Les malformations congénitales des arcs aortiques correspondent à un groupe hétérogène de pathologies liées à des anomalies de développement des arcs branchiaux primitifs pendant la vie intra-utérine. La coarctation de l'aorte est une malformation vasculaire congénitale fréquente qui correspond à un rétrécissement congénital de l'isthme aortique, segment de l'aorte compris entre l'artère sous-clavière gauche et le canal artériel. L'interruption de l'arche aortique est considérée par certains auteurs comme une forme extrême de la coarctation de l'aorte, caractérisée par une discontinuité entre l'aorte ascendante et descendante. Ces anomalies s'intègrent dans la majorité des cas dans le cadre de malformations cardiaque dont ils sont indissociables. L'angioscanner thoracique joue un rôle incontournable dans l'exploration de ces anomalies, leurs bilans préopératoires et leurs suivis à long terme. Notre travail s'inscrit dans le cadre d'une étude rétrospective portant sur 42 patients explorés tous par angioscanner thoracique à la suite de la découverte d'une cardiopathie sur l'examen échocardiographique. L'angiographie a été réalisée dans 6 cas. L'âge moyen est de 2 ans et 6 mois avec des extrêmes allant de 6 jours à 14 ans. Nous avons noté une prédominance masculine avec un sexe ratio de 1,6. Les principales pathologies rencontrées étaient: la coarctation aortique: 18 cas; les hypoplasies de l'arche aortique: 8 cas; les interruptions de l'arche aortique: 7 cas; les anomalies des arcs aortiques: 9 cas. Certaines de ces anomalies étaient associées. Les anomalies extra-cardiaques associées aux cardiopathies congénitales sont relativement fréquentes; le scanner multibarrete permet une bonne analyse des voies afférentes et efférentes au cœur. Grace à une exploration tri-dimensionnelle et multiplanaire satisfaisante, la tomodensitometrie (TDM) permet en complément de l'échocardiographie d'établir un bilan pré-thérapeutique des principales pathologies malformatives surtout pour la détection des anomalies vasculaires extra-cardiaques associées. Elle tend à supplanter l'angiographie dans de nombreuses pathologies malformatives et ce pour plusieurs raisons: il est moins invasif; il offre une image 3D d'excellente qualité utile au chirurgien; il établie le diagnostic anatomique, apprécie la compression trachéale et fait le bilan d'éventuelles malformations associées; il guide le traitement chirurgical.
L'IDM per gravidique est une complication grave qui entraine une morbidité et une mortalité maternelle élevée. Bien que l'athérosclérose soit la cause d'IDM la plus fréquente dans la population générale, elle n'est observée que chez un tiers des femmes enceintes. Chez ces dernières, la cause la plus fréquente d'IDM était la dissection coronaire. Nous rapportons l'observation clinique d'une jeune femme de 24 ans, sans facteur de risque cardiovasculaire, qui a présenté un IDM antérieur étendu 15 jours après un accouchement, en rapport avec une dissection de la partie proximale de l'artère inter ventriculaire antérieure. Le traitement de cette pathologie n'est pas consensuel, et peut faire appel, selon la présentation clinique et angiographique, au traitement médical, à une revascularisation par pontage aorto-coronaire avec une résection de l'hématome de la paroi artérielle, ou à l'angioplastie transluminale. Le pronostic semble assez favorable quoique controversé.
Aortic dissection is an absolute cardiovascular emergency, its diagnosis has benefited from the development of modern imaging techniques including transoesophageal echocardiography, CT scan and Magnetic resonance angiography, its management can be medical, surgical or interventional depending on the type of the dissection and the potential associated complications. Thanks to its recent development, aortic stent-grafting technique is now extending its indication to high-risk patients even before clinical complications in order to prevent their occurrence. We present the clinical case of a patient admitted for uncomplicated type B aortic dissection with a contraindication to beta-blockers who successfully underwent a stent-graft implantation with positive outcome.
Pemphigoid gestationis is a skin-specific autoimmune disorder that can sometimes present as the cutaneous manifestation of a multiorgan disease due to potentially common pathogenic mechanisms. We report a severe form of pemphigoid gestationis in a 32-year-old primigravida woman, who presented at 22 weeks of gestation with headaches and blurred vision, later developing encephalitis, intrauterine fetal demise and dilated cardiomyopathy.
Authors report a case of a malignant psoas tumor diagnosed in a 69-year-old patient with an abdominopelvic CT scan as well as a PET-Scanner performed as part of the assessment of infective endocarditis extensions on aortic stenosis. Authors insist on the rarity of psoas tumors which remain mostly malignant, and on the interest of the CT scan and percutaneous biopsy for a precise histological diagnosis conditioning the therapeutic management. Treatment must be integrated into multidisciplinary approach where surgery occupies preponderant place. This case illustrates a therapeutic quandary regarding to timing of cardiovascular surgery related to carcinological surgery and the complement by radio and chemotherapy.
Introduction:The atrial septal defects are the most common congenital heart diseases; moreover the Sinus venous atrial septal defect (SVD) is a rare cardiac abnormality, particularly difficult to diagnose. It is commonly associated anomalous with anomalous pulmonary venous return. The conventional treatment is Surgery. Case report: We report a case of a 61 year old man with medical history only significant for diabetes mellitus. He undergone a chest X rays for his annual medical examination which showed a cardiomegaly; A transthoracic echocardiographies (TTE) examination done previously in other centers showed a dilated right heart chambers with a moderate pulmonary hypertension, but did not show any ASD. He was referred for a dilated right chambers misdiagnosed as a pulmonary hypertension. The TTE revealed a Sinus venosus atrial septal (SVASD) defect seen in the sub costal view. TEE confirmed the diagnosis showing a large SVASD of 20 mm with left-right shunt (Qp/Qs 2, 05). Conclusion: The diagnosis of SVASD is difficult and requires special imaging. But when there is any suspicion of ASD, especially with patients who have unexplained dilated right ventricle and a pulmonary hypertension, the sub-costal view on the TTE can be useful.
