The article defines the necessity to change the form of medical training according to the requirements of Federal State Educational Standards of Higher Education and the conditions of digitalization in society and the health care system. Information technology that allows solving topical problems in education due to the current circumstances is analyzed. The perspective and lack of research on issues of information technology usage within the system of higher medical education are emphasized. A variety of organizational and technical capabilities has been determined, their achievement being possible due to the increase in the intellectual activity of university lecturers and students based on using information technology. The experience of medical specialties training in the simulation centre of Saratov State Medical University named after V.I. Razumovsky is observed. The results of subjective assessment of student satisfaction are presented. The conclusions on the advantages of training in a simulation centre aligned with the general idea of information technology usage in the process of medical training are made. The article emphasizes the importance of the information literacy of a physician, as well as their ability to work in the context of the healthcare system digitalization. The need for changing the direction of physicians' training, adapting forms and methods of education to the modern requirements is determined. The pedagogical conditions for the functioning of simulation centers with hardware and software are noted, among which is the consideration of the simulator's capabilities to simulate practically significant clinical situations, preparatory organizational work, innovative simulation technology.

Виртуальные пациенты позволяют избежать рисков в отношении реального больного, неоднократно использовать стандартизованные клинические ситуации, обеспечить удаленный доступ к информации. Для создания виртуальных пациентов в команду проекта включены специалисты с компетенциями в разных предметных областях. Каждый виртуальный пациент является структурированной моделью лечебнодиагностического процесса реального больного, наполненной текстовой и мультимедийной информацией. Сформирована выборка из 50 архивных историй болезни пациентов с типичными сердечно-сосудистыми заболеваниями и редкими вариантами патологии. Текстовая информация из историй болезни дополнена мультимедийными результатами инструментальных и лабораторных исследований. Созданная база данных и знаний о виртуальных пациентах предназначена для демонстрации обучающимся завершенных случаев кардиоваскулярных заболеваний в линейной траектории с возможностью Web-доступа. Репозиторий виртуальных пациентов станет фактологической основой проблемно ориентированного дистанционного обучения студентов и врачей.Ключевые слова: виртуальные пациенты, case-технологии, анализ прецедента, дистанционное обучение, стандартизация, мультимедиа, Web-сервис, геймификация, репозиторий, база данных, база знаний.Конфликт интересов. Background Teaching medicine is a complex task because medical teachers are also involved in clinical practice and research and the availability of cases with rare diseases is very restricted. Automatic creation of virtual patient cases would be a great benefit, saving time and providing a wider choice of virtual patient cases for student training. Objective This study explored whether the medical literature provides usable quantifiable information on rare diseases. The study implemented a computerized method that simulates basic clinical patient cases utilizing probabilities of symptom occurrence for a disease. Methods Medical literature was searched for suitable rare diseases and the required information on the respective probabilities of specific symptoms. We developed a statistical script that delivers basic virtual patient cases with random symptom complexes generated by Bernoulli experiments, according to probabilities reported in the literature. The number of runs and thus the number of patient cases generated are arbitrary. Results We illustrated the function of our generator with the exemplary diagnosis “brain abscess” with the related symptoms “headache, mental status change, focal neurologic deficit, fever, seizure, nausea and vomiting, nuchal rigidity, and papilledema” and the respective probabilities from the literature. With a growing number of repetitions of the Bernoulli experiment, the relative frequencies of occurrence increasingly converged with the probabilities from the literature. For example, the relative frequency for headache after 10.000 repetitions was 0.7267 and, after rounding, equaled the mean value of the probability range of 0.73 reported in the literature. The same applied to the other symptoms. Conclusions The medical literature provides specific information on characteristics of rare diseases that can be transferred to probabilities. The results of our computerized method suggest that automated creation of virtual patient cases based on these probabilities is possible. With additional information provided in the literature, an extension of the generator can be implemented in further research.
