BackgroundAggression and Violence against primary care physicians is reportedly common in Pakistan but there is no any documented study to-date on this burning issue.MethodsA formed written questionnaire was distributed among 769 primary care physicians aged 31 ± 7.68 years. Apart from the demographic data, the questionnaire included questions regarding the level of safety that primary care physicians felt during their work setups and on-call duties, along with the experience of aggression against them by the perpetrators & the support provided by the hospital management in such cases.ResultResponse rate was 68% i.e. 524 physicians agreed to participate in the study. It was found that majority (85%) of the physicians has faced mild events, 62% have faced moderate events and roughly 38% were subjected to severe violence. Some physicians revealed more than one form of aggression being faced by them in 12 months preceding months which makes the collective percentage greater than 100%. Verbal abuse is the most frequent type of mistreatment faced by the doctors from the patients or their attendants.ConclusionA considerable number of physicians participated have faced mild violence in which verbal abuse was commonest; followed by moderate and severe events.

Özet Amaç: Şiddet toplumun genelinde olduğu gibi sağlık çalışanları için de önemli bir sorundur. Bu çalışmada sağlık çalışanlarının şiddet algısının, şiddete maruz kalma sıklıklarının ve şiddetten etkilenme durumlarının değerlendirilmesi amaçlanmıştır. Gereç ve yöntem: Denizli ili merkez sağlık kuruluşlarında görev yapan ve çalışmaya katılmayı kabul eden 965 kişi çalışmaya dahil edilmiştir. Araştırmacılar tarafından literatüre dayanarak oluşturulan anket ile şiddetin tanımı, şiddetin olası nedenleri, şiddetin haklı nedenleri, şiddeti önleyebilecek etkenler, şiddete maruz kalma ve şiddetten etkilenme durumları değerlendirilmiştir. Sonuçlar meslek ve cinsiyet dağılımlarına göre incelenmiştir. Bulgular: Şiddet tüm meslek gruplarında ve her iki cinsiyette en sık fiziksel ve cinsel olarak tanımlanmıştır. Sağlık çalışanlarının %39,5'inin mesleki yaşantısında en az bir kez şiddete maruz kaldığı saptanmıştır. Şiddetin herhangi bir türüne maruz kalanlarla kalmayanlar arasında meslek dağılımları açısından anlamlı farklılık saptanmıştır. Sonuç: Çalışma sağlık çalışanlarına yönelik şiddetin fazlalığını, olumsuz etkilenme durumlarını ortaya koymuştur. Bunun yanında şiddeti tanıma ve önleyici tedbirlerle ilgili sağlık çalışanlarının farkındalıklarını da göstermiştir. Şiddeti önleyebilmek için şiddetin tanınması yanında risk faktörlerinin belirlenmesi ve önleyici etmenlere ilişkin daha fazla çalışma yapılması gerekmektedir.

Tideglusib®, a GSK-3 inhibitor, was initially tested for the treatment of Alzheimer’s disease. However, a recent report has suggested its potential off-label use for the treatment of dental cavities. Even if this effect is not yet confirmed, this off-label use can have significant public/dental health consequences, mainly because of the large number of patients with cavities. The purpose of this mini-review is to perform an ethical analysis of the use of Tideglusib in dentistry. The ethical analysis identified three main areas in which ethical breaches could be significant: 1) respect for the autonomy of the patient, 2) issues raised by horizontal shifts in the translational research process, and 3) the conflict between dental beneficence and general non-maleficence. In conclusion, the use of Tideglusib in dentistry should respect the same strict ethical and regulatory criteria from clinical medicine. A translation of the potential risks should be done only after large-scale, phase-III/IV clinical trials, explicitly designed to test the usefulness of this drug in dental medicine.
