Síndrome de Guillain Barré asociado a la infección por el SARS-CoV-2,en paciente con diagnóstico diferencial por dengue RESUMEN Introducción: El síndrome de Guillain-Barré ha sido asociado a la infección por el virus SARS-CoV-2 desde los inicios de la pandemia. Presentación del caso: Varón de 56 años de edad sin comorbilidad, ni hábitos tóxicos, consulta por cuadro de 4 días de evolución con parestesias a nivel de manos y pies en simultáneo, debilidad de miembros inferiores, sin otros síntomas. El paciente 22 días antes, presentó tos y disnea, de 3 días de evolución, el hisopado nasofaríngeo para SARS-CoV-2 fue positivo, con tratamiento ambulatorio. Refirió dengue en diciembre 2019. Examen físico: Presión arterial 140/80 mmHg, frecuencia cardiaca 114 por minuto, temperatura 36 grados, frecuencia respiratoria 20 por minuto. Examen neurológico: paciente lúcido, disminución de la fuerza muscular de miembros inferiores 4/5, reflejos osteotendinosos abolidos en cuatro miembros, sin nivel sensitivo ni compromiso esfinteriano, signo de Babinski negativo, signos meníngeos ausentes. Resto del examen físico sin datos de valor. Pruebas laboratoriales: glóbulos blancos 11,5 x 10 9/L, neutrófilos: 74%, linfocitos: 18%. Eritrosedimentación 14 mm, Serología Elisa: HIV: negativo, serología IGM negativo: herpes simple, citomegalovirus, dengue antígeno NS1: negativo; dengue serología IgG e IgM: positivo (noviembre 2020, test inmunocromatografico, SD, korea), (SARS-CoV-2 IgG/IgM positivo, zika y chikungunya negativo). Anticuerpo anti nuclear (ANA): 1/80, anti desoxirribonucleasa (anti DNA): negativo, serología negativo (anti proteinasa 3/mieloperoxidasa). Líquido cefalorraquídeo:(LCR): (glucorraquia: 63 mg/dL proteína: 88 mg/dL leucocitos: 2 células/μl). La PCR en el LCR para el virus SARS-CoV-2 no fue realizada.E l e c t r o m i o g r a f í a : p o l i r r a d i c u l o n e u r o p a t i a desmielinizante. Tac de tórax: patrón en vidrio e s m e r i l a d o p a r c h e a d o , b i l a t e r a l . R e c i b i ó inmunoglobulina intravenosa con recuperación completa de la fuerza muscular.Conclusión: El SGB se presentó en el día 22 de la enfermedad por COVID-19, hay una relación temporal, responde a un modelo post infeccioso y la patogenia sería de naturaleza autoinmune
Introducción: la anemia intrahospitalaria es el descenso de hemoglobina (Hb) durante la internación por debajo de los valores del ingreso, no asociada a pérdidas objetivables de sangre. Es un fenómeno frecuente que puede afectar la evolución clínica. Objetivo: determinar las características clínicas de la anemia intrahospitalaria en pacientes internados en Dpto. de Medicina Interna del Hospital Nacional de Itauguá desde febrero hasta noviembre de 2018. Metodología: estudio observacional, prospectivo, descriptivo de corte transverso realizado en pacientes de ambos sexos, mayores de 15 años internados en el Dpto. de Medicina Interna. Se consideró anemia a todo descenso de Hb de más de 2 mg/dL a partir de 15 días de internación. Resultados: se estudiaron 80 pacientes, de los cuales 44% fueron mujeres y 56% del sexo masculino. La media de edad fue 52±17 años. El 72% presentó anemia. Los pacientes ingresaron con valor medio de Hb 13±1 g/dL y hematocrito (Hto) 38±8%. A los 15 días se detectó descenso de Hb media a 11±2 g/dL y Hto a 33±5%. A los 30 días la media de Hb fue 10±2 g/dL y Hto 31±5%. La cantidad de sangre extraída para estudios tuvo una media de 348 ±214 cc. La presencia de infección y el uso de antibióticos durante la internación se detectaron en 83% de los pacientes estudiados. Conclusión: la anemia intrahospitalaria tuvo una alta frecuencia (72%) en la muestra estudiada. con las presiones arteriales tanto sistólicas (r=0,34) como diastólicas (r=0,34) y el IMC (r=0,60) arrojaron datos positivos con una p <0,01. Conclusión: el consumo de sal fue casi el doble del recomendado y vario ligeramente con el último informe realizado en nuestro país y con países vecinos. Se demostró una relación positiva entre el consumo de sal elevado con los parámetros clínicos expuestos, se vio igual correlación tanto para la presión sistólica como la diastólica y una mayor relación con el IMC presentándose una línea de tendencia que supone a mayor IMC mayor natriuria y consecuentemente mayor presión arterial.
Introducción: el síndrome de Cushing iatrogénico es el resultado de la utilización inadecuada, excesiva y en forma crónica de los glucocorticoides. Objetivos: describir las indicaciones y complicaciones de la corticoterapia prolongada, además de las características clínicas y demográficas de los pacientes que concurrieron al Hospital Nacional en el periodo 2010-2017. Resultados: se incluyeron 70 pacientes, con edad media 51±16 años (rango 18-91 años), de los cuales 40 (57%) eran mujeres y 30 (43%) eran varones. El IMC medio fue 28,8 kg/m . 2 Los corticoides utilizados eran la prednisona (86%) y la dexametasona (14%). El 60% era por automedicación y 40% por prescripción médica. Las indicaciones del uso de corticoides se debían a enfermedades inmunológicas (62,86%) y no inmunológicas (37,14%). La complicación más frecuente fue la obesidad (97,14%). El valor medio de hemoglobina glicada fue 6,38±1,67 mg/dL (rango 4,6-12,5 mg/dL).Conclusiones: el corticoide más utilizado fue la prednisona. El motivo de uso predominante fue la automedicación. Los efectos colaterales más frecuente fueron la obesidad, la facies cushingoide y las alteraciones dermatológicas.

Int. marzo 2017; 4 (1):46-56 RESUMENIntroducción: el drenaje ventricular externo es una herramienta útil en el manejo de muchos pacientes neurocríticos. Sin embargo este dispositivo no está exento de complicaciones. La ventriculitis es la más importante complicación infecciosa asociada.Objetivos: determinar la prevalencia de ventriculitis en pacientes adultos con drenaje ventricular externo del Hospital Nacional, describir los factores de riesgo asociados e identificar gérmenes más frecuentemente aislados.Resultados: se incluyeron 92 pacientes con drenaje ventricular externo de los cuales el 20% desarrolló ventriculitis. La totalidad de los pacientes con ventriculitis presentaron síntomas de infección luego de 7 días de instalación del drenaje, además la mitad de los mismos requirieron recambio del catéter lo cual constituyó factor de riesgo. Los gérmenes aislados más frecuentes fueron Staphylococcus epidermidis y Acinetobacter baumanii.Conclusiones: la ventriculitis asociada a catéter de drenaje ventricular externo fue 20%. El factor de riesgo estadísticamente asociado fue el recambio del catéter. Los gérmenes aislados más frecuentes fueron Staphylococcus epidermidis y Acinetobacter baumanii.
