ФГУП Научно-технический центр радиационно-химической безопасности и гигиены ФМБА России, Москва Проанализированы модели расчёта радиационного риска, включённые в доклад НКДАР ООН 2006 года. Представлена объединяющая эти модели математическая формула с таблицами унифицированных констант для всех локализаций и показателей риска. В качестве основного критерия надёжности моделей рассматривалось согласие соответствующих им моделей фоновых заболеваемости и смертности, равных отношениям EAR/ERR, c фоновыми данными для композитной популяции МКРЗ, японского и российского населения. Принципиальным недостатком нелинейных по дозе моделей НКДАР является зависимость выведенных таким способом фоновых моделей от дозы. Для линейных моделей риска их фоновые модели в количественном, а для некоторых видов рака и в качественном отношении, расходятся с фоновыми данными. Наборы моделей риска НКДАР и МКРЗ в целом плохо согласуются между собой, результаты сравнительного их анализа не позволяют отдать явное предпочтение ни одному из них. Для оценок радиогенной заболеваемости раком щитовидной железы предпочтительнее применять модели НКДАР, в то время как для раков ободочной кишки, лёгкого и категории «остальные» более точные оценки могут быть получены по моделям МКРЗ. Выбор модели должен осуществляться отдельно для каждой локализации рака с учётом целей проведения оценок и особенностей дальнейшего использования полученных результатов.
Научно-технический центр радиационно-химической безопасности и гигиены, Федеральное медико-биологическое агентство России, Москва, Россия Цель: уточнить квазибиологическую модель радиогенной заболеваемости солидным раком, ранее опубликованную авторами в журнале «Радиационная гигиена», для обеспечения лучшей согласованности с данными японской когорты (когорты LSS) и с моделью МКРЗ в части описания эффекта возраста при облучении. Результаты: исходные представления математической модели дополнены предположением, что стволовые клетки, получившие «предраковое» повреждение, с большей вероятностью по сравнению с неповреждённой клеткой идут по пути воспроизводства, а не дифференцировки. Структура уточнённой модели и её свойства при этом сохранились, но добавился один параметр. Анализ основных математических соотношений уточнённой модели показал, что с её позиций между моделями радиогенной заболеваемости раком, опубликованными в Публикации 103 МКРЗ и Докладе НКДАР ООН 2006 г., несмотря на их внешнее различие, нет противоречий. Заключение: дальнейшая работа с моделью требует верификации значений параметров по эпидемиологическим данным, в первую очередь по данным о заболеваемости солидными раками в японской когорте. Однако, поскольку некоторые из предположений модели, включая вновь введённое, не являются чисто радиобиологическими, то потребуется анализ широкого круга биологических и онкологических закономерностей.

Представлена математическая формула, обобщающая модели, и табулированы значения её констант для различных локализаций рака и показателей риска. Для оценки качества моделей радиогенного риска предложен и применён к моделям МКРЗ критерий соответствия фону, суть которого состоит в оценке степени взаимного согласия определяемых ими моделей фоновых заболеваемости и смертности и их соответствия фактическим данным для подходящего населения. Анализ наборов моделей радиогенного риска МКРЗ выявил ряд логических несоответствий. Так, суммирование моделей для локализаций не приводит к модели для солидных раков в целом ни по смертности, ни по заболеваемости, а модели для некоторых локализаций дают превышение чисел умерших над числами заболевших за жизнь поколения как для радиогенных, так и для фоновых раков. Модели МКРЗ для отдельных локализаций отличаются от моделей, подогнанных Престоном с соавт. к данным когорты LSS, и, следовательно, не вполне соответствуют последним. Выявленные недостатки свидетельствуют о целесообразности более тщательного обоснования моделей для расчётов радиационных рисков, определения коэффициентов риска и тканевых весовых множителей. Введение МКРЗ в Рекомендациях 2007 года (далее -Рекомендации) [1] опубликовала набор моделей радиогенного риска, использованных ею для обоснования рекомендованных значений коэффициентов номинального риска (КНР) и тканевых весовых множителей (ТВМ). НКДАР ООН (далее -НКДАР) почти одновременно с МКРЗ представил в докладе [2] другой набор обобщённых моделей риска (ОМР). Из материалов этих публикаций следует, что оба набора ОМР разработаны путём подгонки под одни и те же массивы эпидемиологических данных, фактически только под данные о заболеваемости и смертности в когорте японцев, переживших атомные бомбардировки (когорте LSS). Как известно, рекомендации МКРЗ составляют основу для подготовки международных и национальных документов, регламентирующих вопросы радиационной безопасности. Так, в последнюю редакцию отечественных норм радиационной безопасности (НРБ-99/2009) традиционно почти без изменений перенесены рекомендованные в [1] значения КНР и ТВМ. С другой стороны, модели НКДАР нередко применяют для подготовки нормативных документов более низкого уровня и для проведения оценок радиогенных рисков при решении некоторых задач радиационной безопасности (см., например, [3-5]). Однако даже поверхностное сравнение наборов ОМР МКРЗ и НКДАР выявляет существенные различия в принятых в них зависимостях радиогенных рисков как от дозы, так и от возрастных переменных.
