OBJECTIVE Pedicle and lateral mass screw placement is technically demanding due to complex 3D spinal anatomy that is not easily visualized. Neurosurgical and orthopedic surgery residents must be properly trained in such procedures, which can be associated with significant complications and associated morbidity. Current training in pedicle and lateral mass screw placement involves didactic teaching and supervised placement in the operating room. The objective of this study was to assess whether teaching residents to place pedicle and lateral mass screws using navigation software, combined with practice using cadaveric specimens and Sawbones models, would improve screw placement accuracy. METHODS This was a single-blinded, prospective, randomized pilot study with 8 junior neurosurgical residents and 2 senior medical students with prior neurosurgery exposure. Both the study group and the level of training-matched control group (each group with 4 level of training-matched residents and 1 senior medical student) were exposed to a standardized didactic education regarding spinal anatomy and screw placement techniques. The study group was exposed to an additional pilot program that included a training session using navigation software combined with cadaveric specimens and accessibility to Sawbones models. RESULTS A statistically significant reduction in overall surgical error was observed in the study group compared with the control group (p = 0.04). Analysis by spinal region demonstrated a significant reduction in surgical error in the thoracic and lumbar regions in the study group compared with controls (p = 0.02 and p = 0.04, respectively). The study group also was observed to place screws more optimally in the cervical, thoracic, and lumbar regions (p = 0.02, p = 0.04, and p = 0.04, respectively). CONCLUSIONS Surgical resident education in pedicle and lateral mass screw placement is a priority for training programs. This study demonstrated that compared with a didactic-only training model, using navigation simulation with cadavers and Sawbones models significantly reduced the number of screw placement errors in a laboratory setting.

Lee ABSTRACT Introduction: Nager syndrome is a rare genetic condition characterized by defects primarily of the face, arms, and hands. Children with Nager syndrome are frequently born with maxillary hypoplasia in conjunction with micrognathia and associated cleft lip/palate anomalies. These abnormalities severely restrict proper feeding, impair normal speech and language development, and contribute to lifethreatening breathing problems. Surgical management options include mandibular osteotomy and distraction osteogenesis to correct the maxillofacial defects. Operative complications include damage to surrounding nerves and vessels, entry to the skull base, and recurrent temporomandibular joint (TMJ) ankylosis. Case Report: Nine-year-old girl with Nager syndrome presents for recurrent bilateral TMJ ankylosis. SuRgical PlannerTM surgical rehearsal platform (SRP) was used in conjunction with Brainlab intraoperative computed tomography (CT) navigation system to decrease operative time, enhance visualization of key anatomical landmarks and extent of dissection, and minimize intraoperative risks and complications. Conclusion: SRP and Brainlab intraoperative CT navigation system first developed for neurosurgery have been successfully applied to craniofacial procedures. This case highlighted the synergistic benefit of SRP and Brainlab image guidance software to enhance a surgeon's ability to increase operative efficiency, minimize surgical complications, and improve overall patient outcome.

ABSTRACT OBJECTIVES: In this article, the authors address the historical evolution of surgical simulation, focusing on its application in Neurosurgery. METHODS: Literature search was done in the PubMed/Medline and Scopus databases, using the terms "history AND simulation"; and "simulation AND neurosurgery". There was no limit for the publication date. RESULTS: We selected 30 articles whose contents included data of interest for the objective of the study. Simulation has been used for centuries in a variety of ways, including the dissection of corpses by the early physicians (such as Galeno) and military training (for example in war games). Anatomical models were created in the eighteenth century and continued to improve in the following centuries. Virtual reality simulation was first used in 1987 and became popular in the early 1990s. Subsequently, synthetic anatomical models were created that reproduce surgical scenarios close to the real one, with great applicability today. CONCLUSIONS: This literature review highlighted evolutionary aspects of simulation and its current application in medical education. Innovations in this field were highly appreciated by members of the neurosurgical community who recognized the vast potential of simulation to revolutionize this specialty, where intraoperative errors can have disastrous consequences. This historical review may contribute to a better understanding of the relevant role of simulation and for its implementation in the medical curriculum, especially in high complexity specialties such as Neurosurgery.
