Vitrectomy Combined With Periocular/Intravitreal Injection of Steroids for Rhegmatogenous Retinal Detachment Associated With Choroidal Detachment

Abstract: For the treatment of rhegmatogenous retinal detachment combined with choroidal detachment, pars plana vitrectomy with periocular/intravitreal corticosteroids was comparable in reattachment rate to pars plana vitrectomy with systemic steroids, suggesting an acceptable alternative for patients with this condition who cannot tolerate systemic steroids.

“…5 Most studies have focused on the treatment of RRDCD, and the underlying etiology and pathogenesis of RRDCD have not been elucidated because of its poor prognosis. 6 Hypotony may be the most important factor in the pathogenesis of RRDCD. 7,8 The vitreous humor enters into the subretina via a retinal tear, which may lead to hypotony.…”

“…A recent retrospective study showed that both systemic steroids and periocular steroids were equally effective in management of RRD/CD in aspect of anatomical reattachment (Wei et al. ). The current study is of prospective nature and conducted a careful OCT follow‐up of reattached macula; less oedematous changes of macula was noted for subtenon TA group at both postoperative 1‐ and 3‐month.…”

“…It is generally accepted that rhegmatogenous retinal detachment (RRD), when complicated by choroidal detachment (CD), has an especially unfavorable prognosis [1][2][3], with clinically apparent intraocular inflammation being a feature of the disease [4,5]. RRD is a degenerative disease with clinical signs of retinal hypoxia, which even if uncomplicated, is associated with expression of inflammatory mediators in ocular tissues and fluids [6].…”
