Background Malakoplakia (MP) and Rosi Dorfman’s Disease (RDD) of the uterus are both parts of Xanthoganulomatouse disease spectrum ; a very rare inflammatory disease process characterized by histiocytic infiltration of the endometrium, and occasionally may be misdiagnosed as cancer. We believe that our case is the first reported case of the coexistence of both RDD and MP. Case presentation A 66-year-old woman presented with recurrent post-menopausal bleeding. .A pelvic ultrasound scan showed endometrial thickness of eight mm and no adnexal masses. An endometrial biopsy was suggestive of Xanthogranuloma endometritis (XE). Thereafter, total abdominal hysterectomy and bilateral salpingo-oophorectomywere performed,and the histopathology confirmed the co-existenceof both uterine malakoplakia (MP) and Rosi Dorfman’s disease (RDD). Conclusion MP and RDD are both rare inflammatory diseases may be misdiagnosed as malignancy. If treated inappropriately it may lead to systemic infection and recurrent infections . Histopathological diagnostic approach will definitely add to the clinical management of such cases .

La inflamación xantogranulomatosa del tracto genital femenino es infrecuentey es aún más rara en las trompas de Falopio y ovarios. Se presenta un caso deooforosalpingitis xantogranulomatosa en una paciente femenina de 45 años quienasistió a consulta por presentar dolor en fosa iliaca izquierda acompañado de fiebre.La exploración bimanual mostró útero ligeramente aumentado de tamaño con masaanexial izquierda firme, no dolorosa, adherida al útero y con limitada movilidad.La evaluación ecográfica transvaginal determinó tumoración ovárica izquierda,heterogénea con paredes gruesas e irregulares con múltiples septos y ecos internossin visualizar el ovario. Durante la cirugía, se encontraron adherencias densas desdela masa hacia la pared lateral pélvica, fosa ovárica y asas intestinales. El útero estabadesplazado por tumoración anexial quística izquierda, de color blanco grisáceo yparedes gruesas que drenaba líquido purulento fétido. El diagnóstico definitivo fueooforosalpingitis xantogranulomatosa. Esta condición es un proceso inflamatoriopoco frecuente que plantea dilemas diagnósticos. Sus manifestaciones clínicasy características de estudios por imágenes pueden simular una neoplasia pélvicamaligna, por lo que es necesario un alto índice de sospecha para su diagnóstico,como diagnóstico diferencial en pacientes con tumoraciones ováricas quísticascomplejas. El examen histopatológico es el estándar de oro para el diagnóstico.
