AbstrActobjective: To study the reliability of the portuguese version of a questionnaire for psychologically and psychosocially diagnosing individuals with temporomandibular disorders (RDC/TMD). Method: Interviews were held with 109 individuals of both sexes who required care at the physical therapy clinic of the Araraquara university Center (uNIARA) from January to July 2006. The questionnaires were applied by a single examiner. Two weeks later, the same questionnaire was reapplied to 36 individuals. To evaluate the internal consistency of the method, Cronbach's alpha coefficient was used. The intra-examiner reproducibility was analyzed using the intraclass correlation coefficient (ρ) for quantitative variables and Kappa (κ) statistics for qualitative variables. Results: The internal consistency for the dimensions of chronic pain intensity, disability, limitations relating to mandibular function, nonspecific physical symptoms with pain items included, and nonspecific physical symptoms with pain items and depression excluded, were 0.8479, 0.8971, 0.8673, 0.8080 and 0.9270 respectively, thus confirming the excellent internal validity of the method. The intra-observer agreement was found to be "excellent" for questions relating to pain duration and intensity and "good" for the question relating to current pain. The lowest κ values were associated with items relating to physical symptoms and depression. The subjects' perception of clicking and creaking also had "satisfactory" agreement, as did the question on seeking professionals for pain treatment. The remaining questions showed "good" and "very good" reproducibility and most of them presented the maximum agreement. Conclusion: The portuguese adaptation of the questionnaire was shown to be reliable for detecting psychological and psychosocial abnormalities relating to temporomandibular disorders. ) e a estatística Kappa (κ), respectivamente às variáveis de natureza quantitativa e qualitativa. Resultados: A consistência interna para as dimensões intensidade da dor crônica e incapacidade; limitação da função mandibular; sintomas físicos não-específicos, incluindo os itens de dor; sintomas físicos não-específicos, excluindo os itens de dor e depressão foi de 0,8479, 0,8971, 0,8673, 0,8080 e 0,9270 respectivamente, atestando ao método excelente validade interna. obteve-se "excelente" concordância intra-examinador para as questões referentes ao tempo de presença da dor e sua gradação, e "boa" para a questão referente à dor presente. Os menores valores de κ relacionaram-se aos itens de sintomas físicos e depressão. A percepção de estalos ou rangidos pelos indivíduos apresentou concordância "regular" bem como a questão referente à procura de profissional para tratamento da dor. As demais questões apresentaram reprodutibilidade "boa" e "ótima", sendo que a maioria dessas apresentou nível máximo de concordância. Conclusão: A versão adaptada para o português mostrou-se confiável para detecção das alterações psicológicas e psicossociais associadas às desordens temporomandib...

Avaliação da relação entre disfunção temporomandibular e dimensão vertical de oclusão em crianças de 7 A 12 anos

Objetivo verificar se existe relação entre a presença de disfunção temporomandibular (DTM) relacionada à variabilidade da dimensão vertical de oclusão em crianças e adolescentes na faixa etária de 7 a 12 anos. Métodos foram avaliadas 96 crianças e adolescentes do Instituto Rogacionista em São Paulo. A pesquisa diagnóstica da disfunção temporomandibular foi por meio do Índice de Helkimo e exame clínico posterior. Para a mensuração dos dados referentes à dimensão vertical foram empregadas as distâncias comissura labial - canto externo do olho e base do nariz – mento. A comparação dos valores médios das medidas antropométricas entre os gêneros e os grupos com e sem disfunção temporomandibular foi realizada empregando-se a análise de variância (ANOVA) complementada pelo teste least significance diference. Resultados a dimensão vertical de oclusão demonstrou medidas distintas nas crianças e adolescentes em todas as faixas etárias avaliadas, foram observadas alterações significantes dessa medida nas idades de 10 e 12 anos de ambos os gêneros. Conclusão pode -se concluir que na amostra estudada houve relação direta entre a presença de DTM e Dimensão vertical de oclusão (DVO), correlação positiva entre as medidas comissura labial – canto do olho externo e Násio – Mento no sexo feminino e alterações significantes na dimensão vertical de oclusão nas idades de 10 e 12 anos para ambos os sexos.

The physiotherapy approach aims at the pain relief and the functional recovery by means of several modalities. Objective: To investigate the effects, short and medium-term, of a multimodal physiotherapeutic approach on TMD diagnosis and severity. Methodology: Individuals with diagnosis of TMD, confirmed by the Axis I of the RDC/TMD, took part in the study. From this evaluation, the Temporomandibular Index (TMI) and its sub-indices were calculated. The participants were treated during ten sessions of physiotherapy, which included therapeutic ultrasound, thermotherapy, manual therapy, stretching and neuromuscular exercises, as well as self-care and home exercises instructions. Assessments were carried out before treatment (AV1), immediately after treatmentFisioter Mov. 2014 abr/jun;27(2):219-27Freire AB, De Nardi AT, Boufleur J, Chiodelli L, Pasinato F, Corrêa ECR. 220 (AV2) and two months after the end of the treatment (AV3). The values of indices and the diagnosis prevalence were compared between the different periods by the t paired test (p < 0.05). Results: The number of diagnoses reduced in all the subgroups and 41.7% of the 24 participants presented no diagnosis after the treatment. A significant decrease in the TMI was observed between AV1 and AV2 (p = 0.000). There was no difference between AV2 and AV3 (p = 0.204) in 13 participants assessed two months after the end of the treatment. Conclusion: The multimodal physiotherapeutic approach resulted in positive effects, short and medium-term, on the symptoms and clinical signs, with deletion of the dysfunction or reduction of its severity in treated patients.[
