Objective: Laparoscopic cholecystectomy (LC) operations are being performed under general anesthesia (GA). Further studies are needed on the issue whether these operations can be performed under spinal anesthesia (SA). In this study we aimed to compare SA with (GA) in terms of efficacy and complications in patients who will undergo LC operations, and to investigate the effects of preemptive analgesia on the development of shoulder pain, transition to general anesthesia, and postoperative analgesia. Method: Sixty patients in ASA I-II risk group between 18-65 years of age undergoing laparoscopic cholecystectomy were randomly divided into general anesthesia (GA, n=30) and spinal anesthesia (SA, n=30) groups. Patients were premedicated with i.v. midazolam and fentanyl preoperatively. Anesthesia was induced with propofol in the GA group, and maintained with Desflurane and remifentanil. In the SA group, spinal anesthesia was provided with intratechal administration of 15 mg bupivacaine at L2-3 level, and block level was increased to T4 by keeping the patient in Trendelenburg position for 7-10 minutes. Demographic data, hemodynamic parameters, operation time, visual analog scale (VAS) scores at postoperative 0 th ,1 st , 4 th , 8 th ,12 th and 24 th hours, patient-surgeon satisfaction, side effects, and occurrence of right shoulder pain in SA group were inquired and recorded. Results: Effective anesthesia was produced in both groups. Hypotension was observed in 5, bradycardia requiring atropin administration in 4, and perioperative shoulder pain in 9 patients in Group SA, but none of them required general anesthesia. Hypotension developed in one patient in Group GA. The postoperative VAS scores were significantly lower in Group SA at 0 th ,1 st , 4 th hours. Patient satisfection scores were higher in Group SA. Conclusion: We concluded that spinal anesthesia may be an alternative method to general anesthesia in patients who will undergo laparoscopic cholecystectomy operations especially when the risk of general anesthesia is too high.
Sevoflurane and propofol have no deleterious effects on homocysteine levels in patients with MTHFR deficiency. Avoidance of NO is the key point for anaesthetic consideration regarding these patients.
İşletmelerin geleceğe yönelik kararlarının başarısını iyi bir performans değerlendirmesi yapılması oldukça etkilemektedir. Bu açıdan firmaların performansının ölçülmesi hem kaynakların etkin kullanımında rol oynarken hem de sektördeki rekabet durumunun tespit edilmesi hakkında yol göstermektedir. Performans değerlendirmesi konusunda kullanılan finansal tablo analizleri ve istatistiksel yöntemler sürekli değişen çevre koşulları altında firmalara genel bir bakış açısı sunmakta yetersiz olabilmektedir. Bu noktada geleneksel yöntemler yerine çok fazla sayıda faktörün birlikte değerlendirilmesini sağlayan Çok Kriterli Karar Verme (ÇKKV) yöntemlerinin kullanımı ön plana çıkmaktadır. Çalışmanın amacı, performans ölçümünün çok sayıda faktörün dikkate alınmasını gerektiren yapısı nedeniyle ÇKKV tekniklerini kullanarak bir uygulama sunmaktır. Bu amaçla, Bulanık ÇKKV yaklaşımı kullanılarak Türkiye İmalat Sanayii'nde faaliyet gösteren Borsa İstanbul'da kayıtlı 8 ayrı sektörde bulunan işletmeler için performans sıralaması yapılmıştır. Literatüre uygun şekilde belirlenen finansal performans kriterleri ışığında uygulama modeli oluşturulmuştur. Öncelikle uzman görüşleri yardımıyla Bulanık Analitik Hiyerarşi Süreci (BAHS) ile değerlendirme kriterleri ağırlıklandırılmıştır. Daha sonra, 8 sektörde faaliyet gösteren 171 işletmenin verilerinden faydalanılarak, TOPSIS (İdeal Çözüme Benzerliğe Dayalı Sıralama Tekniği), VIKOR (Çok Kriterli Optimizasyon ve Uzlaşı Çözümü) ve GİA (Gri İlişkisel Analiz) yöntemleri ile firmaların sektör içi performans sıralamaları elde edilmiştir. Her üç yöntemin uygulanmasından söz konusu firmalar için üç ayrı performans sıralamasına ulaşılmıştır. Bu sıralamaları bütünleştirmek için, Borda Sayım (BS) yönteminden yararlanılmış ve nihai sıralamalar elde edilmiştir. Böylece hem farklı ÇKKV yöntemlerinin sonuçlarının görülmesi hem de bütünsel olarak değerlendirilmesi sağlanmıştır.
Öz: İşletmelerin gelecek planlarını oluştururken değerlendirmesi gereken stratejik konulardan birisi proje yönetimidir ve projelerin başarılı bir şekilde yürütülmeleri gerekmektedir. Projelerin başarısına etki eden birçok kritik faktör bulunmaktadır. Başarı faktörleri günümüz koşullarında maliyet, kalite, zaman gibi klasik değerlendirme kriterlerinin ötesine geçmiş durumdadır. Proje başarısına etki eden faktörlerin değerlendirilmesi karar vericilerin çok sayıda kriteri birlikte değerlendirmesini gerektiren çok kriterli karar verme (ÇKKV) problemi olarak tanımlanabilmektedir. Bu kapsamda çalışmanın amacı proje başarısında etkili olan kritik faktörlerin belirlenerek, bu faktörlerin önem derecesinin ÇKKV tekniklerinden Bulanık DEMATEL (Decision Making Trial and Evaluation Laboratory) yöntemi ile değerlendirilmesidir. Çalışmada literatüre uygun olarak başarı faktörleri belirlenmiş ve uzman görüşleri yardımıyla bunların değerlendirilmesi sağlanmıştır. Bulanık DEMATEL yöntemi ile yapılan örnek uygulamada kritik başarı faktörleri arasındaki ilişkiler ortaya çıkarılarak, önem düzeyleri belirlenmiş ve sıralanmıştır.
