciones sobre programas de salud domiciliaria con perspectiva cultural y con participación de cuidadores, teniendo en cuenta la baja adherencia terapéutica de personas ancianas con dependencia funcional.
ResumenIntroducción: Para el profesional de salud que busca ofrecer un cuidado congruente con los valores, creencias, prácticas y necesidades sentidas es un desafío conocer la heterogeneidad y diversidad cultural de un grupo de población que envejece de forma acelerada en el mundo. Objetivo: Este artículo describe las dinámicas y las prácticas de cuidado en salud de una familia en condición de pobreza que vive con una persona anciana en situación de discapacidad en la ciudad de Bogotá (Colombia). Método: Estudio cualitativo desarrollado con base en la historia oral de vida, tejido con la participación de tres integrantes de la familia Vargas. Las entrevistas en profundidad suman 894 minutos de grabación. La recolección, procesamiento y análisis de datos se efectuó siguiendo el procedimiento propuesto por el profesor e investigador brasilero José Carlos Sebe Bom Meihy. Resultados: Dos grandes temas emergieron: La familia Vargas: entre lo glocal y lo líquido, y Los Vargas: entre la sobrevivencia y la lucha por la dignidad. Conclusiones: El cuidado ocurre como una expresión de solidaridad familiar en una dinámica ambivalente y tensa, con cargas económicas, emocionales y de deterioro en la calidad de vida para las personas que lo asumen. La familia soporta en la esfera privada la dura carga de cuidar, acumulando muchas más desventajas sociales quedando surcados en un círculo de humillación e indignidad.Palabras clave: familia; anciano; personas con discapacidad: cultura; enfermería transcultural Dynamics and care practices in poverty, old age and disability: The Vargas family AbstractIntroduction: For health professionals that seek to provide care which aligns with the values, beliefs, practices and perceived need, it is a challenge to know the heterogeneity and cultural diversity of a population group which is rapidly aging. Objective: This article describes the dynamics and health care practices of a family in poverty living with an elderly person with disabilities in the city of Bogotá (Colombia). Method: A qualitative study was developed based on the oral history of life, constructed with the participation of three members of the Vargas family. In-depth interviews total 894 minutes of recording. The collection, processing and data analysis was carried out following the procedure proposed by the Brazilian professor and researcher José Carlos Sebe Bom Meihy. Results: Two major themes emerged: The Vargas family: Between glocal and liquid and The Vargas: between survival and struggle for dignity. Conclusions: The care occurs as an expression of family solidarity in an ambivalent and tense dynamic, with economic and emotional loads which bring deterioration in the quality of life for those who take them. The family supports in the private sphere the heavy burden of care, accumulating many more social disadvantages and in turn end up trapped in a circle of humiliation and indignity.
