В статье достаточно подробно рассмотрены принципы, лежащие в основе различных методов метаанализа, и ос-новные отличия инструментария для их применения. Представлены и обсуждаются клинические варианты печеноч-ной недостаточности и их описания в литературных источниках различных стран и регионов мира (Россия, Азия, Ев-ропа и Северная Америка).Предложено применение теоретико-множественной модели, атрибуты которой должны учитывать особенности характера и течения болезни при различной природе болезни и в разные периоды ее развития. Полученные данные должны найти применение в последующем метаанализе. На этом этапе, являющемся основным, должны быть сфор-мированы подгруппы больных по сходству клинических проявлений и результатам применения разных схем лечения.Подход с использованием метода метаанализа для оценки мировых литературных данных позволит предлагать наиболее адекватные методы терапии заболевания у конкретного больного в зависимости от характера существенных изменений при диагностике у него определенной формы печеночной недостаточности. Это обеспечит переход (по аналогии) к направленному применению определенных медицинских средств.Ключевые слова: метаанализ, теоретико-множественная модель, интегрированный программный продукт, пе-ченочная недостаточность, выбор медикаментозного воздействия, региональные особенности клиники печеночной недостаточности.Методы метаанализа можно отнести к двум мо-делям -модели с фиксированными эффектами (fixed effect model) и модели со случайными эффек-тами (random effect model) [1]. Модель фиксирован-ных эффектов предполагает, что исследования, для которых выполняется метаанализ, проводились при одних и тех же условиях над одним и тем же объектом. Модель случайных эффектов, напротив, учитывает влияние внешних факторов на резуль-таты исследований с помощью анализа дисперсии между ними.В медицине вопросы подбора лечения непо-средственно связаны с клиническими проявлени-ями болезни, которые определяют подбор лечеб-ных препаратов. К примеру, больных с печеночной недостаточностью можно разделить на группы, определяющиеся проявлением релевантных при-знаков. На этой основе можно подбирать опти-мальную терапию. Но для этого нужны аналогич-ные по проявлениям группы больных с завершен-ными результатами лечения. Эти группы можно формировать по литературным данным с помощью метаанализа. Такой подход может повысить эффек-тивность лечения, так как будут учитываться исходные внешние факторы и развивающиеся в процессе болезни патологические изменения, а в идеальном варианте и «глубинные» молекулярно-генетические и биохимические проявления. Это будет соответствовать персонализированной тар-гетной (направленной) терапии. Одним из условий такого подхода является построение теорети-ко-множественной модели заболевания, варианты которой можно использовать в процессе метаана-лиза. Cell-based immunotherapy is a promising approach for the treatment of chronic infections, autoimmune disorders, and malignant tumors. There are many strategies of cell-based immunotherapy of cancer; these include injection of various immune effector cells, propagated and «trained» in a cell culture. Alternatively, cells presenting tumor antigens on their surface in a form recognized by the immune system can be used to achieve a therapeutic effect. The research results in this field are presented in thousands of texts, and their manual analysis is very complicated. We have developed an approach for automated text analysis in this area of biomedical science. Here we present the first results of the automated analysis of the data extracted from abstracts of scientific articles available in PubMed. These results demonstrate the associations between types of tumors and the most commonly used methods of their cell-based immunotherapy.
