Xanthoma of Bone: A Case Report

Abstract: Bone xanthoma is a rare disease due to the presence of cholesterol deposits in the bone. We report a case of a 56-year-old patient who was hospitalized in orthopedic department for fracture on the left humerus. Histological examination of bone biopsy performed at this site has objectified cholesterol deposits. Laboratory tests showed hypercholesterolemia with hypertriglyceridemia. The diagnosis of bone xanthoma was selected. The fracture was treated surgically. An initial bone scan revealed bone lesions locali… Show more

“…The differential diagnoses of these histological features include Langerhans cell histiocytosis, Rosai-Dorfman disease, Erdheim-Chester disease, and storage disease. More importantly, there is a differential diagnosis of metastatic clear cell carcinoma [1,3,6,11]. Thus, assessing clinical manifestations and radiological features is often necessary.…”

“…Treating the underlying hyperlipidemia helps disease control, while surgical curettage followed by bone grafting resolves the pain and mechanical weakness of the affected skeleton [2,8,9]. Bone nuclear scintigraphy is useful for staging disease and monitoring treatment response [11]. For patients with repeated pancreatitis, DFPP is effective [14].…”

“…Bone scintigraphy allows for mapping of the lesions and post-therapy follow-up [ 15 ]. Therefore, the radiological images remember other conditions, most commonly fibrous dysplasia, non-ossifying fibroma, cyst, and osteoblastoma [ 6 ].…”

Since surgical curettage of the lesion is curative, the skeletal xanthoma should be kept in mind if we come across the typical per operative findings to avoid more aggressive treatment than necessary.…”
