Prior to the COVID-19, various issues in ethics, laws, regulations and security risks limited physicians in their ability to deliver medical care via telemedicine. However, in 2020, telemedicine quickly became mainstream when the US government issued a waiver granting the use of telemedicine to reduce COVID-19 transmission. This study examined the US physicians’ acceptance of telemedicine after its sudden wide-scale implementation. The authors developed a survey based on the Unified Theory of Acceptance and Use of Technology to assess the US physicians’ intentions and attitudes toward the use of telemedicine in the delivery of medical care. This survey was distributed between March and June in 2021 across several voluntary online physician groups. The survey was validated using factor analysis. The 110 participants who completed this study were all active physicians practising telemedicine in the USA. The majority of the participants were female (93%), between 34 and 59 years of age (93%), had at least 10 years of medical experience (84%) and practised in a city (92%). The behavioural intention to use telemedicine was influenced by performance expectancy ( β = 0.34, p<.001), social influence ( β = 0.34, p<.001), facilitating conditions ( β = 0.14, p<.05) and attitude towards telemedicine use ( β = 0.36, p<.001). Attitude towards telemedicine use was influenced by performance expectancy ( β = 0.60, p<.001), effort expectancy ( β = 0.16, p<.05) and social influence ( β = 0.16, p<.05). In addition, telemedicine experience moderated the influence of performance expectancy on behavioural intention negatively ( β =−0.39, p<.001) and moderated the influence of facilitating conditions on behavioural intention positively( β = 0.27, p<.001). This study found that the physicians’ performance expectancy of and attitude towards telemedicine use were significant predictors of physicians’ acceptance. Social influence from colleagues and administrators also played an essential role in impacting physicians’ acceptance of telemedicine. Physicians’ effort expectancy merely affected their attitude towards telemedicine use. Facilitating conditions—such as the inability to perform physical exams, labs, etc.—only influenced the physicians’ intention to use telemedicine. Finally, the survey data suggested that as physicians gained telemedicine experience, their performance expectancy predicted lower behavioural intention than counterparts with less experience.

The Covid-19 pandemic has accelerated the adoption of technology in various sectors, one of which is the healthcare industry. Telemedicine users increased during the Covid-19 pandemic, but only 10% of Indonesia's population. This study aims to analyze the factors influencing the intention to use telemedicine. This research uses a descriptive quantitative method. The sampling technique used non-probability sampling with a voluntary sampling technique. Data analysis applied Structural Equation Modeling using LISREL version 8.8. Data were obtained from 225 respondents in Greater Jakarta and Greater Bandung from January to March 2022, but only 192 were included in the analysis. The results showed that the intention to use telemedicine was directly influenced by attitude (A) and indirectly influenced by interrelated variables such as trust (T), perceived ease of use (PEU), perceived usefulness (PU), information quality (IQ), service quality (SrQ), and system quality (SQ). Implications that telemedicine service providers can apply to increase the use of telemedicine are to create the best experience, user friendly, provide complete information, and increase the reliability of information systems.

To date no treatment protocol of proven efficacy was approved to manage COVID-19. We have developed a novel COVID-19 protocol basing on our early pioneering article that justified repurposing of nitazoxanide/azithromycin combination for early cases of COVID-19 which was followed by two articles to justify addition of non-steroidal anti-inflammatory drugs to nitazoxanide/azithromycin. We are presenting a prospective telemedicine case series involving consented 32 patients/legal guardians including 10 confirmed by PCR and 21 highly suspected COVID-19 Egyptian patients. The patients included 15 adult males, 11 adult females, 2 pregnant patients as well as 4 children. All patients have received a short 5-day-regimen of NSAIDs (diclofenac potassium in most of cases, ibuprofen, lornoxicam, celecoxib, ketoprofen or ketorolac)/nitazoxanide/azithromycin +/- cefoperazone either in full or in part as illustrated in the manuscript. The primary endpoint of this protocol was full relief of all serious COVID-19 clinical manifestations. The primary endpoint was fully achieved in all patients within two weeks. Most of the patients treated early with the protocol have fully recovered during its described five days; the leucocytic/lymphocytic count was significantly improved for those with prior leucopenia/lymphopenia. No significant adverse effects were reported. A novel 5-day-protocol to safely and effectively cure COVID-19 using repurposed inexpensive FDA approved drugs is illustrated and we recommend performing sufficiently powered double blind randomized clinical trials against any current standard protocol.
