Uso De Cateter Vesical De Demora Entre Pacientes Internados Na Clínica Cirúrgica De Um Hospital Universitário

Abstract

OBJECTIVE: Evaluate the use of bladder catheter in hospitalized patients in the surgical clinic of a university hospital, emphasizing its indications and usage time. METHODS: It is a retrospective, observational, transversal and descriptive study, through the collection of data extracted from the electronic medical records of catheterized inpatients in the surgical clinic of the University Hospital of the Federal University of Piauí between July and November 2016. RESULTS: Among 771 inpatients, the prevalence of bladder catheterization was 14.27% -110 patients, with mean age of 56.99 years and higher proportion of females (55.5%). Over 90% of catheterized patients were underwent some surgery. General Surgery and Urology were the specialties with the higher number of catheterized patients. The most frequent site of the first passage of the bladder catheter was the surgical center. The most common indications for indwelling urinary catheter were done due to perioperative/postoperative diuresis monitoring in specialized surgery and to hemodynamically unstable or critical ill patients. One-third of the patients made excessive use of this device, but without a statistically significant difference between those and the group of those who used it for an adequate period of time, considering the variables selected. CONCLUSION: The reality of University Hospital surgical clinic of the Federal University of Piauí is similar to that reported in the literature in which significant proportion of patients use the bladder catheter excessively. It's needed a more meticulous use and a more efficient follow-up for indwelling urinary catheter. KEYWORDS: Indwelling urinary catheter. Physiologic monitoring. Continuity of patient care. RESUMOOBJETIVO: Avaliar o emprego do cateter vesical de demora em pacientes internados na clínica cirúrgica de um hospital universitário, destacando-se suas indicações e tempo de uso. MÉTODOS: É um estudo retrospectivo, observacional, transversal e descritivo, através de coleta de dados extraídos do prontuário médico eletrônico de pacientes internados e cateterizados na clínica cirúrgica do Hospital Universitário da Universidade Federal do Piauí (HU -UFPI) entre julho e novembro de 2016. RESULTADOS: Entre 771 pacientes internados, a prevalência de cateterização vesical foi de 14,27% -110 indivíduos, com idade média de 56,99 anos e maior proporção do gênero feminino (55,5%). Mais de 90% dos pacientes cateterizados foram submetidos a alguma cirurgia. Cirurgia Geral e Urologia foram as especialidades com maior número de cateterizados. O local mais frequente da primeira passagem do cateter vesical foi o centro cirúrgico e suas indicações mais comuns foram monitoramento da diurese em perioperatório/pós-operatório de cirurgia especializada e em paciente hemodinamicamente instável ou crítico.
