RESULTSThe SGUIS correlated moderately well with the number of leaks per week (Spearman's r = 0.610, 95% confidence interval 0.516-0.689, P < 0.001) but less well with the 1-h pad test loss ( r = 0.257, 0.124-0.380, P = 0.002). The test-retest reliability was acceptable, as the mean ( SD ) difference between the first and second SGUIS was 0.337 (2.675), with limits of agreement of -5.012 to -5.686. The change in the SGUIS after treatment correlated well with the improvement in the number of leaks per week ( r = 0.742, 0.662-0.805, 156 samples, P < 0.001) but not as well with the change in 1-h pad test loss ( r = 0.531, 0.405-0.636, 151, P < 0.001), although the trend was similar. CONCLUSIONThe criterion validity of the SGUIS appeared adequate for both the number of leaks per week on the FVC and leakage on the 1-h pad test. The statistical reproducibility of the test was adequate and appeared more responsive to change after treatment than the other measures. The score is suitable for rapid selfadministration by patients with a range of incontinence types, unlike most other currently available test instruments. PATIENTS AND METHODSWomen presenting with urinary incontinence completed two SGUIS tests one week apart. A frequency-volume chart (FVC) was completed before the first attendance and the women had a 1-h pad test before treatment. Patients were treated conservatively under the care of a urogynaecologist or nurse continence advisor, or surgically with an open or laparoscopic colposuspension. After treatment, the SGUIS, FVC and 1-h pad test were repeated.

PURPOSE: This study was designed to estimate the prevalence of, and identify risk factors associated with, fecal incontinence in racially diverse females older than aged 40 years. METHODS: The Reproductive Risks for Incontinence Study at Kaiser is a population-based study of 2,109 randomly selected middle-aged and older females (average age, 56 years). Fecal incontinence, determined by self-report, was categorized by frequency. Females reported the level of bother of fecal incontinence and their general quality of life. Potential risk factors were assessed by self-report, interview, physical examination, and record review. Multivariate logistic regression analysis was used to determine the independent association between selected risk factors and the primary outcome of any reported fecal incontinence in the past year. RESULTS: Fecal incontinence in the past year was reported by 24 percent of females (3.4 percent monthly, 1.9 percent weekly, and 0.2 percent daily). Greater frequency of fecal incontinence was associated with decreased quality of life (Medical Outcome Short Form-36 Mental Component Scale score, P = 0.01), and increased bother (P < 0.001) with 45 percent of females with fecal incontinence in the past year and 100 percent of females with daily fecal incontinence reporting moderate or great bother. In multivariate analysis, the prevalence of fecal incontinence in the past year increased significantly [odds ratio per 5 kg/m 2 (95 percent confidence interval)] zwith obesity [1.2 (1.1-1.3)], chronic obstructive pulmonary disease [1.9 (1.3-2.9)], irritable bowel syndrome [2.4 (1.7-3.4)], urinary incontinence [2.1 (1.7-2.6)], and colectomy [1.9 (1.1-3.1)]. O objetivo deste estudo foi agrupar e atualizar conhecimentos referentes à intervenção fisioterapêutica na incontinência fecal no idoso. A pesquisa da literatura foi realizada nas bases de dados eletrônicas: MEDLINE, LILACS, Cochrane, PubMed e SciELO, no período de janeiro de 2006 a julho de 2011. As palavras-chave usadas em várias combinações foram: fecal incontinence, aged, physical therapy, physiotherapy, exercises, rehabilitation e electrotherapy. A intervenção fisioterapêutica incluiu a avaliação da incontinência fecal constituída pelos sinais e sintomas, exame físico e exames complementares, que incluíram a manometria anorretal, a retossigmoidoscopia, a defecografia, a eletromiografia anorretal, a ultrassonografia endoanal e a latência motora terminal do nervo pudendo; os exercícios para fortalecimento da musculatura do assoalho pélvico, o biofeedback, o treinamento com balão retal e a eletroestimulação constituem a base do tratamento. Essa atualização demonstrou a escassez de estudos abordando tal temática, encontrando apenas cinco estudos publicados nos últimos anos com pacientes idosos incontinentes, os quais enfatizaram em sua maioria os procedimentos já consagrados na literatura refutando as evidências já conhecidas. Porém, uma modalidade consideravelmente nova de tratamento, a neuroestimulação sacral, tem sido investigada com resultados aparentemente positivos, suscitando a necessidade de novos estudos com desenhos metodológicos específicos sobre este problema.
