WHO comparative evaluation of serologic assays for Chagas disease

Abstract: The sensitivities and specificities of at least four of the commercially available EIAs for Chagas disease are probably high enough to justify their use for single-assay screening of blood donations. Our data suggest that the majority of commercially available indirect hemagglutination assays should not be used for blood donor screening and that the RIPA could be considered a gold standard for evaluating the performance of other assays.

“…As shown by the Technical Note N°03/06 from the Brazilian Health Ministry, 19 the ELISA Chagas III showed 99.0% sensibility and 98.0% Sp. Otani and others 25 deduced the same conclusion by showing 99.4% sensibility and 99.6% Sp. Remesar and others 26 used latent class analysis to reach 95.6% Se and 99.8% Sp.…”

“…These data are in good agreement with others. 13,14,25,30 In our hands, 5.5% of the unrelated diseases specimens were incorrectly classified as Chagas disease (false positive) and 4.1% were deemed inconclusive. As expected for a test that uses recombinant antigens, the cross-reactivity to leishmaniasis was less frequent with the Imuno-ELISA Chagas.…”

“…According to the literature, sensitivity of the most widely used serological tests (eg, CLIA recombinant test 19 ) is virtually 100% and accordingly, some studies recommend that the T cruzi screening should be based on a single unique test when the test done is an ELISA assay. 20 However, it is generally held that the infection should be confirmed with a second serological test: this was the screening strategy in our model. We also considered a 4% presumptive falsepositive cases with the first test, for which a third screening was then required to resolve potential discrepancies between the first and the second test.…”
