As doenças tropicais negligenciadas são associadas às populações desfavorecidas socioeconomicamente e muitas delas apresentam alta prevalência no Nordeste brasileiro. Diante disso, o objetivo desta revisão de literatura foi descrever o perfil epidemiológico da leishmaniose, da esquistossomose e da doença de Chagas nessa região, bem como os determinantes sociais envolvidos na manutenção dessas doenças. Trata-se de uma revisão narrativa da literatura do tipo descritiva. Foram detectados cem artigos publicados entre 2012 e 2017 nas bases bibliográficas. Desses, 72 foram selecionados para leitura completa. Após leitura integral e aplicação dos critérios de seleção, restaram 39 artigos. Os resultados convergentes para as três doenças estudadas relacionaram-se às condições inadequadas de vida em sociedade, à baixa escolaridade e à organização falha da estrutura habitacional. A partir da descrição do perfil epidemiológico dessas enfermidades, foi possível ratificar que as variáveis sociais são determinantes para a manutenção dessas condições de saúde. Por fim, os fatores identificados neste estudo foram válidos visando a construção do mapeamento de áreas de risco, além de serem importantes para estudantes da área da saúde, uma vez que compõem o acervo de estudos disponíveis e necessários na capacitação de futuros profissionais no enfrentamento de tais problemas sociais.

Background: The assessment of intravascular volume and fluid responsiveness is challenging in the management of critically ill patients. Diagnostic methods of hemodynamic monitoring must be safe, reproducible, and practical. Objective: To describe the applicability of ultrasound indices of the inferior vena cava in the assessment of intravascular volume and prediction of fluid responsiveness in critically ill patients. Method: A systematic review performed of the PubMed®, Latin American and Caribbean Health Sciences Literature, and Scientific Electronic Library Online databases for articles published in the previous five years. The descriptors used were “inferior vena cava,” “ultrasonography,” “fluid responsiveness,” and “volume status.” Results: The search identified 13 relevant articles. The collapsibility index of the inferior vena cava was 25%–50% as the cutoff point for defining hypovolemia and showed applicability in predicting fluid responsiveness in patients breathing spontaneously with cutoff points of 25%–57%. In mechanical ventilation scenarios, the distensibility index of the inferior vena cava was more effective at predicting fluid responsiveness than the other measurements with variation of 10.2%–20.5%. The inferior vena cava/aorta diameter index was especially useful in the pediatric population for defining intravascular volume; however, in adults, there were many divergences in its applicability. Conclusion: The assessment of intravascular volume and fluid responsiveness through ultrasound indices of the inferior vena cava is applicable and safe for diagnosing and monitoring hemodynamic instability. However, studies of value standardization are necessary due to divergences in the cutoff points used in each index.
Objetivo: verificar se determinantes de saúde e hábitos estão relacionados à adesão terapêutica hemodialítica na região de Paulo Afonso-BA. Métodos: estudo observacional, transversal e analítico. Amostra de 200 pacientes adultos de ambos os sexos, de áreas rurais e urbanas da região, portadores de DRC e que faziam hemodiálise há mais de três meses. Resultados: foi identificada associação entre as pessoas que consomem/consumiam álcool e a não adesão à hemodiálise. Verificou-se que o consumo de álcool e a não adesão à hemodiálise é maior em mulheres que moram na zona urbana do que as de zona rural. Discussão: os achados têm relação com outros estudos, em relação à predominância do sexo masculino, de moradores da zona urbana, ao consumo de álcool e à não adesão à hemodiálise maior em mulheres da zona urbana. Conclusões: entre os pacientes que realizam hemodiálise, há mais homens que mulheres, e mais da zona urbana que rural da região de Paulo Afonso. O consumo de bebidas alcoólicas e a não adesão ao tratamento é maior em mulheres que moram na zona urbana.
