Ventricular Tachycardia and Right Ventricular Fibrosis After Tetralogy of Fallot Surgical Repair

Abstract: V entricular tachyarrhythmias are known sequelae among adults with repaired tetralogy of Fallot. The incidence of sustained ventricular tachycardia and sudden cardiac death in patients 35 years after corrective surgery is estimated at 11.9% and 8.3%, respectively.1 Studies have shown that right ventricular enlargement resulting from chronic pulmonic regurgitation is the most common hemodynamic substrate.2 QRS prolongation (>180 milliseconds) is one of the most sensitive ECG predictors and in turn correlates wi… Show more

“…197,[200][201][202][203][204] Focal origin VTs, possibly due to microreentry, have also been reported. 205 In patients who have had sustained VT, multiple morphologies of inducible VT are common and can be due to use of the same isthmus in different directions, or to VTs using different isthmuses. 197 VT is occasionally related to a scar in an LV location or in other diseased RV myocardium.…”

“…Similarly, in a study from Olmsted County that examined participants with HFpEF, the presence of concurrent AF (diagnosed within 30 days of HF) showed a trend towards poor survival but was not significant in an age- and sex-adjusted model compared with sinus rhythm patients (P=0.11). 4 This study did not directly compare mortality among participants with concurrent versus prevalent AF. Interestingly, individuals with prior AF had larger left atrial volumes compared with those with concurrent AF (55 versus 47 ml/m 2 , respectively, P<0.05).…”

“…Ventricular tachyarrhythmias are known sequelae of this condition. The incidence of sustained ventricular tachycardia and sudden cardiac death in patients 35 years after corrective surgery is 11.9 and 8.3% respectively ( 20 , 21 ). The second patient had very severe hypokalemia (1.5 mmol/L) and cardiogenic shock from a massive pulmonary embolism.…”
