Background In a 2013 survey, we reported distinct discrepancies in delivery of tuberculosis (TB) and HIV services in eastern Europe (EE) vs. western Europe (WE). Objectives To verify the differences in TB and HIV services in EE vs. WE. Methods Twenty‐three sites completed a survey in 2018 (EE, 14; WE, nine; 88% response rate). Results were compared across as well as within the two regions. When possible, results were compared with the 2013 survey. Results Delivery of healthcare was significantly less integrated in EE: provision of TB and HIV services at one site (36% in EE vs. 89% in WE; P = 0.034), and continued TB follow‐up in one location (42% vs. 100%; P = 0.007). Although access to TB diagnostics, standard TB and HIV drugs was generally good, fewer sites in EE reported unlimited access to rifabutin/multi‐drug‐resistant TB (MDR‐TB) drugs, HIV integrase inhibitors and opioid substitution therapy (OST). Compared with 2013, routine usage of GeneXpert was more common in EE in 2018 (54% vs. 92%; P = 0.073), as was access to moxifloxacin (46% vs. 91%; P = 0.033), linezolid (31% vs. 64%; P = 0.217), and bedaquiline (0% vs. 25%; P = 0.217). Integration of TB and HIV services (46% vs. 39%; P = 1.000) and provision of OST to patients with opioid dependency (54% vs. 46%; P = 0.695) remained unchanged. Conclusion Delivery of TB and HIV healthcare, including integration of TB and HIV care and access to MDR‐TB drugs, still differs between WE and EE, as well as between individual EE sites.

Aim - to analyze afterhistory of TB patients who had radical lung resection. Methods of research. On the basis of the results of surgical treatment of 273 patients in 2010-2011, two groups were formed: the first group - patients who were deregistered due to their recovery; the second group -patients who had tuberculous process reactivated after surgery as recurrence or acute condition. Results. The effectiveness of tuberculosis surgical treatment in long-term period was proved in more than 90% of cases. Reactivation process started in 7.7% of cases, in patients who had multiple and extensive drug resistant MTB (53.3%), in patients who had destruction in the lung tissue (62%); in patients with fibrous-cavernous tuberculosis (33.3%), with comorbidity (52.4%), with complications in the early postoperative period (9.5%). Reactivation occurred more often in cases of wide surgical intervention, including cases with follow-up remedial thoracoplasty. If there are indications for surgical treatment, it is better to decide upon surgery within the first year of supervision.

Вопросы коморбидности ВИЧ-инфекции и туберкулеза становятся все более актуальными в период улучшения эпидемиологической ситуации по туберкулезу. ВИЧ-ассоциированному туберкулезу уделяется большое внимание в связи с особенностями процессов, для которых характерны генерализация, прогрессирование и высокий уровень летальных исходов. Цель исследования: изучить особенности туберкулеза у больных ВИЧ-инфекцией при летальных исходах в регионе с высоким распространением ВИЧ-инфекции. Материалы и методы. Для сравнительного анализа в зависимости от исхода заболевания сформировано две группы: 1-я группа -70 пациентов с туберкулезом и ВИЧинфекцией (ТБ/ВИЧ), умерших в течение месяца при поступлении в противотуберкулезный стационар (группа изучения), 2-я группа -70 пациентов с ТБ/ВИЧ, выписанных из больницы с улучшением (группа сравнения). Лечение всем проводилось в соответствии с клиническими рекомендациями. При статистических расчетах применялся c 2 -критерий Пирсона или c 2 -критерий с поправкой Йетса, при числе степеней свободы f=1 (f=(r-1)×(c-1)). Уровень значимости принимался за величину 0,05 и менее. Результаты. Выявление туберкулеза в группах одинаково часто было по обращаемости, в первой группе чаще при поступлении в непрофильные стационары (c 2 =5,28; р=0,0216), с диагнозом пневмонии (c 2 =9,46; р=0,0021), при отсутствии ФГ в текущем году. Летальный исход был чаще у лиц до 30 лет, не имеющих семью (c 2 =18,72; р=0,00001), пребывавших в местах лишения свободы (c 2 =7,96 р=0,0048) при асоциальном образе жизни. Анализ проявлений туберкулеза не выявил отличий, определяющих летальный исход, сложность диагностики туберкулеза возникала при уровне СD4+ менее 300 клеток. Коморбидность туберкулеза и ВИЧ-инфекции характеризуется общностью многих клинических проявлений, независимо от исхода. Наличие отягощенного социального статуса повышает вероятность смерти при коморбидности ВИЧ/туберкулез.
