Unusual Forms of Adrenal and Extra-Adrenal Myelolipomas

Abstract: Myelolipomas are rare benign tumors of poorly understood tumorigenesis composed of mature hematopoietic tissue and fat. They mostly occur in the adrenal glands, but extra-adrenal myelolipomas have been reported in other locations such as the presacral region or retroperitoneum. It is not unusual that they are incidental findings revealed in the study of different diseases. We report 3 unusual examples of myelolipomas. The first is a multiple, unusually large, extra-adrenal myelolipoma, presented as an autopsy … Show more

“…Others suggest that a preoperative biopsy should be considered, as the pathologic results have the ability to strongly impact management, particularly for lesions that are large or invade adjacent structures. 11 Concurrent presentation of myelolipomas with other neoplasms (adrenal adenoma, focal nodular hyperplasia, non-Hodgkin lymphoma) are reported in literature. 12 An incidental finding in our patient was 2 spindle cell tumors on the gastric surface.…”

“…They may coexist with hormone-secreting adrenal masses, but AMLs are always hormonally inactive [2]. Theories about their pathogenesis include reticuloendothelial cell metaplasia of adrenal capillaries, emboli from bone marrow and adrenal embryonic remnants of hematopoietic elements, while there is a report of myelolipoma expressing (3;21) (q25;p11) translocation which indicates a neoplastic process of hematologic origin [5].…”

“…Muchos cirujanos consideran que la biopsia es una contraindicación en cualquier lesión que sea resecable, debido a los riesgos de siembra en el tracto de la aguja de biopsia con células potencialmente malignas, o de infectar la lesión. Otros sugieren que se debe considerar una biopsia preoperatoria, ya que los resultados patológicos intervienen en el manejo, particularmente para las lesiones que son grandes o que invaden las estructuras adyacentes 8 . No existe un tratamiento estándar para este tipo de tumores.…”
