Вопросы коморбидности ВИЧ-инфекции и туберкулеза становятся все более актуальными в период улучшения эпидемиологической ситуации по туберкулезу. ВИЧ-ассоциированному туберкулезу уделяется большое внимание в связи с особенностями процессов, для которых характерны генерализация, прогрессирование и высокий уровень летальных исходов. Цель исследования: изучить особенности туберкулеза у больных ВИЧ-инфекцией при летальных исходах в регионе с высоким распространением ВИЧ-инфекции. Материалы и методы. Для сравнительного анализа в зависимости от исхода заболевания сформировано две группы: 1-я группа -70 пациентов с туберкулезом и ВИЧинфекцией (ТБ/ВИЧ), умерших в течение месяца при поступлении в противотуберкулезный стационар (группа изучения), 2-я группа -70 пациентов с ТБ/ВИЧ, выписанных из больницы с улучшением (группа сравнения). Лечение всем проводилось в соответствии с клиническими рекомендациями. При статистических расчетах применялся c 2 -критерий Пирсона или c 2 -критерий с поправкой Йетса, при числе степеней свободы f=1 (f=(r-1)×(c-1)). Уровень значимости принимался за величину 0,05 и менее. Результаты. Выявление туберкулеза в группах одинаково часто было по обращаемости, в первой группе чаще при поступлении в непрофильные стационары (c 2 =5,28; р=0,0216), с диагнозом пневмонии (c 2 =9,46; р=0,0021), при отсутствии ФГ в текущем году. Летальный исход был чаще у лиц до 30 лет, не имеющих семью (c 2 =18,72; р=0,00001), пребывавших в местах лишения свободы (c 2 =7,96 р=0,0048) при асоциальном образе жизни. Анализ проявлений туберкулеза не выявил отличий, определяющих летальный исход, сложность диагностики туберкулеза возникала при уровне СD4+ менее 300 клеток. Коморбидность туберкулеза и ВИЧ-инфекции характеризуется общностью многих клинических проявлений, независимо от исхода. Наличие отягощенного социального статуса повышает вероятность смерти при коморбидности ВИЧ/туберкулез. Проблема неуклонного роста ВИЧ-ассоциированного туберкулеза (ТБ) в последние годы становится в России все более актуальной. Диагностика ТБ особенно затруднена на стадии выраженного иммунодефицита. Цель исследования -оцени ть возможность применения метода T-SPOT.TB в диагностике ТБ-инфекции у пациентов с ВИЧ-инфекцией.Материал и методы. За 2019-2020 гг. выполнен анализ результатов T-SPOT.TB у 396 пациентов. В исследование были включены пациенты, удовлетворяющие следующим критериям: возраст 18 лет и старше, диагноз «ВИЧ-инфекция», диспансерное наблюдение в Центре по профилактике и борьбе со СПИДом и инфекционными заболеваниями, проведение компьютерной томографии (КТ) легких, наличие анализа крови методом Т-SPOT.TB. На основании результатов теста Т-SPOT.TB б ыли сформированы 2 группы пациентов: 1-я группа -с положительным результатом -Т-SPOT.TB+ (n = 143) и 2-я группа -с отрицательным результатом Т-SPOT.TB-(n = 253).Результаты. ТБ верифицирован обнаружением Mycobacterium tuberculosis у 117 пациентов с ВИЧ-инфекцией, при КТ у всех наблюдались изменения, характерные для ТБ, положительный результат Т-SPOT.TB был отмечен в 88 % (n = 103) случаев. Средний уровень CD4 + -Т-клеток в этой группе составил 230,05 ± 15,3 кл/мкл при 95 % доверительном интервале [190,2]. Диагноз «латентная туберкулезная инфекция» поставлен 40 пациентам с ВИЧ-инфекцией с положительным результатом T-SPOT.TB при отсутствии изменений при КТ.Заключение. T-SPOT.TB может использоваться в диагностическом комплексе наблюдения пациентов с ВИЧ-инфекций как скрининг-метод, позволяющий в последующем не только верифицировать диагноз, но и выявить латентный ТБ.
Background. The main method of screening of patients with HIV infection to detect pulmonary tuberculosis is X-ray diagnostics. A comparative assessment of changes in lung tissue in different methods of X-ray diagnostics and at different levels of CD4+ cells seems relevant.The aim of the study. To compare plain radiography and computed tomography of the lungs as screening methods for detecting tuberculosis in patients with HIV infection at various stages of immunosuppression.Materials and methods. 396 patients with HIV registered at the AIDS Center were examined using plain lung radiography (PLR) and computed tomography (CT). To search for mycobacterium tuberculosis, we used bacterioscopy of a smear with Ziehl – Neelsen staining; HAIN-GenoType MTBDRplus technique; inoculation of BACTEC MGIT 960 liquid media; inoculation of Löwenstein – Jensen dense medium. Statistical processing of numerical material was carried out using the Statistica 5.5 program with a significance level p ≤ 0.05; Pearson χ2 criterion was used for the analysis of qualitative featuresResults. When comparing the results of PLR and CT, the differences were found. When using PLR, the focal lung lesions were described more often (χ2 = 40.79; p = 0.00001), according to CT data, they turned out to be fibrosis (χ2 = 2.33; p = 0.1269). When comparing the PLR and CT data, the differences were obtained in the description of pulmonary fibrosis (χ2 = 20.78; p = 0.00001), focal lung lesions (χ2 = 40.79; p = 0.00001), dissemination (χ2 = 9.16; p=0.0025).Conclusion. When screening HIV-infected patients (at the standard of using plain radiography twice a year) it should be taken into account that CT provides more precise differentiation of focal lung lesions and pulmonary fibrosis, earlier detection of dissemination syndrome and ground-glass, especially at severe immunodeficiency with a decrease in CD4+ T lymphocytes down to 200 cells and less. Timely appointment of CT study will improve the effectiveness of tuberculosis detection at the stage of screening in AIDS centers.
