The relevance of this work is determined by the fact that due to the increasing demands for professional development of specialists it is necessary to apply such approaches to teaching so that students of all learning levels could fully understand the material. In order to create optimal learning conditions for students the use of a differentiated approach to the educational process is presented in the paper. For its realization the introduction of level differentiation and differentiated module-rating system was suggested. In this paper we define the conditions for the implementation of level differentiation, describe the developed training complex containing multilevel tasks, variants of control tests with a choice of difficulty levels. To assist the students to choose the difficulty level a web application was designed. Differentiated module-rating system is a modification of the rating system. It is distinguished by the method of counting ranking: at the end of the semester the student has a range of individual private ratings considered in the exam. Differentiated module-rating system allows them to get an idea of the level of learning the material under study and make the necessary adjustments. The introduction of a differentiated approach into the educational process helps to individualize the learning process of students, contributes to a more lasting and deeper acquiring of knowledge, the formation of positive motivation to learning, creation of a comfortable learning environment, building students’ self-esteem and making them more autonomous
Aim To reveal a relationship between preprocedural laboratory data and adverse cardiac outcomes (CO) in patients with stable ischemic heart disease (IHD) following elective endovascular revascularization (ER).Material and methods This study included 225 patients with IHD admitted for treatment to the Research Institute of Cardiology of the Tomsk National Research Medical Center. The study included patients with documented IHD and hemodynamically significant coronary stenoses requiring elective ER. Patients were divided into groups based on the presence of complications: group 1, 98 patients with adverse CO and group 2, 127 patients without adverse CO. Besides evaluation of complaints, history, and objective status, general clinical and biochemical tests were performed for all patients. Concentration of glycated hemoglobin (НbА1с) was measured by immunoturbidimetry (DiaSys Diagnostic Systems). Serum concentrations of insulin, interleukin-6 (IL-6), endothelin 1 (ET-1), and homocysteine were measured by enzyme immunoassay. Blood lipid profile was determined by enzymatic colorimetry (DiaSys). Content of non-high-density lipoprotein (non-HDL) cholesterol (CS) was calculated as: CS – HDL CS. Insulin resistance (IR) was assessed by the HOMА-IR index. IR was diagnosed at the index of 2.77. Statistical analyses were performed with Statistica 10.0 and Medcalc 19.2.6 software.Results A one-way regression analysis identified predictors for adverse CO following ER. The most significant predictors were fibrinogen (odds ratio (OR), 1.430; 95 % confidence interval (CI), 1.027–1.990), HbA1c (OR 1.825; 95 % CI, 1.283–2.598), homocysteine (OR, 1.555; 95 % CI, 1.348–1.794), ET-1 (OR, 94.408; 95 % CI, 16.762–531.720), triglycerides (TG)/glucose ratio (OR 1.815; 95 % CI, 1.155–2.853). Based on selected factors, logistic regression models were constructed. However, not all models had a high prognostic power. Only concentrations of ET-1 and homocysteine showed a high prognostic capability in respect of the adverse outcome (88.3 and 85.7 %, respectively).Conclusion For patients with IHD, the prognostic capability of ET-1 and homocysteine with respect of the risk for adverse CO following ER was the highest compared to other markers. The results of the study are completely consistent with data of literature and can be successfully used in clinical practice for optimizing the medical care of patients after elective ER.

Разработка информационной основы для создания и реализации мультимедийных моделей диагностики и лечения деперсонализированных случаев сердечно-сосудистых заболеваний (виртуальных пациентов). Материал и методы. Материалами исследования служили тексты завершенных историй болезни и мультимедийные результаты инструментальной диагностики пятидесяти пациентов, прошедших стационарное лечение. После экспертной оценки полноты и непротиворечивости всей информации о завершенном случае заболевания, она была размещена на сервере. Для обеспечения удаленного доступа к контенту использованы технологии Web-программирования. Результаты. Создана информационная модель лечебно-диагностического процесса, т. е. совокупность сведений о его динамике в текстовом и мультимедийном форматах. Реализован интерфейс виртуального пациента в виде закладок, связанных с появлением новой информации о состоянии пациента, влияющей на принятие врачебных решений. Приведена структура текстовой и мультимедийной информации на закладках разного типа. Реализованный вариант технологии виртуальных пациентов обеспечивает Webдоступ к демонстрации лечебно-диагностического процесса деперсонализированных случаев. Заключение. В ходе выполнения проекта создается репозиторий виртуальных пациентов, как мультимедийной модели диа гностики и лечения кардиоваскулярной патологии, обеспеченной Webдоступом. Репозиторий будет использован для повышения клинико-диагностических компетенций врачей путем демонстрации и детального разбора завершенных случаев заболевания. Виртуальные пациенты могут стать одной из методических основ дистанционного повышения клинической квалификации врачей в системе непрерывного медицинского образования.Ключевые слова: непрерывное медицинское образование, дистанционное обучение, case-технологии, проблемно-ориентированное обучение, клинико-диагностические задачи, репозитории. Rating control of students knowledge and skills is the part of Bologna declaration of European Universitites. Optimal realization of rating system brings together the objectivity and the individual approach to student’s evaluation. Analysis of rating control possibilities in medical University and peculiarities of individual rating adaptation in the LMS Moodle are the objectives of this paper.LMS Moodle contains enough elements quantity for evaluation and allows sum up of learning course results. First level of rating system adaptation is various intervals for different course elements. Weight coefficients for learning tasks mean second one. Third rating adaptation level is the method of calculation of final rating score for a student. There is example of rating system in medical informatics course for medical and pediatric students in the paper. Organizational, technological and financial aspects of rating introducing in the medical education are discussed.
Виртуальные пациенты позволяют избежать рисков в отношении реального больного, неоднократно использовать стандартизованные клинические ситуации, обеспечить удаленный доступ к информации. Для создания виртуальных пациентов в команду проекта включены специалисты с компетенциями в разных предметных областях. Каждый виртуальный пациент является структурированной моделью лечебнодиагностического процесса реального больного, наполненной текстовой и мультимедийной информацией. Сформирована выборка из 50 архивных историй болезни пациентов с типичными сердечно-сосудистыми заболеваниями и редкими вариантами патологии. Текстовая информация из историй болезни дополнена мультимедийными результатами инструментальных и лабораторных исследований. Созданная база данных и знаний о виртуальных пациентах предназначена для демонстрации обучающимся завершенных случаев кардиоваскулярных заболеваний в линейной траектории с возможностью Web-доступа.
